To be involved or not to be involved: A survey of public preferences for self-involvement in decision-making involving mental capacity (competency) within Europe.
Conclusions:Self-involvement in decision-making is important to the European public. However, a large proportion of the public prefer to not make decisions about their care in advance of incapacity. Financial hardship, educational attainment, age, and preferences regarding quality and quantity of life require further examination; these factors should be considered in relation to policy. Blood and body fluid exposures: consent for source patient testing
Conclusions This study found that despite guidance, routine source testing is not universal. Incapacity to consent is a contributory factor for some source serology not being tested and clarification of the ethical and legal position would be helpful. Larger studies should explore other reasons why identified source patients are not tested in practice and explore the policy implications of those findings.

Readmission Rates in the State of Florida: A Reflection of Quality?
CONCLUSIONS: Patients were more frequently readmitted if their payer was the government, they were discharged to a SNF, or they had a mental health disorder. Infections were the most common reasons for readmission. Our data suggest readmission rates alone do not necessarily reflect quality of care. PMID: 23423619 [PubMed - as supplied by publisher]

Simple measures to promote sleep can reduce delirium in intensive care patients
(Johns Hopkins Medicine) A lack of sleep among patients in an ICU can cause delirium -- an altered mental state that may delay their recovery and lead to short and long-term confusion and memory problems. A team of doctors, nurses, and others in the medical intensive care unit at Johns Hopkins implemented a project to reduce nighttime noise, light, and staff interruptions, and switching certain medications. They were able to reduce delirium and improve patient perceptions about the quality of their sleep.

Translating and testing the Alberta context tool for use among nurses in Swedish elder care
Conclusion: Modifying an instrument is a detailed process, requiring time and consideration of the linguistic and semantic aspects of the instrument, and understanding of the context where the instrument was developed and where it is to be applied. A team, including the instrument's developers, translators, and researchers is necessary to ensure a valid translation. This study suggests preliminary validity, reliability and acceptability evidence for the ACT when used with nurses in Swedish elder care.

Evaluation of Iodide Deficiency in the Lactating Rat and Pup Using a Biologically Based Dose-Response Model
A biologically based dose-response (BBDR) model for the hypothalamic-pituitary thyroid (HPT) axis in the lactating rat and nursing pup was developed to describe the perturbations caused by iodide deficiency on the HPT axis. Model calibrations, carried out by adjusting key model parameters, were used as a technique to evaluate HPT axis adaptations to dietary iodide intake in euthyroid (4.1–39 µg iodide/day) and iodide-deficient (0.31 and 1.2 µg iodide/day) conditions. Iodide-deficient conditions in both the dam and the pup were described with increased blood flow to the thyroid gland, TSH-mediated increase...

The invisible and visible stigmatization of Parkinson's disease
ConclusionsThe invisible stigma was that of a changing self, a self with PD, a form of disability, which attested to the stigma and mounting isolation. Feelings of isolation were exacerbated by both the stigmatization associated with being different, coupled with the realization that life would never be the same. Facial masking was identified as one of the visible features that were commonly reported by all participants. Implications for practiceHealthcare professionals can be more attuned to the visible and invisible manifestations of PD that are associated with stigma through the incorporation of a holistic assessment to...

Stigma perspective of siblings of children with a major childhood burn injury
ConclusionsStigma experienced by siblings was first described by parents or noninjured siblings; they described how the sibling with changed appearance was stared at, ridiculed, or teased when they entered a new social situation. Only when specifically asked did the children with burn injury talk about their problems, saying, “This always happens when I go somewhere new.” Implications for practiceChildren with changed appearance focused on normalizing their lives in a positive way. Oftentimes, it was a parent or noninjured sibling who would describe manifestations of stigma and ways they tried to protect the child with burn injury.

An overview of the effectiveness of psychological therapy for depression and stepped care service delivery models
Depression is one of the most prevalent health disorders globally and causes significant distress and cost to the sufferer and society. Psychological therapy for depression has been recommended over a number of decades; however, access to these treatments remains limited. Cognitive behaviour therapy, behavioural activation, self-help approaches, interpersonal therapy and non-directive therapy all demonstrate moderate to large effect sizes when compared to treatment as usual. Differences between psychological interventions however are small and unstable when reviewed in meta-analyses suggesting that for most people adding c...

Caring for prisoners: towards mindful practice
Those caring for prisoners are subject to the influences of the dominant custodial discourses of security and risk management that prevail in a prison environment. The conflicting caring and custodial roles adopted by nurses working in prisons and by prison officers constitute the foundation for further exploration of prisoner care in the wider healthcare literature. In this discussion paper, we argue that the identities and practices of those working in prison care are significantly shaped by organisational and societal discourses and that these generate rigidities and defensive mechanisms that limit the development of pr...
