Inhibition of neuropeptide degradation suppresses sweating but increases the area of the axon reflex flare
Abstract
The neuropeptides CGRP (Calcitonin Gene Related Peptide) and Substance P (SP) mediate neurogenic inflammation. Both are degraded by the neutral endopeptidase (NEP) which can be blocked by phosphoramidon. The aim was to evaluate the effect of NEP inhibition on sweating and vasodilatation.
Dermal microdialysis was performed on the skin of 39 subjects. Two fibers were perfused with phosphoramidon (0.01%, 0.02% or 0.2%), two with saline. Acetylcholine (ACh) was either added to the microdialysis perfusate (n = 30, 10‐2 M) or thermoregulatory sweating was induced (n=9).
Co‐application of phosphoramidon reduced choli...

[Cutaneous revelation of Rosai-Dorfman disease: 7 cases].
Abstract
BACKGROUND: Rosai-Dorfman disease (RDD) is a benign form of non-Langerhans-cell histiocytosis. It is identified by a particular histological profile first observed in febrile lymph nodes. Extranodal sites are frequent. The most common site is the skin, which can reveal the disease despite a difficult and delayed diagnosis. Seven cases of cutaneous revelation of RDD were studied retrospectively in order to delineate the clinical characteristics and facilitate diagnosis and treatment of this extremely rare disease.
PATIENTS AND METHODS: Six cases of RDD from 1990 to 2011 were identified in the photograph...

[Changes in mortality in the Reims University Hospital Dermatology Department (1996-2009)].
CONCLUSION: This study shows an increase in the number of deaths in a French university hospital dermatology department over the 14-year period in question, ascribable mainly to an increase in the number of patients receiving end-of-life care for metastatic melanoma. Significant changes were noted in the management of these patients with an increase in palliative care procedures.

[Pseudolymphoma of the skin: Ambiguous terminology. A survey among dermatologists and pathologists].
This study demonstrates that, in France, the term pseudolymphoma is an ambiguous notion. We believe that cases in which it is impossible to differentiate pseudolymphoma from cutaneous lymphoma should be referred to as lymphoproliferations of undetermined significance, since more than 50% of physicians consider that the term pseudolymphoma designates a resolutely benign entity.

[Stevens-Johnson syndrome concurrent with primary Epstein-Barr virus infection].
We report a case of Stevens-Johnson syndrome concurrent with EBV infection.
CASE REPORT: A 17-year-old boy consulted for an eruption suggestive of Stevens-Johnson syndrome, which was supported by the histopathology results. The patient had taken no medication during the previous weeks. Laboratory examinations showed atypical activated T lymphocytes. Serological tests and PCR results confirmed the diagnosis of primary EBV infection. The outcome was spontaneously favorable with only symptomatic treatment being required.
DISCUSSION: Stevens-Johnson syndrome is characterized by "target" lesions and profuse mucous membr...

[Pemphigus vulgaris: An unusual clinical presentation].
DISCUSSION: During the course of pemphigus, the nail findings most commonly reported in the literature are paronychia, onychomadesis and onycholysis. However, these signs are generally seen in patients with known pemphigus vulgaris and only rarely indicate bullous disease. The case we report illustrates a special situation in which pemphigus vulgaris was revealed by unusual skin and nail lesions.

[Persistent aphthous mouth ulcers associated with tocilizumab: Two cases].
We describe for the first time the occurrence of recurrent aphthous mouth ulcers in two patients on TCZ.
PATIENTS AND METHODS: Two patients were treated with TCZ for rheumatological disease. A few weeks after administration of TCZ, they presented with multiple painful mouth ulcers that would not heal until TCZ had been withdrawn. In both cases, the oral ulcers resolved 6 to 7weeks after withdrawal of TCZ and readministration of the drug led to recurrence of oral ulcers within 10days in both patients.
DISCUSSION: We describe for the first time the occurrence of aphthous mouth ulcers induced by TCZ. The causative rol...

[Bifocal Buruli ulcer: Multiple cephalic lesions after initiation of medical treatment].
We report a case observed in the Ivory Coast, one of the areas with the highest prevalence of BU worldwide. The distinguishing features of our case are the early onset of this paradoxical reaction and the multiple cephalic site of lesions.
PATIENTS AND METHODS: A 4-year-old child with no prior medical history was referred for two painless ulcerative cutaneous nodules. Incubation of samples from the edges of these lesions revealed the presence of acid-alcohol resistant bacilli (AARB), which were shown by PCR to be M. ulcerans, the causative agent in BU. Introduction
This issue of Seminars in Perinatology is devoted solely to skin care and cutaneous diseases of the neonate and infant. I think it is an extraordinary issue since to the editors’ knowledge no such issue has been devoted to this subject previously. The time is certainly appropriate to cover diseases of the skin. The first textbook on the subject, aptly titled Textbook of Neonatal Dermatology, by Eichenfield, Frieden, and Esterly was published over a decade ago in 2001. The third edition currently is in production; I recommend it as an invaluable source for all involved in the care of neonates and infants. That said, while...

Demodex quantification methods: Limitations of Confocal Laser Scanning Microscopy (CLSM)
We read with interest the recent paper by Sattler et al.,1 describing the quantification of Demodex mites by confocal laser scanning microscopy (CLSM). Their work is interesting but we feel there are limitations to the use of this method to accurately detect absolute numbers of mites in human skin. Because the role of Demodex in normal or diseased skin is unknown,2 their possible role in modulation of immune reactivity of the follicle3 and an association between the degree of mite infestation and the development of rosacea,4‐6 accurate reproducible in vivo methods of quantifying mites is becoming increasingly important.

