Anaphylactic reaction following administration of nose drops containing benzalkonium chloride
We describe a case of anaphylactic reaction in a 46-year-old female post application of decongestant nose drops containing benzalkonium chloride (BAC). With some latency, the patient complained of cough, dyspnea, sensation of heat, croakiness and pruritus. Since she showed all of these symptoms, typical of an anaphylactic reaction, we proceeded some weeks later with a prick test with solutions containing BAC, a cationic surfactant commonly used as an antibacterial preservative in many medical solutions. The prick test was positive, confirming the assumption of a hypersensitive reaction to BAC.

Gustatory Sweating in the Submandibular Region Following Neck Dissection: A Case With Thermographic Evaluation and Review of the Literature
Gustatory sweating is a fairly common complication after surgery or injury of the parotid gland. The pathogenesis is based on aberrant nerve regeneration between the parasympathetic fibers of the salivary glands and the sympathetic fibers of the cutaneous sweat glands or blood vessels. Inappropriate innervation leads to local cutaneous sweating and vasodilation during mastication. A similar condition has been reported in the cervical region after neck surgery, such as neck dissection, submandibular gland excision, and cervical lymphadenectomy, but the frequency of this condition is extremely rare. Since 1934, when the orig...

Evaluation of the Mandibular Ramus Fracture Line After Sagittal Split Ramus Osteotomy Using 3-Dimensional Computed Tomography
Conclusion: A desirable splitting pattern occurred when a short lingual cut just above the lingula and a lateral bone cut of the mandibular angle were made, extending to the inside through the inferior border of the mandible. These observations also proved that the split patterns of the mandibular ramus could be controlled by the position of the lateral bone cut end.

Development and Demonstration of a Novel Computer Planning Solution for Predefined Correction of Enophthalmos in Anophthalmic Patients Using Prebended 3D Titanium-Meshes—A Technical Note
Ablative surgery of the orbit is often associated with dramatic changes in facial geometry. Surgical intervention is often necessary to correct the functional and esthetic appearance in those patients who are anophthalmic, having an intact eyelid appearance and an orbital prosthesis. The outcome of the surgical correction depends on the shape of the orbital implants and their adequate placement. In the case of comparatively small rearrangements, the effect of implants on soft tissues can be estimated by surgeons on the basis of their experience. However, large deformities in complex cases (including large deformation of so...

Novel Technique to Repair Maxillary Sinus Membrane Perforations During Sinus Lifting
After the first report of maxillary sinus lifting in an atrophic maxilla by Boyne and James, sinus lifting for implant placement progressed rapidly to become an established procedure in implantology. It is a predictable method used when augmenting the bone in the posterior maxilla is necessary. The increase in sinus lifting procedures and bone grafting for the purpose of implant placement in the posterior maxilla has in turn increased the rate of complications. The most common complication during open sinus lifting is perforation of the sinus membrane during surgery; if left untreated, this may result in loss of graft mate...

National Dental Boards Go Pass/Fail—Why It Matters to Oral and Maxillofacial Surgery
All oral-maxillofacial surgeons with full licenses to practice dentistry in the United States had to pass national dental board examinations at some point. If you were like me, you worked hard preparing for these examinations because of their high-stakes nature, ie, you could not be licensed without a passing score. Also, because it was and remains common for oral-maxillofacial surgery (OMS) program directors to use the results of board scores as a key discriminator in their residency admissions process, those seeking entrance into a desired program are especially keen to obtain a Part I board score in the 90s or at least ...

Effects of temporal fine structure stimulation on Mandarin speech recognition in cochlear implant users.
Conclusion: The investigated experimental coding strategies significantly improved tone identification as compared with the participants' everyday audio processor settings. However, this benefit could not be attributed entirely to temporal fine structure stimulation but seems to be caused by decreasing the lower corner frequency of the filter bank. The 6 week habituation period used in this study might have been too short to allow the listeners to derive additional lexical information from the unfamiliar stimulation patterns. Objectives: To evaluate a cochlear implant coding strategy that explicitly conveys temporal fine s...

Temporal Processing in the Auditory System
Abstract  Central auditory processing in humans was investigated by comparing the perceptual effects of temporal parameters of electrical stimulation in auditory midbrain implant (AMI) and cochlear implant (CI) users. Four experiments were conducted to measure the following: effect of interpulse intervals on detection thresholds and loudness; temporal modulation transfer functions (TMTFs); effect of duration on detection thresholds; and forward masking decay. The CI data were consistent with a phenomenological model that based detection or loudness decisions on the output of a sliding temporal integration win...
