The risk of bowel injury during surgery for small bowel obstruction (SBO) has generated interest in conservative treatment modalities. Few data on conservative Gastrografin treatment for SBO in children are available. The authors prospectively included patients with uncomplicated adhesive SBO managed at a pediatric center between March 2009 and September 2010. Patients who were unimproved after 48 hours of conservative treatment received 50 to 100 mL of Gastrografin. Conclusion: Near-complete subspecialization for EA coincided with reduced pediatric intensive care unit stay, successful introduction of cosmetic axillary approaches, and extension of our replacement service to offer all interposition types. It has not reversed the steep decline in trainee experience of EA that has been associated with the greater numbers of trainees that have been employed to reduce working hours.

Conclusions:
Pulmonary nodules are prevalent findings on chest and abdominal CT scans. Although most radiologists recommend follow-up imaging for these findings, recommendations for pulmonary nodules were consistent with the FSG in 34% of radiology reports. Nodule size demonstrated an association with guideline adherence, after adjusting for key variables.

be superfluous. Rather, selective utilization is recommended. Management options vary, depending mainly on patient disposition.

and safety of commonly used oral, topical, and intraarticular drug therapies in this population. A search of several databases

Conclusions While our model favored surgical excision over conservative treatment in the initial management of mesh exposure following
vaginal prolapse repair with synthetic mesh, the difference in QALYs between strategies was less than the MID. Therefore the
strategies are likely similar overall. Individual patient characteristics may ultimately drive clinical decision-making for
this surgical complication.

ConclusionIn this consecutive series of patients with MINCA, CMR imaging may help to differentiate between those with myocarditis, myocardial necrosis and normal myocardium. The incidence of MINCA was higher than previously reported. After excluding cases of myocarditis, MINCA consists of a large group of patients with normal CMR imaging results and a smaller group with myocardial necrosis. Abstract
Trauma is the leading cause of mortality in children over one year of age in industrialized countries. In this retrospective study we reviewed all chest trauma in pediatric patients admitted to Mansoura University Emergency Hospital from January 1997 to January 2007. Our hospital received 472 patients under the age of 18. Male patients were 374 with a mean age of 9.2±4.9 years. Causes were penetrating (2.1%) and blunt trauma (97.9%). Conclusion: Using the excimer laser appears safe in this small feasibility study of subacute and chronic thrombotic occlusions. Angiographically, there was a significant reduction in percent stenosis with the laser alone. The limited IVUS data did not show a reduction in plaque/thrombus volume but the number of interpretable ultrasounds was small. Further data are needed to address the effectiveness of the laser in treating these thrombotic occlusions and its advantages over balloon angioplasty.

Seen from a health insurance provider's perspective, the assessment of the Austrian diabetes DMP shows promising results indicating improved quality of outpatient care as well as overall cost advantages due to the lower hospitalization rates. Due to methodological limitations of the retrospective study and to the restricted data access, further promotion of the DMP must be accompanied by prospective research and preferably controlled trials in order to provide a solid basis for the decision of whether to include diabetes DMP into the insurer's basic benefit package.

CONCLUSIONS:The specialty of the primary decision maker, the number of providers administering RAI, and having access to a tumor board were associated significantly with the use of RAI for stage I thyroid cancer. The findings have implications for addressing nonclinical variation between hospitals, with a marked heterogeneity in decision making suggesting that standardization of care will be challenging. Conclusions
We identified seven risk factors for intraoperative AF in patients receiving lung operations. These findings may eventually help us to improve preoperative and intraoperative management to minimize intraoperative AF.

Conclusions
Extracardiac total cavopulmonary connection in apicocaval juxtaposition can be carried out with favorable midterm outcomes. The route between the inferior vena cava and the contralateral pulmonary artery should be the primary choice when the relevant pulmonary artery is in good shape. Care must be taken in regard to critical conduit oppression by the ventricle in cases with large ventricular volume or poor ventricular function.

The authors’ objective was to determine the impact of a crew resource management program in collaboration with Swiss International Airlines. Participants included operating room personnel: surgeons, anesthesiologists, nurses, and technicians. Conclusions
This first audit of children's diabetes inpatient care organisation demonstrates that some standards can be achieved, but others, such as having children's nurses on every shift in EDs, lack of dietetic advice to ward staff, and liaison with the diabetes team quickly out of hours, are more challenging. Further planned audit outcomes are to produce patient and parent literature for children admitted to hospital and to refine the standards further.
