Subglottis stenosis in Wegener’s granulomatosis

Wegener’s granulomatosis (WG) is a multisystem autoimmune vasculitis of small to medium sized blood vessels. Etiology is unknown. Nectrotising granulomatous vasculitis with destruction of the walls and narrowing of the vessel lumen leads to disturbanses of appropriate organs function. Clasical WG includes symptoms of upper and lower respiratory tract and kidneys, but WG can affect any organ. In about 16-20% of patients subglottic stenosis (SGS) due to granulomatous inflammation and scarring occurs. Conclusion: Although salivary carriage of T. denticola may be a risk indicator for CP, its pathogenicity should not be exclusively endorsed to its detection in saliva, but it might be associated with the synergistic biological interaction of the bacterium with some demographic characteristics of the susceptible host.

Conclusions: The study demonstrated that: (1) the setting time of calcium-silicate cements may be effectively reduced; (2) the expansion is a water dependent mechanism owing to water uptake, because no expansion occurred in cements immersed in oil; (3) a correlation between setting time and expansion in water and PBS exists; (4) fluorine-containing cement showed a significant expansion in water and in PBS; (5) the immersion in FBS/PBS strongly reduced the expansion of MTA and fluoride-doped cement suggesting that fluid contamination (ie, blood) during surgical procedures may greatly affect the expansion of some calcium-silicate cements.

We read with interest, but ultimately some misgiving, the report of Farole and Jamal entitled “A Bioabsorbable Collagen Nerve Cuff (NeuraGen) for Repair of Lingual and Inferior Alveolar Nerve Injuries: A Case Series.” Their study attempts to assess the effect of surrounding a lingual or inferior alveolar nerve repaired via microsurgery with a biodegradable collagen nerve sleeve. Unfortunately, there are significant errors in the design of this study, preventing it from producing significant data.

The article entitled “Comparison of Methohexital and Propofol Use in Ambulatory Procedures in Oral and Maxillary Surgery” (J Oral Maxillofac Surg 66:1996, 2008) deserves commendation and comment. Collection of such extensive data over a 7-year period is remarkable and helpful to the clinician. It is also easy to question some of the reported findings and conclusions because of the massive amount of material accumulated. The opportunity for bias to intrude is almost inevitable.

I read with interest the article by Limeres et al, “Patients' perception of recovery after third molar surgery following postoperative treatment with moxifloxacin versus amoxicillin and clavulanic acid: A randomized, double-blind, controlled study,” describing the effect of antibiotic prophylaxis on patients' quality of life after third molar surgeries. Although the authors arrived at some valuable conclusions, the study also has some major shortcomings.

We read with considerable interest the American Association of Oral and Maxillofacial Surgeons position paper concerning bisphosphonate-related osteonecrosis of the jaws (ONJ). We believe some additional information needs to be addressed that might strengthen the recommendations proposed by the Task Force in the May 2009 issue.

In a recent publication, Allen and Burr summarized most published hypotheses regarding the etiopathogenesis of bisphosphonate (BP)-related osteonecrosis of the jaws (ONJ). Although the report by Allen and Burr is very well written and comprehensive, it was not possible for them to include 2 novel—very recently published—hypotheses that may be of interest to the journal's readers.
