Unintentional parathyroidectomy is a complication of thyroid surgery. To our knowledge, no study has specifically examined the incidence of inadvertent parathyroidectomy exclusively in patients undergoing secondary central compartment surgery for recurrent or persistent thyroid cancer. The records of 40 patients who underwent 42 secondary central compartment surgeries for thyroid cancer were reviewed to determine the incidence of inadvertent parathyroidectomy.Parathyroid tissue was present on permanent pathology in 31% of SCCSs. Unintentional parathyroidectomy did not have an effect on hypoparathyroidism. Concomitant latera...

In this study, we investigate the usefulness of combining calcium and PTH to increase the predictive power.Individual patient data were obtained from 3 studies (152 patients) that fulfilled our criteria (using PTH assay within hours postthyroidectomy to predict symptomatic hypocalcemia).Changes in combined PTH and calcium threshold levels checked 1 to 6 hours after thyroidectomy were excellent in predicting postoperative hypocalcemia. A decrease in PTH of 60%, coupled with a simultaneous decrease in calcium of 10%, 5 to 6 hours postoperatively resulted in a sensitivity and specificity of 100%. However, combined PTH and cal...

We present the case of a 21-year-old woman who underwent endoscopy-assisted surgery with topical mitomycin for a large cholesterol granuloma in the petrous apex that involved the eustachian tube.Case report.Minimally invasive surgery was performed on the cholesterol granuloma with endoscopic assistance, and topical mitomycin (0.4 mg/mL) was administered to prevent scarring of the newly created drainage/ventilation route. With the endoscopic approach, the drainage/ventilation route was secured not only from the tympanic cavity, but also using a guidewire inserted from the pharyngeal orifice of the eustachian tube. The draina...

To report outcome for patients treated with radiotherapy (RT) for basal cell carcinoma of the medial canthus.Retrospective review.Thirty-three patients treated with RT at the University of Florida between 1965 and 2005 for basal cell carcinoma of the medial canthus were retrospectively reviewed. RT was the primary treatment for gross disease in 70% of patients and for positive margin after resection in 30%. The prescribed dose was 50 to 60 Gy at 2.0 to 2.5 Gy per fraction.Surviving patients were followed for a median of 14 years. Tumor recurred at the primary site in 10%. Five-year outcomes will be reported, and we will revisit a previous analysis of the effectiveness of extensive surgery in the treatment of nasal polyposis.Baseline clinical data was collected for 3,128 patients undergoing surgery for CRS (with or without nasal polyps). Outcomes are described in terms of the proportion of patients undergoing revision surgery and mean Sino-Nasal Outcome Test (SNOT-22) scores.A total of 1,459 (52.2%) patients r...

I attributed a quote in my May 2009 essay to Jim Collins, “If you don't like change, you'll hate being irrelevant.” However, upon checking with Mr. Collins' staff, I learned that he did not coin this phrase. They pointed me to a similar quotation by General Eric Shinseki: “If you don't like change, you're going to like irrelevance even less,” which appears in the Fast Company (publisher) book, The Rules of Business: 55 Essential Ideas to Help Smart People (and Organizations) Perform At Their Best, published in 2005. Identification and treatment of extra canals is the cornerstone to successful endodontic practice. Over the years, many diagnostic aids have evolved to facilitate the clinician in detection of hidden canals and to familiarize with the internal radicular anatomy. This case report highlights the rare incidence of 3 canals in the distal root of a permanent mandibular first molar. A spiral computerized tomography scan was used as a noninvasive diagnostic tool to confirm the diagnosis.

Osteochondroma is a benign tumor that is unusual in the maxillofacial region. Anatomy of the region, especially the mandible, is important for surgical intervention for the condylar osteochondroma. The present case report describes evaluation of condylar hyperplasia with emphasis on the advantages and disadvantages of sterolithographic solid model and stereologic techniques for preoperative surgical planning, operation time, and prognosis. Condylar osteochondoma was diagnosed by panoramic radiograph and 3-dimensional computerized tomography (3DCT) as well as histopathologic analysis in a male patient. Before surgery, a ste...

Thalassemia major is a rare inherited anemia, and affected children require blood transfusions every 2-4 weeks to survive. Repeated blood transfusions lead to a build-up of toxic levels of iron in the body, causing organ damage and premature death, primarily due to iron-induced heart disease. Deferiprone is one of a few drugs that are routinely used in medicine for the treatment of iron overload in thalassemic patients. This drug is usually administered daily at high doses (50-100 mg/kg) with a very low toxicity. Agranulocytosis is the most serious side effect of deferiprone, with a reported incidence of 0.6 per 100 patien...

Techniques for preserving alveolar bone height invariably include socket preservation grafting. Although immediate grafting of the socket has been shown to be effective versus not grafting, it is not without its limitations. Firstly, even with grafting there is still predictably some loss of alveolar height in the months following extraction. Secondly, grafting will only preserve the alveolar bone that remains immediately after the extraction. It stands to reason that atraumatic extraction ...

Areas of reduced vascularity, either as a result of trauma or anatomy represent a major challenge in reconstruction. This case study illustrates a treatment modality for those cases resistant to other established therapies in maintaining flap viability. Following a free tissue transfer to reconstruct an avulsed air lobe, poor tissue perfusion was noted within the first 24 hours which was resistant to localized interventions. Medical leech...

This article will demonstrate that the ameloblastic carcinoma may be misdiagnosed upon initial biopsy as ameloblastoma. The following two case reviews illustrate the discrepancies in diagnosis and subsequent treatment modalities. Ameloblastic carcinoma requires further work up and more extensive treatment including radiation and possible neck dissection. The initial presentation, work ups, treatments and outcomes will be discussed.

Presenter: Christopher Ogunsalu, MBBS, BDS, FRACDS, MSc.Med, FICS, FWACS Objective: To introduce and describe a new bone regeneration phenomenon which is related to obstruction of bone formation due to foreign body formation and reaction following conventional (single) guided tissue regeneration. Additionally the correlative validation of this osteo-obstructive phenomenon with histologic, histomorphometric and SPECT is described.
