Medicine: Mind the Gap - Helping Smokers With Behavioral Health Comorbidity Requires a National Effort
Medicine: Mind the Gap is a lecture series that explores issues at the intersection of research, evidence, and clinical practice—areas in which conventional wisdom may be contradicted by recent evidence. From the role of advocacy organizations in medical research and policy, to off-label drug use, to the effectiveness of continuing medical education, the seminar series will aim to engage the National Institutes of Health community in thought-provoking discussions to challenge what we think we know and to think critically about our role in today’s research environment.
Dr. Williams will be speaking about the fact that ...

Sickle Cell Trait Interferes with the Diagnosis of Diabetes by A1C
Hemoglobin A1C is a form of glycated hemoglobin. With the worldwide explosion of the diabetes epidemic, it was welcome news in 2010 that the A1C assay had been standardized and that hemoglobinopathies such as sickle cell trait did not interfere with A1C interpretation. Therefore the American Diabetes Association and the International Diabetes Federation now recommend A1C as a test to determine glucose tolerance status including pre-diabetes and diabetes.
In this Director’s Lecture, data will be presented which contradicts the view that A1C is an appropriate test to use in Africans with sickle cell trait. As the prevalenc...

Role of Active Surveillance in the Management of Men With Localized Prostate Cancer (Day 3)
Prostate cancer is the second leading cause of cancer-related deaths among men in the United States. It is estimated that in 2010, approximately 32,000 American men died of prostate cancer and 218,000 were newly diagnosed with the disease. Most prostate cancers are detected by a blood test that measures prostate-specific antigen (PSA), a tumor marker. More than half of cancers detected with PSA screening are localized (confined to the prostate) and are not aggressive at diagnosis, and are unlikely to become life-threatening. However, 90 percent of patients receive immediate treatment for prostate cancer, such as surgery or...

ARCHIVE ONLY: Identifying Core Behavioral and Psychosocial Data Elements for the Electronic Health Record
The HITECH Act and the Patient Protection and Affordable Care Act place new emphasis on the widespread and meaningful use of electronic health records (EHRs). This is an important advance, with one significant exception: Currently EHRs fail to capture data reflecting crucial health behaviors and psychosocial issues. Such patient-reported variables are both health outcomes themselves, and major determinants of other health outcomes. Capturing a core set of standard patient-reported variables in the EHR would lead to unprecedented data harmonization and opportunities for improving patient care and health research.

Social Context as Risk Regulator: Extending the Stream of Causation in Health Research
Causation is the central problem of health research. Our causal discourse shapes what we study, and how and where we intervene. The potential outcomes model is the most widely accepted causal framework in Epidemiology. However, many “upstream” factors of greatest interest to social scientists are incompatible with this framework. This contributes to a narrow focus on individual-level “down-stream” risk factors and to ineffective interventions that target individuals while ignoring social context. This lecture will review causal discourse in light of the stream of causation metaphor, and suggest the concept of a ri...
