Medical results of preparticipation examination in adolescent athletes
Conclusion Eligibility for competitive sports is restricted in only 5.5% of adolescent athletes at age 12. Eligibility refusals are rare. However, recommendations for therapy and prevention are frequent, mainly regarding the musculoskeletal system. In spite of time and cost consumption, adolescent preparticipation before entering a career in high-performance sports is supported.

Metabolic markers or conditions preceding Parkinson's disease: A case‐control study
AbstractSeveral metabolic markers or conditions have been explored as possible risk or protective factors for Parkinson's disease (PD); however, results remain conflicting. We further investigated these associations using a case–control study design. We used the medical records–linkage system of the Rochester Epidemiology Project to identify 196 subjects who developed PD in Olmsted County, Minnesota, from 1976 through 1995. Each incident case was matched by age (±1 year) and sex to a general population control. We reviewed the complete medical records of cases and controls in the medical records–linkage system to ab...

Animal Biowarfare Research: Historical Perspective and Potential Future Attacks
SummaryA biological attack on livestock or poultry could result in the loss of valuable animals, costs related to the containment of outbreaks and the disposal of carcasses, lost trade and other economic effects involving suppliers, transporters, distributors and restaurants; however, it is not possible to secure all livestock, and livestock are much less well guarded than human targets. Thus, the vulnerability of the livestock industry to the introduction of biological agents varies for the following reasons: (i) the majority of lethal and contagious biological agents are environmentally resilient, endemic in foreign coun...

Innovation and Innovators: Does It Take 10,000 Hours?
I would like to talk this afternoon about innovations and innovators and what might be common themes for both. Innovation in surgery has a rich tradition, and the interest and enthusiasm to innovate has never been greater. The combination of new ideas and hands-on applications is a central tenet of surgery and can explain the historical success of surgeons as innovators, as well as the progress that the innovations have created.

Structure and Age Jointly Influence Rates of Protein Evolution
We report that previously observed structural correlations also hold within each age group – including relationships between solvent accessibility, designabililty, and evolutionary rates. However, age also plays a crucial role: age modulates the relationship between solvent accessibility and rate. Additionally, younger proteins, despite being less designable, tend to evolve faster than older proteins. We show that previously reported relationships between age and rate cannot be explained by structural biases among age groups. Finally, we introduce a knowledge-based potential function to study the stability of proteins th...

Fifty years of Weibel–Palade bodies: the discovery and early history of an enigmatic organelle of endothelial cells1
Summary.  In 1962, a rod‐shaped cytoplasmic organelle of endothelial cells, later called the Weibel–Palade body, was serendipitously discovered by electron microscopy. It contains a set of parallel tubules and is wrapped in a membrane. Subsequent studies in the following decades established the unique localization of this organelle in endothelial cells of all vertebrates studied, meaning that it could serve as a marker of endothelial cells in tissue cultures. However, these studies did not reveal its functional significance, except for an indication that it could be related to an undefined thromboplastic substance. T...

Sorafenib or sunitinib as postoperative adjuvant therapy for Chinese patients with locally advanced clear cell renal cell carcinoma at high risk for disease recurrence.
CONCLUSIONS: Targeted adjuvant therapy postoperatively with sorafenib or sunitinib in patients with CC-RCC who are at a high risk for disease recurrence was well tolerated and effective in reducing the rate of CC-RCC recurrence in these patients. This study is an attempt to assess the utility of adjuvant tyrosine kinase inhibitors (TKIs) after surgery for renal carcinoma. The apparently improved outcomes, compared with a historical control population, are of sufficient interest to support the continuation of an ongoing randomized clinical trial to validate the hypothesis. [News] New treatment options in advanced bladder cancer
In a multicentre phase 3 trial of 360 patients with muscle-invasive bladder cancer, British researchers compared radiation therapy alone against radiation and synchronous chemotherapy with fluorouracil and mitomycin C and found that patients given the combination lived longer than did those on radiation alone. Moreover, patients assigned to combination therapy lived as long as did historical controls who had cystectomy, the gold standard. At 2 years, rates of locoregional disease-free survival were 67% for combination therapy and 54% for patients receiving radiation alone, and at 5 years, overall survival was 48% versus 35%, respectively.

Interaction of Nitrate and Folate on the Risk of Breast Cancer Among Postmenopausal Women.
Abstract Ingested nitrate can be endogenously reduced to nitrite, which may form N-nitroso compounds, known potent carcinogens. However, some studies have reported no or inverse associations between dietary nitrate intake and cancer risk. These associations may be confounded by a protective effect of folate, which plays a vital role in DNA repair. We evaluated the interaction of dietary and water nitrate intake with total folate intake on breast cancer risk in the Iowa Women's Health Study. Dietary intake was assessed at study baseline. Nitrate intake from public water was assessed using a historical database on Io...

