Question: An 87-year-old woman presented to her cardiologist with a 4-week history of dysphagia and a 20-pound weight loss. Three years previously, she developed a 4-cm Stanford type B descending thoracic aorta aneurysm. She also had hypertension and vertebral compression fractures. Vitals and physical examination were normal. Hemoglobin, electrolytes, and calcium levels were normal. A chest CT was ordered (). She underwent an esophagogastroduodenoscopy ().

Question: A 67-year-old man with diabetes mellitus presented to our emergency department with hematemesis. He had undergone laparoscopic cholecystectomy 2 months previously. On arrival, his blood pressure was 97/50 mmHg and his heart rate was 101 bpm. Laboratory tests revealed a hemoglobin concentration of 9.3 g/dL (normal, 12.3–18.3), elevated urea (29 mg/dL; normal, 8.0–20.0), and normal creatinine (0.9 mg/dL). Liver function tests and prothrombin time were within normal limits. He denied taking aspirin or nonsteroid anti-inflammatory analgesics.

Conclusions: Drug-eluting bead TACE is becoming more widely utilized in primary and liver-dominant metastatic disease of the liver. Outcomes of success must be expanded beyond response rates because these are not a reliable surrogate for progression-free survival or overall survival. Ongoing clinical trials will further clarify the optimal timing and strategy of this technology.

Conclusions The results of the present study demonstrate that hepatic steatosis measured by echography is associated with carotid atherosclerosis
in a large population mostly carrying cardiovascular or metabolic risk factors, independently of MS, cardiovascular diseases,
diabetes mellitus and/or insulin resistance.

Our aim was to test the feasibility of utilizing a second endoscopic arm to improve the dissection. An Olympus prototype blind probe, with an external diameter of 6 mm and a 2.8-mm working channel, was used as a second endoscopic arm. Its purpose was to lift the lesion during dissection. The main endoscope served both to perform the dissection and to visualize the second endoscopic arm in the monitor.
