OHE trouble
This is my first time posting on the Veterinary side of the forum, but I feel like my problem is more suited for this one.
Today was my first OHE ever (it was on a cadaver dog).
I've been in the surgery suite during dozens of them, but have always been running anesthesia and never really saw what was happening at the level of the incision.

SRNA observation
I spoke to a SRNA the other day and was amazed at what I perceived as her extreme naivete. She goes to a private college with tuition up to 50k per year. I made a joke about joining the military and having no loans. She then said that she "doesn't think the military can feed her" and that she's glad that she can pay all her loans with "the first year as a practicing CRNA".
I'm familiar with med students and false salary expectations, but I almost laugh at her when I heard this from a SRNA.
Maybe, I'm wrong and CRNAs are starting off at 300k per year. I do know I'm not going hungry with the military....

ITE Study Plan
Anyone using ACE questions to study? Taken 3 tests and am still missing ~40 questions! So hard to keep going. ITE is coming up soon, and haven't figured out a good study plan from now till test day in March. Plan to reread sections of M&M and do as many ACE questions as possible.

Letter of intent
Hi all,
I am a MS4 and I have narrowed my top choice to two very different anesthesia programs. I am leaning towards one but I want to do a second look before making a commitment to number 1. Unfortunately, I can't go until Feb 18, two days before rank lists are due.
Can anyone comment if this is too late to make an effective letter of intent to a program? Are their rank lists done and filed by that point? My home PD has been a great advocate and I think will help make a call on my behalf, but I just worry that I need to act now if I want a program to know that they are my number 1.

Ophthalmic Anesthesia
I'm currently on an Ophthalmology rotation and I was wondering about the anesthesia used for their procedures. I've mostly seen them just use topical anesthesia, but I was wondering if there are any procedures that require the presence of an anesthesiologist.

Private Pilot
While I am a supporter of the second amendment I don't demonstrate my support by shooting/collecting guns as a pass time. Instead for my expensive vice I like flying.

Ok, don't make fun of me for asking this, I've only done 5 months of anesthesia.
What do you guys think is the correlation of MP score and laryngoscopic view. No correlation, little correlation?

What next to do

Roth Conversion, whos doing it?
I'm 40, have a big chunk in an old IRA that needs to be rolled over and can't decide about the conversion. I can afford to pay the tax to convert to Roth but my gut tells me to leave it as as traditional. International Degrees (MBBS) to US
Hi guys,
My parents have had a few friends who've sent their kids to foreign countries to complete their medical education faster than in the US with the intention of having them return to practice here. When they do, it seems like they only get accepted into either internal medicine or anesthesiology. Does anyone have a clear idea why? Some people have said that the USMLE Step 1 really hurt them. Not doing away electives
I'm an MS4 and applied only to anesthesia. I did an anesthesia elective at my home institution but there is no residency program there, so I was only with attendings. I've received quite a few interview invites but I'm a little concerned that not doing away electives at residency programs will hurt my chances of matching. Pharmokinetics question

I'm finishing up a 4 year hitch as a USAF flight surgeon and starting as a CA-1 this summer, so I've been trying to do some reading on my latest and last deployment from Baby Miller. I'm getting stuck on the pharmokinetics of volatile anesthetics and my question is this:
I understand that P(A) equates with P(br) and therefore relates to both induction and recovery from anesthesia. I also understand that uptake of anesthetic into the blood lowers P(A) which slows onset of induction. Why is this though? Don't you want the anesthetic to diffuse from the alveoli into the blood? How else can the anesthetic get to...

I'm new here but was hoping to canvas some advice...
I'm currently a CT1 anaesthetics trainee with a non working wife and two small children. Our lives so far have been spent two years here, four years there e.t.c. with the outcome of leaving small pockets of friends in various locations which are hard to keep in touch with. My wife and I struggle with the frequent moves, but at least I get to go to work. Questions for Shadowing
Hello all and sorry if this has been asked before, but...
I'm going to shadow an anesthesiologist at my mom's hospital on Tuesday and I wanted to come up with some things to ask him. However, I'm not sure how important this actually is. I think what is more important is knowing what to take away from this experience that can be helpful in using towards my application to medical school a couple years down the line.

Anybody know of a good resource/workshop/course for basic TTE? A few of my group members and I are interested in gaining some experience for basic pre-op/post-op evals, like for when people are crapping out or looking wonky for whatever reason and calling cardiology would take too long/be overkill. There was a brief TTE demo at the SCA/ASE echo course in san diego, but I'm looking for something a bit more comprehensive. Sterile probes
I've started doing single shot blocks without any ultrasound probe cover. Gel on probe. Get a good picture. Hold probe in place. Prep site of injection with other hand. Grab needle and go. Anyone else not using a probe cover?

SOAP, couples match, and the NRMP--haaalp
hi guys,
so i'm applying in anesthesia, and my fiance is applying in internal.
we've cast a very wide net and have had a fairly-decent response from programs with regard to similar geographic areas.
BUT (and here's a big but) ...we're thinking that, to improve our chances, i should rank advanced positions. and since we can't rank my prelim spots as part of our couples rank list, that means i'm basically guaranteeing myself that if i get that advanced spot, i'm going to have to SOAP into a prelim (either in IM or surg, i'll take anything)
my question is--if we have to SOAP before we get our official match outcome, how ...

Hospital run anesthesia groups
I went to a job fair this past week which I initially thought would be a clever way to meet folks who had contacts with the anesthesia groups at their hospitals. What I wasn't expecting was the number of private practice groups who were now becoming hospital employees.
With the ever changing healthcare climate is this the wave of the future? Should those of us who will be facing the job market soon accept this as the new norm?

Thinking about adding cerebral oximetry to our CV anesthesia practice, and I am interested in any thoughts on the FORE-SIGHT absolute cerebral oximetry system.
Anyone use this system or heard good or bad things about it? Anyone using a different system that they like or don't like?
We used the Invos system in residency and fellowship, but the requirement to obtain a baseline measurement was very problematic since we didn't use it on every case. Sometimes we would wish we had if part way through a case, sometimes a sensor would get dislodged etc.

Anesthesia vs Surgery
I dont mean to rehash this topic, and I've gone through the previous threads and they've been very helpful.
I've just got one tiny question if you guys don't mind. I know there are many Anesthesiologists who contemplated a career in Surgery and I'm wondering if the urge to cut things open and sew them up goes away?
I really hate almost everything about Surgery. I don't like the culture, the hours, the overall crudeness and I really don't like the fact that its more or less GI. On the other hand, I love the culture behind Anesthesia, the rush when things go wrong, the physiology, and I really like the people and the hour...

Hyperthyroidism
I did a preop eval on a 55 year old patient for ORIF Hip. Labs showed undiagnosed hyperthyroidism (Free T4 over 5, TSH less than .03). Recent history of 50 lb weight loss. When should surgery and anesthesia proceed? (Source: Student Doctor Network Forums)

lung isolation and difficult airway
What are folks doing for these? I have changed to a double lumen over a tube changer a few times, also used the bronchial blocker with mixed results. I've found that using a large scope to mainstem a standard tube is easy and fairly effective. (Source: Student Doctor Network Forums)

Dental Anesthesiology Denied Specialty

ADA House does not approve new dental specialty
After almost a year of speculation, the ADA House of Delegates voted on October 22, 2012 to not approve Resolution 16, which called for the recognition of dental anesthesiology as a dental specialty.
The House reiterated its position the following day, when on October 23, delegates voted against a reconsideration.
