Heart experts at Johns Hopkins are calling premature the early halt of a study by researchers at Walter Reed Army Medical Center and Washington Hospital Center on the benefits of combining extended-release niacin, a B vitamin, with cholesterol-lowering statin medications to prevent blood vessel narrowing. Cardiovascular atherosclerosis, as it is also known, is believed responsible for one in three deaths in the United States each year. (Source: Cardiovascular / Cardiology News From Medical News Today)

The MHRA has published a Public Assessment Report summarising adverse drug reaction reports and published literature on statins, which concluded that sleep disturbances, memory loss, sexual dysfunction, depression and interstitial lung disease (leading to breathing problems) may be associated with statin use.
The product information for all statins will be updated with warnings on all of these side effects. (Source: NeLM - Cardiovascular Medicine)

We evaluated a 47-year-old woman for recurrent migraine and syncope. The patient had 7 children (not examined by the authors), all of whom also experienced migraine and syncope. The patient's father, now deceased, had reportedly experienced migraine and episodes of feeling faint. All 5 of the patient's siblings reported migraine, and 4 of the 5 reported syncope. The case of our patient, which we discuss herein, suggests a genetic link between these 2 conditions, both of which include vascular dysregulation in their pathogenesis. To our knowledge, the medical literature contains no previous description of familial assoc...

Electrocardiographic (ECG) lead configuration of modified chest leads (MCL) remains problematic for many health care providers. The consequences include potential iatrogenic injury to patients. Development of a new MCL that incorporates a unipolar limb lead (instead of a bipolar limb lead system) could potentially reduce configuration errors. Such a lead would need to be investigated for diagnostic accuracy before being examined for ease of configuration relative to MCL1. (Source: Journal of Electrocardiology)

constitute a heterogenous group of heart muscle conditions that all have, in common, the symptoms of heart failure. Diastolic dysfunction with preserved systolic function is often the only echocardiographic abnormality that may be noted, although systolic dysfunction may also be an integral part of some specific pathologies, particularly in the most advanced cases such as amyloid infiltration of the heart. By far, the majority of restrictive cardiomyopathies are secondary to a systemic disorder such as amyloidosis, sarcoidosis, scleroderma, haemochromatosis, eosinophilic heart disease, or as a result of radiation treatmen...

The American College of Cardiology/American Heart Association and the European Society of Cardiology each recently published updated guidelines for management of patients with acute ST elevation myocardial infarction. In this article, we highlight the most important new recommendations, review their supporting data, and describe differences between the guidelines.Key aspects of these updates include detailed guidance regarding the selection of a reperfusion strategy and the incorporation of newer adjunctive antithrombotic agents. Both new guidelines suggest caution in the administration of intravenous β-blockers, avoidanc...

A 71-year-old man was brought to the hospital by ambulance and admitted because of intermittent back pain. Physical examination revealed continuous heart murmur. Chest X-ray showed cardiomegaly and pulmonary congestion. The patient received continuous hydration for renal dysfunction and severe metabolic acidosis (base excess of –16.5 mmol/l and pH of 7.24) and underwent enhanced chest computed tomography. The chest computed tomography showed not only ascending aortic dissection of Stanford type A but also pulmonary artery dissection with an aortopulmonary window (red arrow = aortopulmonary window, T = true lumen, F = fal...

Vanderbilt University Medical Center researchers are studying statins, the class of drugs long associated with lowering cholesterol, as a way to reduce H1N1-related deaths. Gordon Bernard, M.D., associate vice-chancellor for Research at Vanderbilt and a critical care pulmonologist, believes statins may reduce flu-related deaths in the intensive care unit by as much as half. The topic of the J-curve relationship between blood pressure and coronary artery disease (CAD) has been the subject of much controversy for the past decades. An inverse relationship between diastolic pressure and adverse cardiac ischemic events (i.e., the lower the diastolic pressure the greater the risk of coronary heart disease and adverse outcomes) has been observed in numerous studies. This effect is even more pronounced in patients with underlying CAD. Indeed, a J-shaped relationship between diastolic pressure and coronary events was documented in treated patients with CAD in most large trials that scrutinized this re...

Echocardiography in hypertrophic cardiomyopathy diagnosis, prognosis, and role in management

Hypertrophic cardiomyopathy (HCM) is diagnosed on the basis of left ventricular (LV) hypertrophy for which there is insufficient explanation (e.g. mild hypertension or mild aortic stenosis with marked hypertrophy). Echocardiography is an invaluable tool in the diagnosis and follow-up of patients with HCM. Echocardiographic assessment requires a comprehensive assessment in several imaging planes with careful attention to correct beam alignment in order to minimize errors in the measurement of LV wall thickness and appropriate identification of hypertrophy with an unusual distribution.
