Meet the face behind Boston Children’s Hand Transplant Program
Amir Taghinia, MD As a child, Amir Taghinia, MD, was fascinated by the mechanics behind how things worked. “I was always taking things apart and trying to fix them,” says Taghinia, surgical director of Boston Children’s Hospital’s newly launched Hand Transplant Program. “I ruined more than one toy truck back in the day, but it never upset me much. For me, the excitement of seeing how the individual parts worked together, and the challenge of trying to re-create it, was more fun than the toy anyway. ” It’s a fascination that would stay with Taghinia from grade school to medical schoo...

The future of robotics: in a transhuman world, the disabled will be the ones without prosthetic limbs
Bertolt Meyer's amazing bionic hand controlled by an iPhone app is a glimpse of the advances being made in prosthetics. But in years to come, will everyone want one?Bertolt Meyer is used to being viewed as not fully human. Born with a stump where his left hand should have been, he spent his childhood wearing a hook connected to an elaborate pulley and harness. "To open the hook and grasp things I had to flex my shoulders like this," he says, striking a he-man pose. "The harness was very uncomfortable. To stop it chafing my skin, I had to wear a shirt underneath it at all times. I was always sweating."Even when, at the age ...

Tissue expansion for correction of scars due to burn and other causes: A retrospective comparative study of various complications
Conclusion: This set of data suggests that the burn scar patient is at no greater risk to suffer complications from tissue expansion. Other factors like age, TBSA and anatomical site have far more influence on the expander complication rate than the initial indication for tissue expansion. Effects of keratinocyte growth factor on skin epithelial differentiation of human amnion epithelial cells
Abstract: The aim of the present study was to determine the effects of KGF on the differentiation of cultured human amnion epithelial cells (HAECs) towards skin keratinocyte. HAECs at passage 1 were cultured in medium HAM's F12: Dulbecco's Modified Eagles Medium (1:1) supplemented with different concentrations of KGF (0, 5, 10, 20, 30 and 50ng/ml KGF). Dose–response of KGF on HAECs was determined by morphological assessment; growth kinetic evaluation; immunocytochemical analysis; stemness and epithelial gene expression quantification with two step real time RT-PCR. KGF promotes the proliferation of HAECs with maximal eff...

Confessions of a Worrywart
While reading Susan Orlins’s poignant memoir, I was reminded continuously of the catchphrase from the late ’60s and early ’70s feminist movement: “The personal is political.” The specifics of Orlins’s life story are hers alone, but as many of her experiences, thoughts, and feelings are universal to all women, her book was a pleasure to read. Dubbed Confessions of a Worrywart: Husbands, Lovers, Mothers, and Others, and featuring such colorful chapter titles as “Shrinks are Like Boyfriends Who Can’t Dump You,” “Saving E-Mail, Saving Voicemail,” and “In Search of Grief,” Orlins’s story weaves th...

PIP implants should be removed for preventative reasons
Breast implants manufactured by Poly Implant Prothèse (PIP) have a poorer mechanical durability and, in a number of cases, contain an irritant substance in significantly higher levels than other silicone breast implants. The Medical Products Agency and the National Board of Health and Welfare therefore recommend that clinics that have, in the past, used PIP implants inform the women affected of these risks and also that PIP implants should be removed for preventative reasons, except in individual cases where there are medical reasons for not doing so. Yes, nanoscience can enhance humans – but ethical guidelines must be agreed
People 'enhanced' into spider-climbing individuals with hugely projected breasts and Einstein-brains… Where will it stop?Engineers are trained to try to figure out how to achieve things that humans cannot and in nanoscience and nanotechnology that challenge is no different. Many of the most exciting advances in the field try to improve human incapacities with things such as memory, hearing, stamina or intellect. In my field of nanomedicine, the notion of human enhancement is, in a lot of cases, a way to deal with disease: enhancing vision, cognitive functions or improving a person's ability to move independently.I have al...

