Balloon Dilatation of the Paranasal Sinuses: A Tool in Sinus Surgeryemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Chronic sinusitis affects millions of patients. Balloon technology is a tool that has enhanced the surgeon's ability to treat patients suffering from chronic sinusitis. Three companies have developed products to dilate sinus ostia. An extensive literature review reveals that balloon catheters have an impressive safety profile and seem to be an effective tool for ostial dilatation. These tools are particularly effective in frontal recess dilatation and have great potential for office use under local anesthesia.

Chronic rhinosinusitis (CRS) is the single most common self-reported chronic health condition in the United States and is estimated to affect 16% of the adult population annually. Despite the prevalence of this disease, there still exists an incomplete understanding of CRS pathophysiology. In this review, the authors highlight technological advances in rhinology: real-time polymerase chain reaction, epithelial cell culture, flow cytometry, genomics/single-nucleotide polymorphism detection, microarrays, and genetic/nongenetic animal models of sinusitis. The purpose of this review is to describe these methodologies and their...

Neuroendoscopic surgery encompasses minimally invasive approaches to the skull base using endoscopic techniques. There are unique technologic challenges with endoscopic endonasal skull base surgery, such as a limited working space, difficulty in visualization and identification of neurovascular structures and removal of tissue, hemostasis, and dural reconstruction. Technologic advances that have enabled this surgery include specialized operating suites, neurophysiologic monitoring, imaging and visualization technologies, dissection instrumentation, hemostatic materials, and reconstructive materials. Advances in each of the...

Complications of midline-open tracheotomy in adultsemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Percutaneous tracheotomy is progressively replacing open tracheotomy as a consequence of promising results of comparative studies. However, this comparison has four considerable weaknesses: 1) selected indications (high-risk patients excluded for percutaneous tracheotomy); 2) varying spectra of complications included in different studies; 3) varying operative settings (experienced surgeons exclusively, surgeons in training, or both); and 4) missing differentiation between different surgical techniques. Our study was performed to collect complete datasets of unselected patients who all underwent a tracheotomy in a uniform t...

Nasolabial cyst is a rare epithelial and non-odontogenic cyst of the jaw. It is situated behind the ala nasi, extending backwards into the inferior nasal meatus and forward into the labio-gingival sulcus. Predominant symptoms are swelling of the nasal vestibule, local pain, and nasal obstruction. Radiology is not specific and CT scan may be contributive. Surgical excision is the first line treatment; it proves the diagnosis and prevents recurrence. PMID: 19800645 [PubMed - as supplied by publisher]

We report a case involving a 49-year-old woman with vascular loop syndrome. The patient was evaluated because of complaints of decreased hearing sensitivity in her right ear. Central auditory tests were performed. The patient was found to have an asymmetry on pure-tone audiometry and an auditory processing deficit, suggesting central pathology. Magnetic resonance imaging confirmed central involvement, revealing a vascular loop extending into the right internal auditory canal. This case report demonstrates that auditory deficits may result from vascular loop compression and that these deficits may benefit from a battery of ...

We report a rare case of an intrathyroidal fourth branchial pouch sinus in an adult. A 36-year-old man presented with a recurrent left-sided deep neck abscess. A diagnostic evaluation by barium pharyngoesophagography in conjunction with computed tomography revealed the presence of a sinus tract that extended inferiorly from the left piriform sinus to the left thyroid lobe. The synchronous use of both of these imaging modalities with shared contrast enabled us both to establish the diagnosis and formulate a successful surgical approach. The patient underwent elective neck exploration with resection of the left thyroid lobe ...

Postauricular cutaneous mastoid fistula secondary to chronic suppurative otitis media (CSOM) is an unusual complication. Bilateral manifestation along with primary acquired cholesteatoma and atelectasis of the pars tensa as an end-stage complication in the natural course of atticoantral CSOM is rare. This kind of complication has a very morbid effect on the ear, and it poses a therapeutic challenge in terms of eradicating disease and restoring function. Conclusions  If implants are placed in severe atrophic mandible, iatrogenic fracture of the mandible may occur during or after implant surgery because implant placement weakens the already-compromised mandible. A few millimeters of cortical bone should remain on both the labial and the lingual sites after the hole for insertion of an implant has been drilled. A 3-D surgical planning should be recommended at least in severe atrophic mandibles in order to prevent a severe reduction of bone tissue. We examined two rare cases of postmortem brainstem tissue from SD patients compared to four controls. In the SD patients, small clusters of inflammation were found in the reticular formation surrounding solitary tract, spinal trigeminal, and ambigual nuclei, inferior olive, and pyramids. Mild neuronal degeneration and depigmentation were observed in the substantia nigra and locus coeruleus. No abnormal protein accumulations and no demyelination or axonal degeneration were found. These neuropathological findings may provide insights into the pathophysiology of SD. Laryngoscope, 2009
