Simulating an emergency department: the importance of modeling the interactions between physicians and delegates in a discrete event simulation
Conclusion:
This example shows the importance of accurately modeling physician relationships and the roles in which they treat patients. Neglecting these relationships could lead to inefficient resource allocation due to inaccurate estimates of physician and delegate time spent on patient related activities and length of stay. A study of general practitioners' perspectives on
electronic medical records systems in NHSScotland
Background:
Primary care doctors in NHSScotland have been using electronic medical records within their practices routinely for many years. The Scottish Health Executive eHealth strategy (2008-2011) has recentlybrought radical changes to the primary care computing landscape in Scotland: an informationsystem (GPASS) which was provided free-of-charge by NHSScotland to a majority of GP practiceshas now been replaced by systems provided by two approved commercial providers. The transitionto new electronic medical records had to be completed nationally across all health-boards by March2012.
Methods:
We carried out 25 in-depth s...

EEGgui: a program used to detect electroencephalogram anomalies after traumatic brain injury
Conclusion:
The software presented here was a successful modification of EEGLAB in the Matlab environment that allows detection of epileptiform ECoG signals in animals after TBI. The code allows import of large EEG or ECoG data records as standard text files and uses fast fourier transform as a basis for detection of abnormal events. The software can also be used to monitor injury-induced changes in spectral entropy if required. We hope that the software will be useful for other investigators in the field of traumatic brain injury and will stimulate future advances of quantitative analysis of brain electrical activity afte...

Impact of behaviour and lifestyle on bladder health
Bladder conditions, including UTI, UI, and bladder cancer, are highly prevalent and affect a wide range of populations. There are a variety of modifiable behavioral and lifestyle factors that influence bladder health. Some factors, such as smoking and obesity, increase the risk or severity of bladder conditions, whereas other factors, such as pelvic floor muscle exercise, are protective. Although clinical practice may be assumed to be the most appropriate ground for education on behavioral and lifestyle factors that influence bladder health, it is also crucial to extend these messages into the general population through pu...

Abstract 28: Implementation of a Nationwide Health IT System for Cardiac Cath Labs in the VA Hospital System: the CART Program [Concurrent III Session A: Oral Abstract Presentations on EHS & Health IT]
Conclusions. The CART system was successfully implemented nationwide, achieving full use in the 75 VA cardiac cath labs. The final stage, full Clinical Use, took the longest and was most variable by site. These results reinforce that successful HIT implementation does not end with technical installation and training, and must support clinical use as part of routine care delivery. Our results also reinforce the importance of senior leadership support for HIT implementation to support clinical care. Abstract 263: Predictors of Physical Activity Increase among People Enrolled in a Worksite Wellness Center [Poster Session III]
Conclusion: A significant proportion of people, over one-third, enrolled in a wellness center increase their PA level or maintain high levels of PA at 12 months after enrollment. Increasing age and BMI, and a measure of social support; interaction with close friends, and perceived muscular strength and flexibility, predict increase in PA. Presence of CV risk factors do not predict increase in PA. Tailoring programs to address these domains may help younger, less fit, and those with less social support improve their PA level. Making Sense of Mobile- and Web-Based Wellness Information Technology: Cross-Generational Study
Conclusions: Our findings inform our suggestions for improving the design of future collaborative health and wellness applications that target specific age groups. We recommend that collaborative ICT health applications targeting young adults should integrate with existing social networking sites, whereas those targeting middle-aged and older adults should support small social networks that rely on intimate personal relationships. Systems that target middle-aged adults should support episodic needs, such as time-sensitive, perhaps intermittent, goal setting. They should also have a low barrier to entry, allowing individual...

Author's response
We would like to thank Drs Campbell-Taylor1 and Iqbal et al2 for their letters of interest regarding our paper.3 Nonetheless, we are unclear as to the nature of Dr Campbell-Taylor's ‘quibble’ with our methodology. In contrast to the assertion that we did not take confounders into account, we actually adjusted for comorbidities, previous pneumonia, smoking status and alcohol intake as well as age and social deprivation. The concern over whether pneumonitis and pneumonia can be differentiated from our data is more valid and we agree that there may be some errors in diagnosis and/or coding. However, these errors a...

