Cryptogenic Organizing Pneumonia During Adjuvant Chemotherapy With Oxaliplatin, 5-Fluorouracil, and Leucovorin (FOLFOX) for Colon Cancer
We describe a case of cryptogenic organizing pneumonia (COP) occurring in a 78-year-old woman after receiving 2 cycles of modified FOLFOX6 as adjuvant chemotherapy for treatment of resected nonmetastatic colon cancer. This patient presented with respiratory symptoms including cough with scant clear sputum and wheezing on day 10 of the second cycle of mFOLFOX6. Despite therapy with systemic antibiotics and supplemental oxygen, she had a steady and relentless progression of her respiratory symptoms and status, with chest radiographs revealing progressive bilateral pulmonary infiltrates. Further chest radiograph evaluation de...

Benign Prostatic Hyperplasia: Implications for Pharmacologic Treatment and Perioperative Care
This review provides an update on the treatment of benign prostatic hyperplasia and the pharmacologic considerations for perioperative care. By age 85 years, approximately 90% of men have prostate histologic characteristics consistent with benign prostatic hyperplasia. Pharmacologic treatment with an alpha1 receptor antagonist may reduce symptoms and, when given in combination with a 5-alpha-reductase inhibitor, may decrease the risk of urinary retention and the need for surgical intervention. Transurethral resection of the prostate has been the historical standard when surgical intervention is indicated. However, recent e...

Contemporary Anticoagulation Reversal: Focus on Direct Thrombin Inhibitors and Factor Xa Inhibitors
Several oral direct thrombin inhibitors (DTIs) and factor Xa (FXa) inhibitors have recent Food and Drug Administration approval or are under investigation in late-stage clinical trials for the prevention and treatment of thromboembolic events. Rapid reversal of anticoagulation is typically recommended in patients with severe or life-threatening bleeding and in patients requiring surgery or invasive procedures. However, no antidote exists for DTIs or FXa inhibitors though replacement of coagulation factors using clotting factor concentrates is routinely considered in some clinical scenarios. Clotting factor concentrates ava...

Awareness and Perceptions of Vermont's Prescribed Product Gift Ban and Disclosure Law by Prescribers and Pharmacists
Conclusions:
A high proportion of Vermont providers are unaware of the current law. Most agreed or had no strong opinions about the requirements; however, at least a quarter disagreed with banning small gifts and meals. Having any interaction with pharmaceutical representatives changed how providers perceived the requirements. These data may be useful for other states considering similar laws.

Asthma Education in the Student Laboratory
The Integrated Pharmacy Skills Laboratory (IPSL) at South University School of Pharmacy is dedicated to providing future pharmacists with the necessary knowledge and skills to care for asthma patients. Asthma is a chronic respiratory disorder that is characterized by airway inflammation, hyper responsive airways, and bronchoconstriction. It affects more than 30 million people and accounts for $30 billion in health care costs. Therefore, it is vital for our students to be educated in asthma management including medications and devices to improve patient outcomes. Students, as future pharmacist, learn to manage asthma patien...

Combination Pharmacotherapy in the Treatment of Pulmonary Arterial Hypertension: Continuing Education Article
Pulmonary arterial hypertension (PAH) is a disorder of the small pulmonary arteries characterized by progressive fibrotic and proliferative changes that result in an increased pulmonary vascular resistance. It is a progressive and debilitating disease that leads to right ventricular dysfunction, impairment in activity tolerance and eventually right-sided heart failure, and premature death. The treatment goals for PAH include improvement in symptoms, improvement in functional class and exercise class, decreased morbidity, and preventing mortality. Combination therapy in the treatment of PAH is an emerging therapeutic option...

Pharmacotherapy of Lung Transplantation: An Overview
Lung transplantation has become a viable treatment therapy for end-stage lung disease patients. The most common etiologies of end-stage lung disease, which can require a transplant are chronic obstructive pulmonary disease (COPD), idiopathic pulmonary fibrosis (IPF), pulmonary arterial hypertension (PAH), and pulmonary fibrosis (PF). Listing criteria are institution and program specific. Approximately 1500 lung transplants were performed in 2008; and at 5 years post transplant, one-half are expected to survive. The surgery itself is associated with various complications, including surgical, infectious, and mechanical. Immu...

Application of Physicochemical Properties and Process Parameters in the Development of a Neural Network Model for Prediction of Tablet Characteristics.
Abstract
The importance of in silico modeling in the pharmaceutical industry is continuously increasing. The aim of the present study was the development of a neural network model for prediction of the postcompressional properties of scored tablets based on the application of existing data sets from our previous studies. Some important process parameters and physicochemical characteristics of the powder mixtures were used as training factors to achieve the best applicability in a wide range of possible compositions. The results demonstrated that, after some pre-processing of the factors, an appropriate prediction p...

Mesalazine granules once-daily dose provides similar results to twice-daily dose in patients with active, left-sided ulcerative colitis.
The objective of the MOTUS study was to show non-inferiority of once-daily mesalazine versus twice-daily dosing for the induction of remission in active ulcerative colitis. As many patients with ulcerative colitis experience left-sided disease, the aim of the new sub analysis was to assess specifically the efficacy in patients with left-sided ulcerative colitis.

After adjustment for Paediatric Index of Mortality (PIM)-2 score and centre, children who received early parenteral nutrition were about twice as likely to die as those who did not (odds ratio 2.11, p=0.0002).
Also, median length of stay was significantly longer for th...

NHS Choices assessment of press report that women's lung cancer death rates set to rise
Source: NHS Choices
Area: News
NHS Choices has assessed the study, published in Annals of Oncology, which predicted cancer deaths from all forms of cancer across the European Union in 2013, and reported that lung cancer is now the leading cause of cancer-related deaths in British women.
The rise in lung cancer deaths may be caused by:
. the long-term effect of the greater number of women smokers during the 1960s and 70s . the fact that lung cancer remains challenging to treat, as it is often not diagnosed until it is at an advanced stage.
The study also found that between 2009 and 2013, overall death rates...

According to a report in Biospace, an open-label, randomised phase II study of bavituximab in combination with gemcitabine in patients with previously untreated, advanced stage IV pancreatic cancer resulted in more that a doubling of overall response rate (ORR) and a marginal improvement in overall survival (OS) when compared with gemcitabine alone (control arm).
In the trial which enrolled 70 patients, patients treated with a combination of bavituximab and gemcitabine had a 28% tumour response rate as compared to 13% in the control arm. Median OS, the primary endpoint of the trial,...

The best available evidence to support the use of lenalidomide and dexamethasone for the treatment of relapsed or refractory multiple myeloma comes from two phase III studies (MM-009 and MM-010).
