Bizarre parosteal osteochondromatous proliferation (BPOP) was first described by Nora et al. in 1983. These rare lesions demonstrate specific radiological and histological features which distinguish them from other benign cartilaginous and bony tumours. The following case report describes the first documented case of BPOP presenting in the zygomatic bone, and the third reported case involving the facial skeleton.

An aesthetic incision for neck dissection

Introduction: Various incisions have been described for neck dissections since the original description by Criles in 1906. With time, surgeons have improvised and developed a preference for a particular one which works well in their hands. The prime concern is access, which can be impaired by certain incisions and potentially affect lymph node harvest. The other concern is of course aesthetics and despite the fairly extensive surgery that patients suffering from head and neck cancer undergo, scarring from the surgical wounds contributes to the morbidity.

Introduction: Recent work on dysplastic lesions has shown that promoter methylation may be a predictive biomarker for malignant transformation to squamous cell carcinoma. Saliva shows promise as a tumour surrogate, but has not been investigated in the context of dysplasia. We investigated two methods of saliva collection for DNA yield and quality, as determined by epigenetic marker analysis, and the logistics of collection and storage.

Introduction: Gastrostomy feeding is an important method of providing enteral nutrition in head and neck cancer patients. At the Royal Sussex County Hospital, a fast-track service has been established for the provision of radiologically inserted gastrostomies (RIGs) in head and neck cancer patients, in order that cancer treatment is not delayed. The purpose of this study was to evaluate this service and assess the complications of RIGs.

Background: Angiotensin-converting enzyme inhibitors (ACEI) are frequently used in the treatment of many cardiac conditions such as hypertension and congestive heart failure. It is the treatment of choice for a majority of patients in the United Kingdom. A well recognised side-effect of this treatment is angio-oedema. This can range from benign facial swelling to life threatening airway compromise. Most cases of angio-oedema occur within the first few weeks after commencing treatment, but delayed reactions can present months or years later. Isolated cases of life threatening post-operative angio-oedema in patients on long-...

We present the case of a 62-year-old woman who presented a histological diagnostic conundrum. The patient presented with a four year history of a slow growing painless papular lesion on her left nostril. On clinical examination, a firm, red lesion 15mm in diameter was located just lateral to the left nostril, penetrating deeply beneath the alar base to involve the nasal sill. It was palpable from the inner aspect of the lip. Biopsy suggested the diagnosis of basal cell carcinoma. This was compatible with the clinical appearance. However histological examination of the excised lesion showed that it was either a trichoblasto...

Oral cancer survival is related to stage at presentation. Despite advances in surgery and radiotherapy there has been no dramatic change in oral squamous cell carcinoma survival since 1980. “A patient presents with a lump in the neck” is a frequent examination question. This can represent a metastasised oral cancer and shows that historically that oral cancer (as well as other head and neck malignancies) present at an advanced stage.

Introduction: Angiogenesis has been extensively investigated in several tumour models; however, chemotherapeutic agents based upon these models have not been very effective. It is logical to assume this lack of efficacy is due to the host tumour interface. Furthermore it is a reasonable hypothesis that the transition between the host vasculature and tumour is not binomial but a gradual transition from tumour and mosaic vessels to host capillaries. The existence of such pure tumour and mosaic vessels would suggest the possibility of tumour vascular mimicry in the connecting vessels.

Introduction: Peri-operative blood transfusion is associated with reduced prognosis in a number of solid malignancies. We investigate its role in a head & neck squamous cell cancer cell line. Growth of these cell lines was analogous to endothelial growth. Direct exposure to transfusion products exaggerated this effect. It was logical therefore to assess the effects of anti-endothelial antibodies on this interaction.

However, excessive fibrous tissue intrusion or dislocation of the BSM occasionally interrupted bone regeneration. To avoid such problems, we examined CA membrane, which is useful for guided bone rege...

Myringoplasty is considered the simplest reconstructive procedure of the middle ear. There are many variations and types of myringoplasties. In the past, these variations were categorized according to the types of tissue and the way the tissue was fixated in the middle ear. Currently, the types of myringoplasties are differentiated only by the fixation of the transplant, because a majority of ear surgeons use only fascia, perichondrium, or cartilage. The percentage of successful healings after myringoplasties does not depend on the tissue used. More important is the condition of the middle ear at the time of the operation ...

The evident superficial vascular blood supply appeared to represent the vascular supply to the overlying mucosa. A review of the patient's medical history failed to reveal any relevant findings. No regional lymphadenopathy was observed.

We report a case of nonendodontically treated first upper premolar crown root fracture in which the palatal cusp fracture extended below the cementoenamel junction. Reattachment of the palatal cusp in its original position by acid-etch and flowable composite allowed the creation of a standard access opening as in an intact tooth, avoiding apex location errors and contamination of the root canal. During crown-lengthening surgery, the palatal cusp fragment was hollowed out and used as a natural individual matrix for placement of the core material.

Abstract: Orbital lymphatic malformations are benign cystic malformations of the lymphatic system. The present report shows two cases with symptoms of orbital complications of acute rhinosinusitis with proptosis, compressive optic neuropathy, loss of vision and cellulites in children. Magnetic resonance imaging (MRI) revealed a well-demarcated intraorbital mass with heterogeneous signal conformable with lymphatic malformation in both cases. A tumor extirpation was performed via lateral orbitotomy in both cases. Postoperatively the symptoms and especially the loss of vision improved completely. Histological analysis of the ...

Abstract: Inner ear malformations are frequently found in patients with congenital hearing loss. It has been reported that normal hearing is rare in patients with severe inner ear vestibular malformations. A 9-year-old boy had had complained of recurrent dizziness and disequilibrium for 2 months. Clinical and neuro-otological examinations showed peripheral involvement of the vestibular system, while audiological investigation was normal. High-resolution magnetic resonance imaging, with three-dimensional reconstruction, showed dysplasia of the bilateral lateral semicircular canals (LSCCs). Isolated vestibular malformation m...

The pathology of the ear is complex, which is not surprising given the profoundly complex physiology that underpins the working of a normally functioning ear. This book takes a very refreshing approach to the subject of chronic otitis media by inviting some of the foremost experts on the basic science of the ear, both in health and disease to contribute to a very pure approach to the subject.
