Long‐term effects of melatonin on quality of life and sleep in hemodialysis patients (Melody study): a randomized controlled trial
ConclusionsThe high drop‐out rate limits the strength of our conclusions. However, although a previous study reported beneficial short‐term effects of melatonin on sleep in hemodialysis patients, in this long‐term study the positive effects disappeared during follow up (6‐12 months). Also the quality of life parameter vitality did not improve. Efforts should be made to elucidate the mechanism responsible for the loss of effect with chronic use.

Ketamine for Chronic Pain: Risks and Benefits
Summary The anesthetic ketamine is used to treat various chronic pain syndromes, especially those that have a neuropathic component. Low‐dose ketamine produces strong analgesia in neuropathic pain states, presumably by inhibition of the N‐methyl‐D‐aspartate receptor although other mechanisms are possibly involved, including enhancement of descending inhibition and anti‐inflammatory effects at central sites. Effect of repeated doses of netazepide, a gastrin receptor antagonist, omeprazole and placebo on 24‐h gastric acidity and gastrin in healthy subjects
ConclusionAlthough repeated doses of netazepide led to tolerance to its effect on pH, the accompanying increase in plasma gastrin is consistent with continued inhibition of acid secretion, via gastrin receptor antagonism and gene up‐regulation.

Neuropeptide receptors as potential drug targets in the treatment of inflammatory conditions

Population pharmacokinetics of rituximab with or without plasmapheresis in kidney patients with antibody‐mediated disease
ConclusionsPlasmapheresis removes an important amount of rituximab when performed less than three days after infusion. The removal of rituximab led to a significant decrease of AUC. This pharmacokinetic observation should be taken into account for rituximab dosing: e.g., an additional third rituximab infusion may be recommended when three plasmapheresis sessions are performed after the first rituximab infusion.

Netazepide, a gastrin/CCK2 receptor antagonist, causes dose‐dependent, persistent inhibition of the responses to pentagastrin in healthy subjects
ConclusionsNetazepide is an orally active, potent, competitive antagonist of human gastrin/CCK2 receptors. Antagonism is dose‐dependent and persists during repeated dosing, despite tolerance to the effect on pH. Further studies are required to explain that tolerance. Netazepide is a tool to study the physiology and pharmacology of gastrin, and merits studies in patients, to assess its potential to treat gastric acid‐related conditions and the trophic effects of hypergastrinaemia.

The association between Parkinson's disease and anti‐epilepsy drug carbamazepine: a case‐control study using the UK General Practice Research Database
ConclusionsThere was little to no evidence that use of carbamazepine is associated with reduced risk of Parkinson's disease. Although the study was underpowered, it does indicate that any effect of carbamazepine is likely to be small.

The Crohn's disease activity index (CDAI) is similarly elevated in patients with Crohn's disease and in patients with irritable bowel syndrome
ConclusionsOur study demonstrates that the CDAI does not discriminate patients with symptoms due to active Crohn's from patients with IBS. Patients with IBS can have CDAI scores in the clinically meaningful range. Objective measures, such as CDAI haematocrit score and CRP, are more specific markers of inflammation.

The use of prophylactic gastroprotective therapy in patients with nonsteroidal anti‐inflammatory drug‐ and aspirin‐associated ulcer bleeding: a cross‐sectional study
ConclusionsA substantial proportion of high‐risk NSAID and ASA users had not received prophylaxis with gastroprotective agents before ulcer bleeding. These bleeding episodes may be preventable with better adherence to gastroprotective agent use.