From adaptive behavior to human cognition: a review of Enaction
Critics of the paradigm of enaction have long argued that enactive principles will be unable to account for the traditional domain of orthodox cognitive science, namely "higher-level" cognition and specifically human cognition. Moreover, even many of the paradigm’s "lower-level" insights into embodiment and situatedness appear to be amenable to a functionalist reinterpretation. In this review, I show on the basis of the recently published collection of papers, Enaction, that the paradigm of enaction has (a) a unique foundation in the notion of sense-making that places fundamental limits on the scope of functionalist ...

A Historical Perspective on Sepsis.
Abstract In North America, approximately 700,000 cases of sepsis occur each year, with mortality ranging between 30% and 50%. The American Journal of Pathology has featured numerous articles on the topic, revealing mechanistic insights gleaned from both experimental rodent models and human sepsis. Nonetheless, there remains urgent need to determine the basis for sepsis-related complications and how they can be avoided, as well as how they can be most effectively treated once recognized. This Historical Perspective reviews what we currently understand about the mechanisms of sepsis, as well as the barriers that rema...

What is a nurse? Is there an international consensus?
Conclusion:  Until proper consensus is reached on what a nurse is and does, any skill mix or clinical role developments will only have limited international relevance, especially in OECD countries. If nursing qualifications are to be valid even across the European Union, then recommended standards such as those of the ICN, must be specified in terms of what different categories of nurses actually can do, and their responsibilities and roles within that scope of practice. Standardization of definitions of categories of nurse internationally should reduce confusion and promote better understanding of patterns of nurse staf...

State tobacco revenues compared with tobacco control appropriations--United States, 1998-2010.
Abstract In 1999, CDC published Best Practices for Comprehensive Tobacco Control, which outlined the elements of an evidence-based state tobacco control program and provided a recommended state funding range to substantially reduce tobacco-related disease, disability, and death. Best Practices recommended that states invest a combined $1.6-$4.2 billion annually in such programs and subsequently updated that recommendation to $3.7 billion annually in 2007. To analyze states' historical investments in tobacco control and calculate the amount of funding necessary to achieve Best Practices recommendations, CDC tracked ...

Vaccination therapy for non-small-cell lung cancer: review of agents in phase III development
The historical results of cancer vaccination for non-small-cell lung cancer (NSCLC) were disappointing. In the current decade, however, new insights in the interaction between tumours and the immune system have led to the development of immunotherapy as a fundamentally new concept for the treatment of NSCLC. Modern NSCLC vaccine strategies rely on better identification of antigenic targets, addition of strong immunoadjuvants, and use of more efficient delivery systems. These treatments have convincingly demonstrated to elicit potent immune responses and have shown promising efficacy signals and excellent tolerability in ph...

Appropriate Use Criteria for Echocardiography: New Applications for a New Era of Utilization
Appropriate Use Criteria (AUC) for a variety of cardiac imaging procedures were developed in response to the dramatic increase in the performance of and expenditures on cardiac imaging over the last two decades. The cumulative growth in medical diagnostic imaging of 85% (2000-2009), including a near doubling in the volume of transthoracic echocardiography (TTE), has exceeded the growth of any other physician service. This increase in utilization has raised questions from governmental and private payers about potential misuse and overuse of cardiac imaging procedures and placed cardiac diagnostic imaging in the vortex of th...

Estimation of historical effective population size using linkage disequilibria with marker data
This study considers the impact of several developments on the estimation of Ne using both simulated and equine high‐density single‐nucleotide polymorphism data, when Ne is assumed to be constant a priori and when it is not. In all models, estimates of Ne were highly sensitive to thresholds imposed upon minor allele frequency (MAF) and to a priori assumptions on the expected r2 for adjacent markers. Where constant Ne was assumed a priori, then estimates with the lowest mean square error were obtained with MAF thresholds between 0.05 and 0.10, adjustment of r2 for finite sample size, estimation of a [the limit for r2 as...

Historical influence of predation pressure on escape by Podarcis lizards in the Balearic Islands
Island tameness (reduced escape behaviour on islands where prey have experienced prolonged relaxation of predation pressure) is known in several taxa, although the relationships between recent predation pressure and escape on islands are poorly known. We investigated escape by numerous populations exposed to differing predation pressure of two sister species of Podarcis lizards in the Balearic Islands. Our main findings are that flight initiation distance was greater in Podarcis pityusensis than Podarcis lilfordi and increased as predation pressure increased in P. pityusensis. Island tameness led to extinction of P. li...

We analyse how women were named and positioned, and the attendant constructions of abortion, in South African newspaper articles on abortion from 1978 to 2005, a period during which there were radical political and legislative shifts. The name 'woman' was the most frequently used (70% of articles) followed by 'girl/teenager/child' (25%), 'mother' (25%), 'patient' (11%) and 'minor' (6%). The subject positionings enabled by these names were dynamic and complex and wer...
