Scimitar syndrome and the lung
The respiratory morbidities and lung function tests associated with scimitar syndrome were investigated in a series of 81 children followed by Chemin et al. A high rate of pulmonary infections and wheezing was observed during the last 12 months of follow-up, and one-third of children had been re-hospitalized for respiratory cause. Lung function tests in 20 children showed reductions in total lung capacity and FEV1. Percutaneous transhepatic biliary drainage complicated with hepatic hydrothorax
Percutaneous transhepatic biliary drainage (PTBD) is a well-established method in the treatment of malignant obstructive jaundice (used in almost 70% of cases) when endoscopic alternatives fail. Peripheral venoarterial extracorporeal membrane oxygenation improves survival in myocardial infarction with cardiogenic shock
Despite early revascularization, cardiogenic shock complicating acute myocardial infarction carries high mortality. Intra-aortic balloon pump (IABP) support was recently found to confer no mortality benefit over medical therapy in the Intraaortic Balloon Pump in Cardiogenic Shock II (IABP-SHOCK II) trial. This communication focuses on our experience with venoarterial (VA) peripheral extracorporeal membrane oxygenation (pECMO) in patients with acute myocardial infarction complicated by cardiogenic shock.

Surgical and neurologic outcomes after robotic thymectomy in 100 consecutive patients with myasthenia gravis
Conclusions: Robotic thymectomy is a safe and effective procedure. We observed a neurologic benefit in a great number of patients. A better clinical outcome was obtained in patients with early Myasthenia Gravis Foundation of America class.

Conclusions: PET/d-chest enhances the detection of regional progression of NSCLC after SBRT over currently recommended practices. Caregiver Status: A Simple Marker to Identify Cardiac Surgery Patients at Risk for Longer Postoperative Length of Stay, Rehospitalization, or Death.
CONCLUSIONS:: The patients who underwent cardiac surgery who had a paid caregiver had a significantly longer length of stay independent of comorbidity. The increased risk of rehospitalization/death associated with having a paid caregiver was explained by demographics and comorbidity. These data suggest that caregiver status assessment may be a simple method to identify cardiac surgery patients at increased risk for adverse clinical outcomes. A combined algorithm for T-wave alternans qualitative detection and quantitative measurement
Conclusions: A novel TWA analysis algorithm utilizing the wavelet transform and correlation technique is presented in this paper. TWAs are not only correctly detected qualitatively in frequency domain by energy value of T waves, but the alternans frequency and amplitude in temporal domain are measured quantitatively.

Pulmonary focal fibrosis associated with microscopic arterio-venous fistula manifesting as focal ground-glass opacity on thin-section CT
Conclusion: We speculate that the focal fibrosis was produced by a prolonged congestion due to the AVF and that the dilated vessels and thickening of interlobular septa on thin-section CT related to the AVF. Microscopic AVF may be one of the etiologies of focal fibrosis showing focal GGO on thins-section CT. Dilated vessels and thickened interlobular septa around the GGO might offer a clue to the diagnosis of this disease entity. In addition, it should be noted that focal fibrosis may increase in size.

N-terminal pro-B-type natriuretic peptide concentrations and long-term outcome after cardiac surgery: a prospective cohort study
Conclusions Preoperative NT-proBNP concentrations and the EuroSCORE have equivalent, and moderate, predictive accuracy for mortality 3 yr after cardiac surgery. EuroSCORE uses clinical data but is not routinely used for individual clinical risk prediction. NT-proBNP measurement would incur additional costs but can be measured quickly and objectively. With such similar predictive accuracy, factors such as the ease of calculation and cost will likely determine their use in clinical practice.

Rupture of the NovaFlex balloon during transcatheter aortic valve implantation and subsequent dissection of the right iliac arteries [CASE REPORTS]
We report a case with a very rare complication of transcatheter aortic valve implantation. Rupture of the NovaFlex balloon (Edwards transfemoral balloon catheter) occurred during the inflation of the Edwards SAPIEN valve, resulting in dissection of the right common and external iliac arteries during withdrawal of the balloon catheter. The NovaFlex balloon is a part of the Edwards NovaFlex XT transfemoral delivery system.

Selective sympathectomy for hyperhidrosis: technique of robotic transthoracic selective postganglionic efferent sympathectomy [SURGICAL TECHNIQUE]
Selective postganglionic efferent sympathectomy for hyperhidrosis is associated with excellent relief of hyperhidrosis and a low rate of compensatory hyperhidrosis. However, this technique can be difficult using conventional videoendoscopic techniques. We performed this technique using robotic technology in 55 patients. There was complete relief of hyperhidrosis in 53/55 (96%) patients, partial relief in 2/55 (4%) patients and a compensatory hyperhidrosis rate of 7%.

Cysteinyl leukotriene receptor antagonist montelukast ameliorates acute lung injury following haemorrhagic shock in rats [BASIC SCIENCE]
CONCLUSIONS The results of the present study reveal that montelukast may ameliorate lung injury in shocked rats by interfering with inflammatory and oxidative pathways, implicating the role of leukotrienes in the pathogenesis of haemorrhagic shock-induced lung inflammation.

Total aortic arch replacement with a novel four-branched frozen elephant trunk graft: first-in-man results [AORTIC SURGERY]
CONCLUSIONS The graft adds to the ‘frozen elephant trunk’ concept for treating the arch and proximal descending aorta. Early experience demonstrates an excellent 30-day survival. Combining the frozen elephant with a four-branched arch graft increases the armament of the surgeon in the treatment of complex and diverse aortic arch pathology.

CONCLUSIONS The Hybrid OR concept enables the exact diagnosis of coronary status and downstream malperfusion sites and influences the design of surgical and/or endovascular treatment, without time delay and at negligible risk to the patient.

Acute type A aortic dissection: long-term results and reoperations [AORTIC SURGERY]
CONCLUSIONS Acute type A dissection in the current era is associated with a decreasing acceptable operative mortality risk and has a satisfactory long-term survival for hospital survivors. These factors were both involved were associated with a lower 30-day mortality. A substantial proportion of patients will require reoperations on the aortic valve or the aorta.

Asymmetric speed modulation of a rotary blood pump affects ventricular unloading [TX [amp ] MCS]
CONCLUSIONS The present study is an initial step to more accurate speed modulation of RBPs to optimize the cardiac load control. To develop future control algorithms, the concept of high speed during diastole having a maximal unloading effect on the LV and high speed during systole increasing the pulse pressure is worth considering.
