Topical Silicone Gel for Burn Scars
I have read the studies the promote the use of silicone sheets for scar treatment.  I know the claims Mederma and other silicone gel scar treatments make.  What do I tell my patients?  “It doesn’t matter what you use.  It matters that you use it.  Mederma is non-scented and less greasy than Vit E or cocoa butter.  If that will entice you into doing your dailyscar massage, then use it.” For most scars, I will stand by the above.  Burn scar are characterized by an increasing occurrence of redness, thickness, stiffness, pain, and itching, and a disturbance in pigmentation and surface ...

A complete summary (probably TMI) about my time at the human #microbiome meeting #HMP2010
Well, I am writing this on my flight heading back from St. Louis after going to the "Human Microbiome Research Conference" I have already posted some summaries of the meetings Day 1 and Day 2  and will soon do one about Day 3.  And you can find out more about the meeting from multiple other sources such as here, here, here and here.  But what I thought I would do here is give some of the story behind the meeting - why I went and what it was like behind the scenes, at least for me.  Note - I know this is very long - but hopefully it will give people a feel both for some of the science, some of...

Itchy Bottom? Pre-Moistened Wipes Can Cause A Rash
Itchy behinds are part of my everyday. It shouldn’t be part of yours. Itching on your bottom is usually a minor annoyance, but it can be debilitating. A patient of mine was unable to work because his derrière itching was so intense. A common cause of itching bottom, called pruritus ani, is allergic contact dermatitis. Places where your skin transitions from outside to inside, such as the lips or the anus, are susceptible to dermatitis because your skin’s barrier is limited in these locations. Without a thick layer of keratin, irritants on the outside easily get into your skin. Moist toilet tissues, such as baby wipes...

Has Your Home Aging Skin Treatment Been Disappointing?
Have you found yourself getting frustrated over the lack of improvement you have seen from the aging skin treatment you’ve been using? This is not a unique experience I can assure you, because literally millions of people feel exactly the same way you do.  The problem lies in the fact that the cosmetic companies aren’t giving their customers the effective, high quality ingredients they need. Most of the wrinkle reducing formulas on the market are primarily synthetic, save for the errant natural ingredient appearing here and there.  These are the products that are typically built around the inclusion of livestock deri...

There is a hurricane coming
It is so exciting! It will be a big rainy, stormy day and then we can all compare beach erosion. Unless you live on the beach in which case, you need to move out for a few days. Or your boat is in the water and is in an unprotected area and yo need to move it out of the way. (If you aren't sure, ask people who have had to helicopter their boats out of the marshes back to the harbors after the storm surge.)We are watching continually updated storm projections including the famous 'spaghetti' plot (I'm not kidding - that's the technical term) where all the computer models of the storm track are put on top of each other - and...

It's never easy....
The life of a general surgeon is one fraught with contingency, soul-crushing doubt, unexpected disaster, and overwhelming stress. I wouldn't wish it upon my worst enemy. Fortunately, I was brainwashed to a sufficient degree during residency such that I actually don't mind my job. One of the reasons general surgery is so tough is that it is nearly impossible to map out your week according to a strict schedule. Maybe at some point in a career, when you're the established, Big Kahuna of the group, you can load up your work week with elective breast biopsies, hernia repairs and lap choles and leave the middle of the night disa...

A Dermatology Visit: 10 Tips To Get The Most Out Of It
Having a high-quality doctor’s visit takes effort on your doctor’s and yours. Here are 10 tips to get the most out of your next visit with a dermatologist: 1. Write down all the questions you have and things you want to discuss with me. Be sure to list any spots you’d like me to check or any moles that have changed. Have a loved one lightly mark spots on your skin they are concerned about. 2. Know your family history: Has anyone in your family had skin cancer? What type? Patients often have no idea if their parents have had melanoma. It matters. If possible, ask before seeing me. 3. Know your history well: Have...

EMR Technology Experiences Growing Pains: Resistant Doctors, Computer Glitches, and Unrealized Benefits
Last week I had my first visit with my new primary care doctor. I picked him based on recommendations (plus he’s one of the few that accepts my insurance), and also because he seemed to be an eager adopter of electronic medical records (EMR). On his website, there was a portal for making appointments on-line, asking questions of the doctor and staff by e-mail and once a registered patient, I could also use a secure system to access my medical records. With EMRs being portrayed as key drivers of quality and savings in health reform, I felt encouraged by my new doctor’s embrace of the technology. But when he greeted me i...

Eye Wrinkle Cream Reviews are Filled with Hype!
Eye wrinkle cream reviews generally don’t give you enough information about what is necessary for truly repairing the issues surrounding the formation of lines, skin slackening, bags, and dark circles. What you get is mostly a sales pitch, when what you need is pertinent information about the ingredients, how they work, and any possible side effects these ingredients could have.  Let me give you an example. One of the latest fads when it comes to “repairing” lines and wrinkles around the eyes is compounds that temporarily paralyze the muscles to lessen the affects of making facial expressions. This does nothing to a...