Deficiency of n‐6 polyunsaturated fatty acids is mainly responsible for atopic dermatitis‐like pruritic skin inflammation in special diet‐fed hairless mice
In conclusion, our results indicate that deficiency of n‐6 PUFAs is mainly responsible for AD‐like symptoms by HR‐AD feeding. Thus, this model could be useful for studying the pathomechanisms associated with deficiency of n‐6 PUFAs in AD.

Use of dehydrated human amniotic membrane allografts to promote healing in patients with refractory non healing wounds
We describe the results of EpiFix treatment in four patients who had not achieved wound closure with both conservative and advanced measures, and had been referred for a definitive plastic surgery procedure. Healing was observed in a variety of wounds with one to three applications of the dehydrated amniotic membrane material. The material was well tolerated by patients. Healed wounds did not recur in long‐term follow‐up. Further investigation of the use of dehydrated amniotic membrane in broader application to various types of dermal wounds should be considered.

Human Dermal Exposure to Galaxolide from Personal Care Products
Synopsis
Musks are synthetic fragrances applied on personal care and household products as fixatives, by retarding the release of other fragrances with higher volatility. Galaxolide is the most used polycyclic musk since the 90th decade, and it has been detected in several environmental and biological matrices, particularly in human tissues and fluids. For exposure assessment purposes, large monitoring data need to be obtained and rapid but reliable analytical techniques are requested. The main objective of this study is to develop and validate a new and fast analytical methodology to quantify galaxolide in personal care p...

There is a world beyond protein mutations: The role of non‐coding RNAs in melanomagenesis
Abstract
Until recently, the general perception has been that mutations in protein coding genes are responsible for tumorigenesis. With the discovery of V600EBRAF in about 50% of cutaneous melanomas there was an increased effort to find additional mutations. However, mutations characterized in melanoma to date cannot account for the development of all melanomas. With the discovery of microRNAs as important players in melanomagenesis protein mutations are no longer considered the sole drivers of tumors. Recent research findings have expanded the view for tumor initiation and progression to additional non‐coding RNAs. The ...

NF1 gene silencing induces upregulation of VEGF expression in both Schwann and non‐Schwann cells
Abstract
Neurofibromatosis type I (NF1) is associated with typical hypervascular tumors, including neurofibroma, glioma, malignant peripheral nerve sheath tumors (MPNST), and glomus tumors. Previously, we and other groups reported that neurofibromas showed high‐level expression of vascular endothelial growth factor (VEGF), a potent angiogenic factor involved in neovascularization. However, the molecular mechanism underlying the upregulation of VEGF in neurofibromas remains unclear. In the present study, we examined the effects of Nf1 gene silencing on VEGF expression in Schwann cell and non‐Schwann cell line and the up...

[Persistent post-occupational dermatitis: a rare variant of a common skin disease].
Abstract
Persistent post-occupational dermatitis is a rare variant of occupational eczemas characterized by skin symptoms which persist after the elimination of occupational allergens. Further diagnostic criteria are the followings: absence of dermatitis in past history of patients, presentation confined to both hands almost exclusively, and causal role of occupational allergens with both irritative and allergic origins is confirmed. The author presents a case which meets all these diagnostic criteria. Epidemiology of occupational dermatoses, impact on quality of life and social-economic aspects are also discussed....

Advances in the Diagnosis of Drug Eruptions.
Abstract
Drug eruptions affecting the skin or mucosas (toxicoderma) are the most common adverse effects of drugs and represent one of the more common diagnostic challenges for the dermatologist. A better understanding of the pathogenic mechanisms of drug reactions, pharmacogenetics, and pharmacoepidemiology will help us to resolve the main dilemmas and to anticipate and even prevent such reactions. Many drug eruptions are due to T cell-mediated hypersensitivity reactions that can involve activation of different proinflammatory mechanisms, which would explain the varied manifestations. Some aspects defy the classica...

Toxic Epidermal Necrolysis and Antifolate Drugs in Cancer Chemotherapy.
Abstract
Folates are one-carbon donors essential for synthesizing purines, pyrimidines, serine, and methionine. They correspond to anionic hydrophilic molecules essential to DNA synthesis in mammalian cells. The latter cells lack the capacity to synthesize folates. In some patients, high dosages of antifolate drugs (eg: methotrexate, pemetrexed) used in cancer chemotherapy alter the keratinocytes, endothelial cells and Factor XIIIa+ dermal dendrocytes in a range of various severities. Such conditions clinically designed under the heading antifolate cytotoxic skin reaction (ACSR) occasionally resemble the toxic epid...

Toxic Epidermal Necrolysis.
Abstract
Toxic epidermal necrolysis (TEN) is a severe mucocutaneous drug-induced syndrome that causes massive keratinocyte apoptosis and therefore hydro-electrolytic disorders and systemic infection. TEN approximately affects one to two cases per million per year. Mortality rate may reach thirty percent of cases. Thus, TEN constitute a therapeutic emergency at diagnosis. Typically, clinical examination shows a mucocutaneous detachment involving more than thirty percent of body area. Definitive diagnosis is made on cutaneous biopsy with histological exam that shows the blister of necrotic keratinocytes. Main differe...

Discoid lupus erythematosus at a site of previous injury.
Abstract
A 71-year-old man with three patches of discoid lupus erythematosus (DLE) confined to the right preauricular region drew our attention because of the unusual linear arrangement of the lesions. Twenty-five years previously, the patient had suffered a trauma in the same area from falling off his motorcycle. We believe that, despite the great lapse in time, this injury may have facilitated the onset of DLE in the very same area, through long-term destabilization of the local neuroimmune network. The case fits the recently coined concept of the immunocompromised district, a cutaneous region with altered immune...
