Pulmonary Sequestration Presenting as an Esophageal Submucosal Tumor.
We present the case of a patient whose initial symptom was mild dysphagia. Esophagoscopy showed the presence of a submucosal tumor in the lower-third section of the esophagus. The tumor was excised via video-assisted thoracoscopic surgery. The histopathology results indicated extrapulmonary sequestration. To our knowledge, such a case of extralobar sequestration presenting as an esophageal submucosal tumor has not yet been reported. PMID: 23303580 [PubMed - as supplied by publisher]

Re: ‘Vitamin D Deficiency and PAD: A Close and Often Overlooked Relationship’ Vitamins and Homocysteine Metabolism
We reported that in PAD patients successfully treated for hyperhomocysteinemia, Nɛ–Hcy–Lys is still associated with disease progression and higher risk of adverse cardiovascular events. Moreover, high intake of vitamins C and E and carotenes is associated with lower tHcy.

Hypertrophic Cardiomyopathy in Childhood The Gradient Is Not the Disease. Excessive Use of Experimental Invasive Interventions
We read with much interest the study of Sreeram et al. (1) from Germany and United Kingdom in which the authors used the novel strategy of radiofrequency ablation of the ventricular septum to produce intramuscular scars and reduce outflow tract obstruction in young children with hypertrophic cardiomyopathy (HCM). The study population is unique for HCM: 1) sizable for this age group (mean age: 10 years, with children as young as 2 years [n = 2] and with 25% 6 years of age or younger); 2) symptoms attributed to HCM although seemingly very modest (i.e., tiredness in 28 patients), with some being virtually asymptomatic; and 3)...

Supplement on Primary Hyperparathyroidism
Primary hyperparathyroidism (PHPT) is a relatively common endocrine disorder that has evolved from a symptomatic hypercalcemic disorder associated with overt skeletal and renal complications to a condition that is now recognized at an earlier asymptomatic stage. This special supplement of the Journal of Clinical Densitometry (JCD) is dedicated to the diagnosis and management of this current phenotype of PHPT. We cover in detail advances in diagnosis, presentation, and impact of the disease on BMD and fracture risk. Also discussed are aspects of PHPT in the context of cognitive, cardiovascular, gastrointestinal, and musculo...

A systematic review of abusive visceral injuries in childhood-Their range and recognition.
CONCLUSIONS: Visceral injuries may affect any organ of the body, predominantly abdominal viscera. A non-motor vehicle related duodenal trauma in a child aged<five years warrants consideration of abuse as an etiology. In the absence of clear evidence for a screening strategy, clinical vigilance is warranted in any young child with suspected abuse for the presence of abdominal injury, where the absence of abdominal bruising or specific symptoms does not preclude significant injury. Increased occurrence of spinal fractures related to ankylosing spondylitis: a prospective 22-year cohort study in 17,764 patients from a national registry in Sweden.
CONCLUSIONS: Despite the improved treatment of AS the annual number of vertebral fractures requiring inpatient care increased during the last two decades. Possible explanations are population growth, greater awareness of fractures, improved diagnostics, improved emergency care reducing fatalities, and a higher activity level of patients receiving modern medical therapy. Obviously the improvement of medical treatment did not reduce the susceptibility of these patients to unstable fractures. Thus the restrictive injury prevention recommendations for patients with AS cannot be defused, but must be critically revised to improv...

Feasibility of transapical aortic valve replacement through a left ventricular apical diverticulum
Transapical aortic valve replacement is an established technique performed in high-risk patients with symptomatic aortic valve stenosis and vascular disease contraindicating trans-vascular and trans-aortic procedures. The presence of a left ventricular apical diverticulum is a rare event and the treatment depends on dimensions and estimated risk of embolisation, rupture, or onset of ventricular arrhythmias. The diagnosis is based on standard cardiac imaging and symptoms are very rare. In this case report we illustrate our experience with a 81 years old female patient suffering from symptomatic aortic valve stenosis, respir...

Extracellular matrix scaffolds for cartilage and bone regeneration.
Abstract Regenerative medicine approaches based on decellularized extracellular matrix (ECM) scaffolds and tissues are rapidly expanding. The rationale for using ECM as a natural biomaterial is the presence of bioactive molecules that drive tissue homeostasis and regeneration. Moreover, appropriately prepared ECM is biodegradable and does not elicit adverse immune responses. Successful clinical application of decellularized tissues has been reported in cardiovascular, gastrointestinal, and breast reconstructive surgery. At present, the use of ECM for osteochondral tissue engineering is attracting interest. Recent d...

Predicting symptoms of post-traumatic stress among patients undergoing orthopedic surgery on the basis of routinely collected cardiovascular data
The present study examined whether in-hospital assessments of heart rate and blood pressure predicted symptoms of post-traumatic stress (PTS) in 110 patients undergoing total knee replacement surgery. After controlling for conceptually relevant factors, in-hospital cardiovascular functioning predicted symptoms of PTS three months following surgery. Specifically, lower pre-surgical and post-surgical systolic and diastolic blood pressure predicted more symptoms of avoidance and PTS (total). In contrast, higher heart rate prior to and shortly following surgery predicted marginally more intrusive thoughts. The present findings...

Novel applications of an injectable radio-opaque hydrogel tissue marker for management of thoracic malignancies.
CONCLUSIONS: This cadaveric series demonstrates the excellent visibility of a radio-opaque injectable hydrogel marker in the human thorax by multiple common imaging techniques. The hydrogel marker forms a well-localized bleb within tissue, which can assist with triangulation of disease during minimally invasive thoracic surgery. Esophageal applications include radiographic delineation of tumor defined by endoscopy, and image guidance for radiotherapy. Future in-vivo studies are warranted, because radiopaque injectable compounds are promising alternatives to metal fiducials. PMID: 23287908 [PubMed - as supplied by publisher]