Rapid Melanoma Diagnosis with Help from Image Processing Algorithms
Researchers from Missouri University of Science and Technology have received a patent for "Automatic Detection of Critical Dermoscopy Features for Malignant Melanoma Diagnosis." The system relies on taking close-up photos of suspected lesions using a specialty built device. These are then processed on a computer that grades the presence of various image characteristics and comes up with a total score that suggests potential melanoma. Moss and his fellow researchers use photographs taken with a device called a dermatoscope. The scope contains a magnifier, a light source and a transparent plate, which is placed on skin coat...

Study Finds Women Who Drink Beer are More Likely to Develop Psoriasis
BBC News reports that a new study in the Archives of Dermatology links women beer drinkers with an increased psoriasis risk. The study followed 82,000 female nurses. The study found women who drank five or more beers a week doubled their risk of psoriasis. In the study, researchers said they observed a 72% increased risk of psoriasis in women who drank more than 2.3 alcoholic drinks per week compared with non-drinkers. For women who drank five glasses of beer per week their risk of developing psoriasis was 1.8 times higher again. This was a large study so the connection between women beer drinkers and psoriasis could ...

I've been avoiding my health
So now its time for an update on how I am doing. First of all, my back hurts, all the time, and it doesn't matter if I am sitting, standing, walking, or lying down. It hurts. Sometimes it hurts all across my upper back. Sometimes it hurts sharply in a specific place. The only difference is that if I move around, it hurts less in some areas and if I sit down, I can ice it. Currently I am waiting for my back doctor to call me back to find out the next steps - either an office visit or more needles. I do want to talk to him about trying acupuncture. This week I see by my therapist and my meds therapist. They are the ones that ...

4 ways to calm your skin
Everyone wants that summer glow, but sun and heat can be hard on skin, upping your risk for sunburn, breakouts, age spots, and irritation. What's Up With Teen Stars Getting Plastic Surgery?
There is an interesting article in Popeater.com about the trend of more and more teen stars having plastic surgery. The most recent obvious example is singer Charice, who reportedly received Botox injections to thin her lower face. While Charice's doctor, Dr. Vicki Belo, is called a "plastic surgeon," she's actually a dermatologist. For the Popeater article on teen plastic surgery and its Hollywood connections, click here. Drug To Treat Spider Veins: Asclera Now FDA-Approved
The FDA recently (March 2010) approved Asclera (poliocanol) injection for the treatment of small spider veins (tiny varicose veins less than 1 millimeter in diameter) and reticular veins (those that are 1 to 3 millimeters in diameter). Asclera is a detergent sclerosant and produces endothelial damage through interference with the cell’s surface lipids.and acts by damaging the cell lining of blood vessels. This causes the blood vessel to close, and it is eventually replaced by other types of tissue. (more…) *This blog post was originally published at Suture for a Living*

Neutrophils and Dermatology
On Thursday, July 29, I visited Mayo Clinic to assess Cycle 31 of pomalidomide (CC-4047). Still stable. IgG was up about 3.5%, and M-spike went from 1.0 to 1.1 g/dL. But we've been here before. In February, IgG was a little bit higher than it was Thursday, and M-spike was 1.1 just last May. The numbers may have a slight upward trend, but they do seem to bounce around on their way up. I'll not worry this time. Maybe next time. Neutrophils: My neutrophil count was 930 cells per microliter, just below the threshhold. How the Health Insurance Industry Saved Obamacare
Why Big Health Insurance Supported Obamacare, Part III Podcast: As we have seen, the fact that the health insurance industry was going to support healthcare reform after the 2008 elections was a foregone conclusion.  The question was: How would the insurance industry support healthcare reform? When the time came, the support the insurance industry gave to President Obama’s efforts to reform healthcare followed four simple rules: 1) Do not actively oppose Obamacare. In stark contrast to its behavior during the Clinton’s effort to reform healthcare in 1993-94, this time the insurance industry never engaged its ...

Doctors hiring scribes because of electronic medical records
by Douglas Perednia, MD I was visiting with my physician colleague Debbie the other day, which is always a pleasure. She is, by far, one of the best dermatologists I know. She had purchased an electronic medical record (EMR or EHR) a year or two ago and I asked her how it was going. “Pretty well,” she said, “but I’ve had to start using a scribe. It’s just too inefficient any other way. I was losing too much time and money.” Of course she’s also losing money by hiring a scribe, but Debbie is very persistent and always wants to do the right thing. This is one reason why her patients and I admire her so much. Th...

Jock Itch: How To Get Rid Of It
I’ve been seeing a lot of jocks lately. The anatomical area, not the athletes. Summertime means heat and humidity, sports, and itchy groins. Jock itch is a general term for an itchy rash in the groin. Heat, sweat, and skin rubbing on skin can leave the area looking like you slid into second base, groin first. There are three main causes of an itchy groin. Classic jock itch is caused by a fungus, the same fungus that causes athlete’s foot. This fungus often causes a red scaly rash on the inner thighs. It tends to be dry and can have bumps or pimples. The fungus is often spread from your feet or from contaminated sports ...
