Postoperative gastrointestinal (GI) dysfunction is one of the most frequent complications in surgical patients. Most cases are associated with episodes of splanchnic hypoperfusion due to hypovolaemia or cardiac dysfunction. Conclusions
During ischaemia, all anaesthetics provide haemodynamic stability and preservation of LV contractility, whereas propofol and sevoflurane, but not desflurane, caused increased LV diastolic stiffness. Desflurane and sevoflurane provide superior cardioprotection compared with propofol.

Conclusions
These findings confirm the importance of mental workload to the performance of anaesthetists, and suggest that raised mental workload is likely to be a common problem. Although further studies are required, the method described may provide a useful method for the measurement of the mental workload of anaesthetists.

Serum from patients receiving propofol/paravertebral anaesthesia for breast cancer surgery inhibited proliferation, but not migration, of ER-MDA-MB-231 cells in vitro, to a greater extent than that from patients receiving sevoflurane/opioid anaesthesia–analgesia. This implies that anaesthetic technique alters the serum molecular milieu in ways that may affect breast cancer cell function, possibly by altering anaesthetic and opioid drug administration and resultant pain scores.

The actions of the agents in single cells reflect their known influence on human hypoxic ventilatory response, consistent with the notion that the cellular process underlies the whole-body effect. The responses to elevated extracellular potassium, which depolarizes the cell membrane, indicate that (in addition to molecular mechanisms previously proposed), voltage-activated calcium channels may also be involved in the anaesthetic effect.

Conclusions
All commonly used local anaesthetics induce neuronal apoptosis in clinically used concentrations. The neurotoxicity correlates with lipid solubility and thus with the conduction blocking potency of the local anaesthetic, but is independent of the chemical class (ester/amide).

A new review from Sweden finds that diclofenac a medication that works for some adults also relieves acute pain in children. However, a U.S. researcher is not convinced that the review showed the alternate, costlier drug to be an improvement over standard pain-relief drugs, or that it is any safer for children.

Researchers in Germany found that when they treated volunteers with a placebo that they believed to be a painkiller, scans showed reduced signs of pain-related activity in their spinal cords. The study is the work of Dr Falk Eippert and colleagues, from the University Medical Center Hamburg-Eppendorf, one of the largest hospitals in Hamburg, and is published online in the 16 October issue of Science.

We describe two patients with an abnormal spinal anatomy in whom ultrasound-assisted spinal anesthesia was unsuccessful. In patients with coronary stents scheduled for surgery the question arises whether and how antiplatelet therapy should be continued. Risks of perioperative bleeding and of acute stent thrombosis have to be considered simultaneously. The bleeding risk depends primarily on the kind of surgery and on patient comorbidity. Catastrophic interpretations of pain as a signal of impending threat usually elicits fear, and escape or avoidance behavior. According to the fear-avoidance (FA) model these behaviors may initially be adaptive, but they paradoxically worsen the situation when engaged in later on. Avoidance behavior may fuel pain, disability and depression. A particular strength of the FA model is that it generates testable hypotheses about the association amongst its constituent elements. Although correlational studies have accumulated, yet few studies have used prospective designs.

Unexpectedly, inflammation markers do not appear to be involved with the chemopreventative effect of aspirin on colorectal adenomas, according to a brief communication published online October 12 in the Journal of the National Cancer Institute. Aspirin has been shown to prevent the recurrence of colorectal polyps, but it's not clear how it works.

Nycomed has launched an intranasal fentanyl spray (Instanyl) for the management of breakthrough pain in adults already receiving maintenance opioid therapy for chronic cancer pain.
The Summary of Product Characteristics defines patients receiving maintenance opioid therapy as those who are taking at least 60 mg of oral morphine daily, at least 25 micrograms of transdermal fentanyl per hour, at least 30 mg oxycodone daily, at least 8 mg of oral hydromorphone daily or an equianalgesic dose of another opioid for a week or longer.
