Post ‐TAVR conduction abnormalities leading to permanent pacemaker implantation: Risk factors, prevention, and management
AbstractTranscatheter aortic valve replacement (TAVR) often leads to conduction abnormalities, necessitating pacemaker implantation. This review of 38 meta-analyses identified preexisting right bundle branch block  (RBBB), LAHB, and new-onset left bundle branch block as key risk factors, with a higher PPM risk in male and older patients. Procedural factors like transfemoral access and self-expandable valves also increase this risk. Prevention focuses on tailoring TAVR to individual electrophysiological and anatomical profiles. However, there's a lack of consensus in managing these conduction disturbances post-TAVR, highl...

Long ‐term outcomes of cardioneuroablation with and without extra‐cardiac vagal stimulation confirmation in severe cardioinhibitory neurocardiogenic syncope
ConclusionThis study demonstrates that concluding CNA with denervation confirmation via ECVS yields a higher success rate and a substantially reduced risk of syncope recurrence compared to procedures without ECVS confirmation. (Source: Journal of Cardiovascular Electrophysiology)

Posttransplant atrial fibrillation: Evidence of early posttransplant recipient ‐to‐donor atrioatrial connections
ConclusionAtrio-atrial connection between recipient and donor can form as early as a few months post heart transplant. Radiofrequency ablation appears to be an effective treatment for atrial fibrillation propagated through donor-to-recipient connections. (Source: Journal of Cardiovascular Electrophysiology)

Acute epicardial pulmonary vein reconnection: Nondurable transmural lesion or late manifestation of conduction through intercaval bundle
ConclusionsEpicardial reconduction accounted for the majority of acute PVR. AEPVR was associated with anatomic characteristics and multiple ablation-related parameters, which could be explained by nondurable transmural lesion or late manifestation of conduction through intercaval bundle. First clinical data on artificial intelligence ‐guided catheter ablation in long‐standing persistent atrial fibrillation
ConclusionDISPERS-guided ablation using machine learning software (the Volta VX1 software) in addition to PVI in long-standing persistent AF ablation resulted in high long-term success rates regarding AF and AT elimination. Most arrhythmia recurrences were reentrant AT. After a total of 1.46  ± 0.68 procedures, freedom from AF/AT was 82%. Despite prolonged procedure times complication rates were low. Randomized studies are necessary to evaluate long-term efficacy of dispersion-guided ablation using AI. (Source: Journal of Cardiovascular Electrophysiology)

Predictors of the right ventricular perforation caused by active ‐fixation pacing and defibrillator leads: A single‐centre experience

A systematic review and meta ‐analysis of new‐onset atrial fibrillation in the context of COVID‐19 infection
AbstractNew-onset atrial fibrillation (NOAF) in COVID-19 raises significant clinical and public health issues. This systematic review and meta-analysis aims to compile and analyze the current literature on NOAF in COVID-19 and give a more comprehensive understanding of the prevalence and outcomes of NOAF in COVID-19. A comprehensive literature search was carried out using several databases. The random effect model using inverse variance method and DerSimonian and Laird estimator of Tua2 was used to calculate the pooled prevalence and associated 95% confidence interval (CI). Results for outcome analysis were presented as od...

High ‐density mapping of Koch's triangle during sinus rhythm and typical atrioventricular nodal re‐entrant tachycardia, integrated with direct recording of atrio‐ventricular node structure potential
ConclusionDetailed recording of the potentials of nodal structures is possible by means of multipolar catheters for ultrahigh-density mapping, allowing 100% of the AVNRT CL to be covered.
