Patients appearing to dental professionals with orofacial pain arising from intracranial tumors: a literature review
Background: A small proportion of patients with orofacial pain appearing to dentists and dental specialists will have intracranial tumors as the underlying cause. These patients may undergo unnecessary dental interventions before the correct diagnosis is made. Method: A search of the literature using the PubMed database was performed to identify case reports of this occurrence. Cases were analyzed for common characteristics or presenting features that may aid dentists in identifying patients with intracranial tumors. Results: Twenty-eight cases were identified. Features consistent with the diagnoses of trigeminal neur...

Is early extraction really the silver bullet to avoid nerve injury in lower third molar removal?
I read the article ‘Early extraction: a silver bullet to avoid nerve injury in lower third molar removal?’ by Zhang et al. with great interest. The authors concluded that ‘lower third molar removal before the age of 23 is effective at avoiding some postoperative complications, especially nerve injury’. In their study there was no reversible paresthesia in the young patient group (≤23 years), suggesting that operations are ‘nerve injury free’, and safe in this group.

Dermatomyositis: a contemporary review for oral health care providers⁎
Dermatomyositis (DM) is an autoimmune muscle disorder characterized by skin rashes and progressive muscle weakness. This disease is a subset of the idiopathic inflammatory myopathies (IIM) and affects both children and adults. Increased incidence of malignancy has been observed in adult DM, making early diagnosis, treatment, and monitoring crucial. In the past, limitations of these diagnostic criteria and classifications often made it difficult to identify the disease in the early stages. However, in recent years a new understanding of the pathogenesis and clinical features of DM has developed, which enables clinicians to ...

A National Study on Craniosynostosis Surgical Repair.
Conclusions :  Craniosynostosis surgery is safe with low rates of mortality and acute complications. LOS >4.2 appears to be associated more with comorbidities than with complications. Higher rates of comorbidities and LOS >4.2 days for patients age 1 to 3 years warrant addition research to assess potential barriers to care. PMID: 23030675 [PubMed - as supplied by publisher]

Causal Attributions of Cleft Lips and Palates Across Cultures.
Conclusions :  This study provides evidence that causal attributions for clefts are influenced by culture. As harmful beliefs about cause may continue to impact affected individuals and their families even after a repair, it is insufficient to provide surgical care alone. Care of the entire person must include attempts to change misinformed cultural beliefs through educating the broader community. PMID: 23030676 [PubMed - as supplied by publisher]

The Perspectives of Children and Young People Living With Cleft Lip and Palate: A Review of Qualitative Literature.
Conclusions :  This review demonstrates that there is a paucity of evidence about the experiences of young people living with CL/P. No studies of children and only two studies of young people met all inclusion criteria. Identified papers implied that more attention is needed within families and services to help young people manage everyday difficulties such as bullying and self-consciousness due to facial difference. Scoring Systems of Cleft Related Facial Deformities: A Review of Literature.
Conclusions :  Although there is a wide variety in study design, 3D imaging seems most reliable in assessing cleft-related facial deformities. However, scoring on 2D photographs is easier to perform and more applicable in daily practice because all cleft patients are photographed through their treatment. The role of positron emission tomography and positron emission tomography/computed tomography in thyroid tumours: an overview
Abstract  Positron emission tomography (PET) and PET/computed tomography (PET/CT) with different tracers have been increasingly used in patients with thyroid tumours. The aim of this article is to perform an overview based on literature data about the usefulness of PET imaging in this setting. The role of Fluorine-18-Fluorodeoxyglucose (FDG) PET and PET/CT in differentiated thyroid carcinoma (DTC) is well established, particularly in patients presenting with elevated serum thyroglobulin levels and negative radioiodine whole-body scan. Iodine-124 PET and PET/CT may serve a role in staging DTC and obtaining les...

LigaSure small jaws versus cold knife dissection in superficial parotidectomy
The objective of this study was to evaluate the use of LigaSure small jaws in superficial parotidectomy for blood loss, operative time and facial nerve function. This was a prospective non-randomized study of 35 patients with a benign parotid tumor. Sixteen patients had LigaSure small jaws parotidectomy, and 19 patients had cold knife parotidectomy. Operative time, blood loss, facial palsy and other complications were assessed. The use of LigaSure was associated with a significant reduction in mean operative time (128 min vs. 155, p = 0.04) and intraoperative blood loss (40 mL vs. 115 mL, p&nbs...

Temporomandibular joint arthritis in sickle cell disease: a case report
We report a rare case of aseptic arthritis in the temporomandibular joint of a patient with sickle cell anemia. A 22-year-old woman with sickle cell disease, in the 18th week of gestation, was referred by her hematologist to investigate a sudden mouth opening limitation and severe pain on her left cheek. The patient received a standard pain assessment protocol, clinical examination, and complementary exams (complete blood count, hemoglobin electrophoresis, blood solubility test, panoramic radiograph, and magnetic resonance imaging [MRI]). The blood results were consistent with a sickle cell crisis and the MRI showed an inf...

Powered saline irrigation is useful for endoscopic removal of fungus balls in maxillary sinuses
Paranasal sinus fungus balls (mycetomas) are an extramucosal mycosis that usually affects immunocompetent people and are most frequently found in the maxillary sinuses.1‐3 The current gold standard treatment is to completely remove the fungus balls in order to improve the aeration of the involved sinuses.1,3 Endoscopic middle meatal antrostomy is the most popular approach to manage maxillary sinus fungus balls. © 2012 Blackwell Publishing Ltd

Kaczmarzyc et al.: A systematic review of the recurrence rate for keratocystic odontogenic tumour in relation to treatment modalities
I read with great interest the paper by Kaczmarzyk et al. The authors should be commended for their mammoth task of reviewing the existing literature on this topic and trying to put the results of the many retrospective case series into perspective. At the same time, however, I was somewhat taken aback by the harsh tone of their paper suggesting that all 168 papers reviewed, except two, had not contributed to existing knowledge on the recurrence tendency of keratocystic odontogenic tumour (KCOT). That certainly is not the case and does not acknowledge all the authors who have struggled with this intriguing pathological ent...

Tonsillectomy healing.
CONCLUSIONS: Edema and pain are greatest immediately after surgery, probably as a result of thermal effects and expression of inflammatory mediators that stimulate pharyngeal nociceptors. Pain gradually decreases over time, with an increase in analog pain measures on postoperative days 3 to 5 corresponding to the maximal wound inflammation documented in experimental models. Epithelial ingrowth beneath a fibrin clot begins shortly after wounding. Separation of the fibrin clot about 7 days after surgery exposes vascular stroma. Involution of the vascular stroma and completion of epithelial coverage correlate with decreased p...

Efficacy of computed tomographic scans in the evaluation of patients with esophageal foreign bodies.
CONCLUSIONS: CT scanning appeared to be sensitive and specific in investigation of patients with an ingested foreign body.
