Applying representational state transfer (REST) architecture to archetype-based electronic health record systems
Conclusions: Using REST addressed many architectural concerns in a successful way, but an additional messaging component was needed to address some architectural aspects. Many of our approaches are likely of value to other archetype-based EHR implementations and may contribute to associated service model specifications. Author's response
We would like to thank Drs Campbell-Taylor1 and Iqbal et al2 for their letters of interest regarding our paper.3 Nonetheless, we are unclear as to the nature of Dr Campbell-Taylor's ‘quibble’ with our methodology. In contrast to the assertion that we did not take confounders into account, we actually adjusted for comorbidities, previous pneumonia, smoking status and alcohol intake as well as age and social deprivation. The concern over whether pneumonitis and pneumonia can be differentiated from our data is more valid and we agree that there may be some errors in diagnosis and/or coding. However, these errors a...

Standardized Clinical Assessment And Management Plans (SCAMPs) Provide A Better Alternative To Clinical Practice Guidelines [Payment & Practice]
Variability in medical practice in the United States leads to higher costs without achieving better patient outcomes. Clinical practice guidelines, which are intended to reduce variation and improve care, have several drawbacks that limit the extent of buy-in by clinicians. In contrast, standardized clinical assessment and management plans (SCAMPs) offer a clinician-designed approach to promoting care standardization that accommodates patients’ individual differences, respects providers’ clinical acumen, and keeps pace with the rapid growth of medical knowledge. Since early 2009 more than 12,000 patients have b...

Mental Health Act 1983: use of urgent treatment in clinical practice [Original papers]
Aims and method To understand circumstances in which urgent treatment provisions are used in clinical practice, by means of a retrospective study. A list of patients to whom Sections 62, 64B and 64G of the Mental Health Act 1983 were applied during the 1-year study period was obtained from the information technology department. Case notes were traced for detailed information on the circumstances of use of these provisions. Results The most common reason for urgent treatment was to continue the established treatment plan rather than to start a new treatment (45% Section 62, 84% Section 64). The urgent treatment provisions w...

Psychiatrists' access to electronic laboratory results [Original papers]
Aims and method A completed audit cycle, consisting of email surveys of all psychiatrists and a review of acute trust information technology registers was undertaken in Derbyshire Healthcare NHS Foundation Trust, to assess whether psychiatrists could access electronic laboratory results from the neighbouring acute trust laboratory. This was replicated in Nottinghamshire Healthcare NHS Trust, with an initial audit; plans were then implemented to increase access. Results In Derbyshire, 14% of psychiatrists could access electronic results initially, with limited ward access. By re-audit, all wards had access, and 28% used tho...

Change in Adoption of Electronic Health Records by US Children's Hospitals
Electronic health record (EHR) uptake by US hospitals has been slow, including among children’s hospitals. The Health Information Technology for Economic and Clinical Health program, which began in 2011, offers incentives for adoption and meaningful use of EHRs. Using an annual survey, we evaluated how children’s hospitals have progressed in EHR adoption from 2008 through the start of the Health Information Technology for Economic and Clinical Health program and assessed their ability to meaningfully use EHRs. Perceived barriers of heart failure nurses and cardiologists in using clinical decision support systems in the treatment of heart failure patients
Background: Clinical Decision Support Systems (CDSSs) can support guideline adherence in heart failure (HF) patients. However, the use of CDSSs is limited and barriers in working with CDSSs have been described as a major obstacle. It is unknown if barriers to CDSSs are present and differ between HF nurses and cardiologists. Therefore the aims of this study are; 1. Explore the type and number of perceived barriers of HF nurses and cardiologists to use a CDSS in the treatment of HF patients. 2. Explore possible differences in perceived barriers between two groups. 3. Assess the relevance and influence of knowledge management...

Protecting the Privacy Rights of Adolescents
Privacy and security of health information are now considered to be basic rights of patients. Unfortunately, the Health Insurance Portability and Accountability Act of 1996, and Health Information Technology for Economic and Clinical Health Act of 2009 appear to offer little to address the unique privacy needs of minors and young adults, who may routinely seek health care under their parents' supervision and as part of their parents' insurance coverage. The Secure Medical Research Workspace: An IT Infrastructure to Enable Secure Research on Clinical Data
Abstract Clinical data have tremendous value for translational research, but only if security and privacy concerns can be addressed satisfactorily. A collaboration of clinical and informatics teams, including RENCI, NC TraCS, UNC's School of Information and Library Science, Information Technology Service's Research Computing and other partners at the University of North Carolina at Chapel Hill have developed a system called the Secure Medical Research Workspace (SMRW) that enables researchers to use clinical data securely for research. SMRW significantly minimizes the risk presented when using identified clinical data, the...

Computing Health Quality Measures Using Informatics for Integrating Biology and the Bedside
Conclusions: HQMF is an expressive language for defining quality measures, and it will be important to understand and implement for CQM computation, in both meaningful use and population health. However, its current form might allow complexity that is intractable for current database systems (both in terms of implementation and computation). Our translator, which supports the subset of HQMF currently expressible in i2b2-XML, may represent the beginnings of a practical compromise. It is being pilot-tested in two Query Health demonstration projects, and it can be further expanded to balance computational tractability with th...

Medicine an evolving profession
The number of medical practitioners in the developed world has increased but in relative terms their incomes have decreased. Published comments suggest that some doctors are dissatisfied with what they earn. However doctors are still perceived as having a high status in society. Publicly available data suggests that doctors chose to live and work in affluent suburbs where arguably the need for their skills is less than that in neighbouring deprived areas. The gender balance in medicine is also changing with more women entering the workforce and a greater acceptance of part-time working arrangements. In some countries docto...

Reducing unnecessary lab testing in the ICU with artificial intelligence
Conclusions: Reducing frequent lab testing and the potential clinical and financial implications are an important issue in intensive care. In this work we present an artificial intelligence method to predict the benefit of proposed future laboratory tests. Using ICU data from 746 patients with gastrointestinal bleeding, and eleven measurements, we demonstrate high accuracy in predicting the likely information to be gained from proposed future lab testing for eight common GI related lab tests. Future work will explore applications of this approach to a range of underlying medical conditions and laboratory tests. (Source: In...

The impact of EHR and HIE on reducing avoidable admissions: controlling main differential diagnoses
Conclusions: Viewing medical history via an EHR IS and using HIE network led to a reduction in the number of seven day readmissions and single-day admissions for all patients. Using external medical history may imply a more thorough patient examination that can help eliminate unnecessary admissions. Nevertheless, in most instances physicians did not view medical history at all, probably due to the limited resources available, combined with the stress of rapid turnover in ED units. Issues and Models of Renal Supportive Care in Rural Areas
Patients in rural areas are both economically and medically disadvantaged Access to specialist services in rural areas is limited. More care is likely to be out‐sourced to local physicians, GPs and palliative care nurses who will need ‘on the ground’ outreach support from renal/palliative care services Referral to these services may low due to knowledge of availability and previous exposure of the referring physician to the use of these services. Developments in Information Technology are likely to play a significant role in management (telemedicine), education and advice in these specialist areas. (Source: Nephrology)
