A universal torque wrench system
Dental implant manufacturers supply various forms of torque limiting devices for use with implant restorations. A clinician may use these instruments to ensure adequate torque delivery with appropriate precision. Throughout the early period of dental implant prosthetics, screw loosening was a major complication, but a later study found that while screw loosening remained a potential prosthetic complication, it was less common than the early studies suggested. Nevertheless, inadequate tightening with insufficient torque has been cited as a possible reason for screw loosening.

Fabrication of a unilateral oral commissure retractor
The fabrication of a unilateral device to treat microstomia after trauma is presented. Maximum opening was recorded before treatment. A dual arch impression that captured the dentition and commissure on the affected side was made with vinyl polysiloxane (VPS) impression material. A 1 mm vacuum formed template (VFT) was fabricated over the maxillary dentition, and a 2 mm VFT was fabricated around the commissure. A 0.9 mm (0.036”) stainless steel wire was attached with acrylic resin to both templates in order to apply a constant force to the commissure. The use of a helix in the wire allowed for flexibility and adjustment ...

Is seeing believing?
One of our readers wrote the following. I routinely use a “light” version of Photoshop to print and digitally store photos of my patients. I also use it to reduce the photos to a size that permits me to put all of them on a page of glossy photo paper. I routinely try to increase the brightness and occasionally the contrast of the photos so that the starting and finish photos are as identical as possible.Recently, we had a patient come to the office to have her braces removed. I had started the case two and a half years earlier and then almost immediately the family moved. Apparently, things didn't work out for them, a...

Onychophagia and postorthodontic isolated gingival recession: Diagnosis and treatment
This clinical report describes the diagnosis and the management of isolated-type recession defects of complex etiology in 2 healthy postorthodontic patients. The lesions were confined to 1 mandibular incisor and were associated with an abnormal buccolingual inclination of the affected tooth despite a lingual retainer made with a round stainless steel twisted wire. After careful questioning, it was determined that the recession defects were indirect effects of habitual onychophagia. The concomitant fingernail-biting habit and the lingual bonded retainer led to the indirect development of bone dehiscence and, consequently, gingival recession.

Impacted incisors associated with supernumerary teeth treated with a modified Haas appliance
This report describes the successful approach to treatment for a young boy who had 2 supernumerary teeth associated with impaction of the left central and lateral permanent incisors. Treatment consisted of extracting the supernumerary teeth and performing maxillary expansion with a modified Haas appliance to guide the left central incisor into its appropriate position. The teeth erupted spontaneously after maxillary expansion and an increase in space. A fixed edgewise appliance was placed incrementally to correct the maxillary anterior tooth positions and finish the treatment. Adequate esthetics and function were achieved.

Distalization of the mandibular dentition of an adult with a skeletal Class III malocclusion
This case report describes the orthodontic treatment of an 18-year-old woman with a skeletal Class III malocclusion and a midline deviation. The treatment plan consisted of distalizing the mandibular dentition asymmetrically and producing space for retraction of the mandibular anterior teeth. Short Class III elastics, an open-coil spring, and the mulitloop edgewise archwire technique were used, combining the entire maxillary dentition as integrated anchorage. The active treatment period was 26 months. Normal overbite and overjet were obtained, and facial balance was improved.

Correction of a dental arch-width asymmetric discrepancy with a slow maxillary contraction appliance
A boy, aged 12 years 3 months, sought treatment for a complete unilateral Brodie bite. His maxillary dental arch was asymmetric, and his dentition was 10 mm wider than normal values for his age. The transverse discrepancy was his chief complaint because it caused a chewing dysfunction. We used a special slow maxillary contraction appliance, which contains a screw, connectors, and retainers, to contract the maxillary transverse asymmetric dental arch. This was followed by preadjusted fixed appliances to level the teeth and adjust the occlusion. A symmetric and functional Class I occlusion was achieved.

Dynamic simulation of the self-tapping insertion process of orthodontic microimplants into cortical bone with a 3-dimensional finite element method
Conclusions: This in-vitro finite element analysis showed that, during the self-tapping insertion of orthodontic microimplants, stresses high enough to fracture cortical bone can develop. After the self-tapping insertion, the radial stresses calculated at the interfacial bone were higher than the threshold value to trigger pathologic bone resorption.

Three-dimensional assessment of transverse skeletal changes after surgically assisted rapid maxillary expansion and orthodontic treatment: A prospective computerized tomography study
Conclusions: The results showed that, for registering transverse skeletal changes after surgically assisted rapid maxillary expansion, 3-dimensional superimposition is a reliable method, circumventing projection and measurement errors. Surgically assisted rapid maxillary expansion had a significant but nonuniform skeletal treatment effect. Despite careful surgical separation, pronounced posterior tipping occurred. No correlation was found between the severity of tipping and the patient's age.

Long-term stability of anterior open-bite closure with bilateral sagittal split osteotomy
Conclusions: Approximately 90% of the treated patients had a positive incisor overlap 4.5 years after orthodontic appliance removal. Despite a 60% loss of mandibular surgical closing rotation, only 10% of the patients relapsed to no incisor overlap in the long term. Bilateral sagittal split osteotomy and surgical closing rotation of the mandible by using rigid internal fixation should be considered a stable alternative in the treatment of mild-to-moderate anterior open-bite malocclusions.

Residents' journal review
Achieving a stable result is a constant concern in orthodontic treatment. Without retention, there is a high risk of the teeth relapsing to their original positions. One option for retention is the removable thermoplastic appliance, or Essix retainer. Full-time Essix wear immediately after removal of the fixed appliance is recommended, but there are various opinions on duration. Recommendations range from 1 week of full-time wear to 3 months of full-time wear. There were 2 experimental groups in this study. One wore the Essix full time for 3 months, and the other for 1 week. The rest of the 6-month observation period was n...

When she called to make the appointment, she said, “Dr Jones’s office was like a factory.”

The Society of Orthodontic Educators
How many of you reading this editorial have participated in teaching orthodontic residents at an accredited graduate program in the United States or Canada? Many of you participate as part-time clinical faculty with a commitment of 1 day per month up to 1 or 2 days per week. Part-time faculty members are truly the backbone of orthodontic education. I can remember back to my orthodontic residency and the immense amount of valuable clinical information that I gleaned from the part-time clinical faculty, all of whom had private practices in the nearby community.

Accumulation of inactive p53 protein in oral squamous cell carcinoma: stabilization by protein interaction
Our previous study showed that p53 protein is accumulated in >60% of cases of oral squamous cell carcinoma (OSCC) and its overexpression is related to poor prognosis. However, the mechanism behind this is still elusive. The present study attempts to dissect p53 alterations at various levels from gene to function in tumor biopsies to understand whether molecular alterations have any relationship with the accumulation of p53 protein in OSCC. Protein‐stabilizing mutations were observed in only 13.8% of the samples. Neither p53 polymorphisms nor human papillomavirus (HPV) infection (<2%) has any major role in augmented...