Summary: We experienced three patients with long-standing unilateral complete facial paralysis who previously underwent temporalis muscle transfer to the cheek for smile reconstruction. All patients complained of insufficient and uncomfortable buccal motion synchronised with masticatory movements and incomplete eyelid closure with ptotic eyebrow. To attain a near-natural smile and reliable eyelid closure, temporalis muscle was displaced from the cheek to the eyelid, and a neurovascular free latissimus dorsi muscle was transferred for the replacement of cheek motion. As a result, cheek motion synchronised with the contralate...

[News] Cosmetic implants could delay breast cancer diagnosis
Cosmetic breast implants are radio-opaque, which makes the mammograms of women with implants difficult to read; even with specialised radiographic techniques, about a third of the breast is not adequately visible. Therefore, although the available evidence does not suggest that cosmetic implants themselves increase the risk of breast cancer, concerns remain that they might cause a delay in diagnosis. It is also unclear whether women who have undergone cosmetic breast augmentation—more than 300 000 in the USA alone in 2011—are more likely to die from breast cancer than are women diagnosed with the disease who have not...

A psychological perspective on male rejuvenation
Male rejuvenation, defined as a process in men to both limit the impact of aging on body image and experience greater virility, is growing among middle-aged and older men. While rejuvenation was primarily in the domain of the younger male athlete with the use of performance enhancing hormones or in the aging woman through the use of cosmetic surgery, it is now more common among middle-aged and older men. Male rejuvenation can occur both through aesthetic surgical means and hormonal manipulation through anabolic steroid use. In this article, the authors review the psychological issues and perceptions surrounding male aesthe...

Fire Risk During Ophthalmic Plastic Surgery
A high oxygen concentration was reported as the major risk factor behind 74% of all surgical fires. Given that surgical fires are a dreaded complication of combining electrocautery and oxygen use, minimizing increases in oxygen concentration above atmospheric levels should be the goal of not just anesthesia staff, but also surgeons. With 28% of surgical fires occurring on the face, reducing this risk factor is of particular interest to the ophthalmic surgeon. (Source: Ophthalmology)

Platelet gels
(PG) are blood‐derived biomaterials that are generally obtained through the activation of a platelet‐rich plasma or a platelet concentrate by thrombin or calcium chloride, resulting in the simultaneous conversion of fibrinogen into a fibrin gel and in the generation of a platelet releasate rich in a physiological cocktail of growth factors. To reinforce the physical strength of the fibrin network, a fibrinogen‐rich fraction – generally cryoprecipitate – can be added to the platelet fraction prior to activation, resulting in the generation of platelet fibrin glue (PFG). PG and PFG, prepared from single donations,...

Atypical fibroxanthoma of the medial canthus: a rare presentation.
Conclusion: This case emphasises the importance of a diagnostic biopsy. A panel of immunohistochemical markers is essential to make the correct diagnosis of AFX and distinguish it from malignant fibrohistiocytoma and non-keratin expressing squamous cell carcinoma. It usually follows a benign course, but can occasionally demonstrate aggressive local recurrence as well as distant metastasis. Invasion beyond the dermis and a rapid rate of recurrence are suggestive of a more aggressive clinical course. Paraffin fixed sections are preferred over frozen section techniques for this tumour. Medial canthal AFX has not been reported...

[Financial rating optimization in pressure ulcers management: Yes, but at what price?].
Abstract The surgical management of pressure ulcers remains very expensive even if preventive measures and improved care pathways allowed to reduce spending in this domain in recent years. Since 2004, the funding of French hospitals by "fee-for-service" and the needs of saving health spending necessarily compels us to interest ourselves in these purely economic considerations and sometimes modify our requirements for hospital stay to optimize a "patient' valorisation group". In the future, this may lead the surgeon to bias the real needs of the patient for the benefit of hospital establishment. Through a medico-eco...
