Jacob disease, or osteochondroma of the mandibular coronoid process, is a benign skeletal tumor that is a rare entity in the oral and maxillofacial region. Though first noted by Langenbeck in 1853 as an enlargement of the mandibular coronoid process, the first case report was described by Jacob in 1899, who reported a pseudojoint between the coronoid process and the zygomatic arch due to an osteochondroma formation of the coronoid process. At present, there have been 35 known cases of this condition. This case report presents an additional number to that group.

Sagittal split ramus osteotomy of the mandible (SSRO) is perhaps the most common mandibular osteotomy performed around the world. The broad area of bone overlap that occurs with this procedure facilitates the application of stable internal fixation devices. The SSRO is currently stabilized by the use of positional screws and/or miniplates. Typically, 3 positional screws are used that engage the buccal cortex of the proximal segment and the lingual cortex of the distal segment. The miniplate technique, introduced by Michelet et al, uses miniplates with monocortical screws attached to the buccal cortex of the proximal and distal segments.

We read with great interest the study report by Farah et al, comparing 2 pharmacologic protocols (Clonidine associated with remifentanil and dexmedetomidine associated with isoflurane) for induced arterial hypotension in patients undergoing orthognathic surgery. The authors showed that no significant differences were present between the 2 protocols regarding intraoperative blood pressure, heart rate, surgical bleeding, and awakening and extubation time. Moreover, 10 patients in the Clonidine-remifentanil group required esmolol to control the heart rate versus 2 patients in the dexmedetomidine-isoflurane group.

Miloglu and Hororli are absolutely right when they point out that an HU value of 0 corresponds to water, whereas the HU value for air is defined as −1,000. We measured values from −1,024 to −865 HU for the surrounding air in computer tomographic image data. This is an error in our report that escaped our proofreading. We accept the responsibility for the error, and we want to thank Miloglu and Harorli for their diligent reading. Hence, the first paragraph of the Results section (page 1882) should be corrected.

Conclusions: Our results indicate that in the leukocytes of patients with stage II and III SCC, the p38 MAPK pathway performs an essential role in the induction of iNOS expression, and the process of lipid peroxidation is not dependent on NO. In contrast, in patients with advanced-stage SCC, iNOS expression did not seem to be linked with the p38 MAPK pathway, and NO directly influenced the process of lipid peroxidation.