Standardized Clinical Assessment And Management Plans (SCAMPs) Provide A Better Alternative To Clinical Practice Guidelines [Payment & Practice]
Variability in medical practice in the United States leads to higher costs without achieving better patient outcomes. Clinical practice guidelines, which are intended to reduce variation and improve care, have several drawbacks that limit the extent of buy-in by clinicians. In contrast, standardized clinical assessment and management plans (SCAMPs) offer a clinician-designed approach to promoting care standardization that accommodates patients’ individual differences, respects providers’ clinical acumen, and keeps pace with the rapid growth of medical knowledge. Since early 2009 more than 12,000 patients have b...

Differences between endogenous and exogenous emotion inhibition in the human brain.
Abstract
The regulation of emotions is an integral part of our mental health. It has only recently been investigated using brain imaging techniques. In most studies, participants are instructed by a cue to inhibit a specific emotional reaction. The aim of the present study was to investigate the alternative situation where a person decides to inhibit an emotion as an act of endogenous self-control. Healthy participants viewed highly arousing pictures with negative valence. In the endogenous condition, participants could freely choose on each trial to inhibit or feel the emotions elicited by the picture. In an exoge...

Mental Health Act 1983: use of urgent treatment in clinical practice [Original papers]
Aims and method To understand circumstances in which urgent treatment provisions are used in clinical practice, by means of a retrospective study. A list of patients to whom Sections 62, 64B and 64G of the Mental Health Act 1983 were applied during the 1-year study period was obtained from the information technology department. Case notes were traced for detailed information on the circumstances of use of these provisions.
Results The most common reason for urgent treatment was to continue the established treatment plan rather than to start a new treatment (45% Section 62, 84% Section 64). The urgent treatment provisions w...

Psychiatrists' access to electronic laboratory results [Original papers]
Aims and method A completed audit cycle, consisting of email surveys of all psychiatrists and a review of acute trust information technology registers was undertaken in Derbyshire Healthcare NHS Foundation Trust, to assess whether psychiatrists could access electronic laboratory results from the neighbouring acute trust laboratory. This was replicated in Nottinghamshire Healthcare NHS Trust, with an initial audit; plans were then implemented to increase access.
Results In Derbyshire, 14% of psychiatrists could access electronic results initially, with limited ward access. By re-audit, all wards had access, and 28% used tho...

Change in Adoption of Electronic Health Records by US Children's Hospitals
Electronic health record (EHR) uptake by US hospitals has been slow, including among children’s hospitals. The Health Information Technology for Economic and Clinical Health program, which began in 2011, offers incentives for adoption and meaningful use of EHRs.
Using an annual survey, we evaluated how children’s hospitals have progressed in EHR adoption from 2008 through the start of the Health Information Technology for Economic and Clinical Health program and assessed their ability to meaningfully use EHRs. (Read the full article) (Source: PEDIATRICS)

Can Health IT Tools Enable Improved Documentation of Quality, Safety Measures, and Regulatory Requirements in Radiology Reports?
Radiology reports are the product of our work as diagnostic radiologists. These reports serve many functions, including clinical, legal, billing, compliance, and documentation functions, to name just a few. There are many attributes of a high-quality radiology report []. Radiology reports also serve as the primary mechanism for the documentation of radiologists' cognitive input to the electronic health record to help inform and guide a patient's care plan. Perceived barriers of heart failure nurses and cardiologists in using clinical decision support systems in the treatment of heart failure patients
Background:
Clinical Decision Support Systems (CDSSs) can support guideline adherence in heart failure (HF) patients. However, the use of CDSSs is limited and barriers in working with CDSSs have been described as a major obstacle. It is unknown if barriers to CDSSs are present and differ between HF nurses and cardiologists. Therefore the aims of this study are; 1. Explore the type and number of perceived barriers of HF nurses and cardiologists to use a CDSS in the treatment of HF patients. 2. Explore possible differences in perceived barriers between two groups. 3. Assess the relevance and influence of knowledge management...
