Noninvasive respiratory support in the perioperative periodemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Purpose of review: The aims of this review are: to discuss the physiological rationale for noninvasive respiratory support (NRS) in the perioperative period; to give some practical recommendations to safely apply NRS; and to review available clinical data on preventive and curative NRS after cardiac, thoracic and abdominal surgery. Recent findings: The most important morbid postoperative pulmonary complication is atelectasis formation, which increases significantly the risk for pneumonia and hypoxic acute respiratory failure. NRS refers to techniques allowing respiratory support without the need for an invasive airway. Tw...

Heralding what they hope is a new era of personalized genomic medicine, experts in the US have identified the gene behind a patient's inherited neurological disorder, in this case a form of Charcot-Marie-Tooth disease, by sequencing his complete genome. Long-term effect of the ICU-diary concept on quality of life after critical illnessemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Critically ill patients often spend time in the intensive care unit (ICU) either unconscious or sedated. On recovery, they are often in a state of confusion with memory loss that may be associated with a longstanding reduction in health-related quality of life (QoL). We hypothesised that the ICU-diary concept could improve their QoL by filling in their memory gaps. A non-randomised, prospective study in a non-academic eight-bedded general ICU. A group of patients (n=38) were selected to receive the ICU-diary concept (keeping a diary with photos while on the ICU plus a follow-up meeting) when a long and complicated course w...

After tracheal intubation, anesthesia was titrated using isoflurane in oxygen-enriched air, adjusted to maintain a bispectral index (BIS) value of 46[ndash]54. The pleth variability index (PVI) is a new algorithm used for automatic estimation of respiratory variations in pulse oximeter waveform amplitude, which might predict fluid responsiveness. Because anesthesia-induced hypotension may be partly related to patient volume status, we speculated that pre-anesthesia PVI would be able to identify high-risk patients for significant blood pressure decrease during anesthesia induction. We measured the PVI, heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), and mean arterial pressure (MAP) in 76 adult healthy patients under light sedation with fentanyl to o...

Pain quality may identify subgroups among patients with suspected neuropathic pain responsive to IV lidocaine. Further investigation is warranted to validate and extend these findings. We report a case of abdominal compartment syndrome occurring due to uterine perforation while the patient was undergoing hysteroscopic uterine adhesiolysis for uterine synechia. The cause of the patient’s abdominal compartment syndrome was irrigation fluid moving from the uterine cavity into the peritoneal space via defects in the uterus. Anesthesiologists must be alert for these complications during hysteroscopy procedures. [Rhabdomyolysis and compartment syndrome of two forearms after robotic assisted prolonged surgery.]email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
We report the case of an obese 30-year-old female patient who underwent a 12-hour duration robotic laparoscopic surgery for severe endometriosis, in Trendelenburg position. This was complicated by a two forearms rhabdomyolysis, with subsequent compartment syndrome with multiple neuropathy. Physicians must be aware of the cumulative risk for postural complications when extreme positions are associated to long duration procedures in predisposed patients.
