Crown‐Rump Length Discordance And Adverse Perinatal Outcome In Twins: The Stork Multiple Pregnancy Cohort
ConclusionIn the absence of aneuploidy or structural fetal abnormality, CRL discordance is of poor predictive value for adverse perinatal outcome in either MC or DC twin pregnancies. CRL discordance should not routinely be used to identify twin pregnancies at high risk of adverse perinatal outcome.

Arizona twin project: a focus on early resilience.
Abstract The Arizona Twin Project is an ongoing longitudinal study designed to elucidate the genetic and environmental influences underlying the development of early competence and resilience to common mental and physical health problems during infancy and childhood. Participants are a sample of 600 twins (25% Hispanic) recruited from birth records in the state of Arizona, United States. Primary caregivers were interviewed on twins' development and early social environments when twins were 12 and 30 months of age. Measures include indices of prenatal and obstetrical risk coded from hospital medical records, as well...

Uterine arteriovenous malformation involving the whole myometrium
Uterine arterio‐venous malformations (AVM) are uncommon, yet potentially life‐threatening. Here is a rare case of extensive arteriovenous malformation involving the whole myometrium in a 32 years old woman. US‐3D reconstruction and Doppler of the uterus reveals a heavy vascularisation into myometrium and the presence of a small cavity without any vessel. Due to failure of two previous conservative management of AVM, a vaginal hysterectomy was performed. Pathologic findings conclude to the presence of numerous fibrous nodular vascular formations involving the whole uterine wall from serosa to endometrium in relation t...

Screening for depression and anxiety in women with breast and gynaecologic cancer: course and prevalence of morbidity over 12 months
ConclusionsWomen are most vulnerable to psychological morbidity at diagnosis. Symptoms improve significantly over time. Reported rates are lower than those in the literature. Regular screening by self‐report is acceptable to patients but may not be the most efficient method of improving patient outcomes. Copyright © 2013 John Wiley & Sons, Ltd.

Practice Variation, Bias, and Experiential Learning in Cesarean Delivery: A Data‐Based System Dynamics Approach
ConclusionsThe repetitive nature of medical decision making, learning from past practice, and accumulating experience can account for increases in CD decisions and practice variation across physicians. Policies aimed at improving medical decision making should account for providers' feedback‐based learning mechanisms.

Anesthesia for fetal surgery
Abstract: Fetal surgery pushes the limits of knowledge and therapy beyond conventional paradigms by treating the developing fetus as a patient. Providing anesthesia for fetal surgery is challenging for many reasons. It requires integration of both obstetric and pediatric anesthesia practice. Two patients must be anesthetized for the benefit of one, and there is little margin for error. Many disciplines are involved, and communication must be effective among all of them. Conducting anesthetic research with vulnerable populations, such as the pregnant woman carrying a fetus with a birth defect is difficult, and many question...

Novel biomarkers for predicting intrauterine growth restriction: a systematic review and meta‐analysis
ConclusionsNone of the novel biomarkers evaluated in this review are sufficiently accurate to recommend their use as predictors of IUGR in routine clinical practice. However, the use of biomarkers in combination with biophysical parameters and maternal characteristics could be more useful and merits further research.

Ovarian metastasis from adenocarcinoma of the lung
A case of a young woman, affected by an unresectable pulmonary adenocarcinoma, diagnosed by broncoscopy with biopsies and treated with platinum‐pemetrexed based chemotherapy, with an incidental finding of a left ovarian mass. At ultrasound examination, a solid lesion was detected in the left ovary. Final pathology revealed an ovarian metastasis from low differentiate adenocarcinoma of the lung. This experience gives us an example of an ovarian metastasis from lung cancer, which at ultrasound examination appears as solid ovarian mass, with lobulated margins, moderate vascularisation, adjacent to normal ovarian parenchyma....

Trends in the prevalence, risk and pregnancy outcome of multiple births with congenital anomaly: a registry‐based study in 14 European countries 1984–2007
ConclusionsThe increase in babies who are both from a multiple pregnancy and affected by a congenital anomaly has implications for prenatal and postnatal service provision. The contribution of assisted reproductive technologies to the increase in risk needs further research. The deficit of chromosomal anomalies among multiple births has relevance for prenatal risk counselling.

Symptomatic deep venous thrombosis of the lower extremity in a pregnant woman successfully treated with endovascular procedures
We report a pregnant woman with symptomatic deep venous thrombosis of the lower extremity distributing in the left iliac, femoral, popliteal and calf veins. Systemic anticoagulation therapy failed; however, treatment was successful through a combination of various endovascular procedures, including catheter‐directed thrombolysis. During the therapies, an optional inferior vena cava filter was implanted to protect against the development of pulmonary embolism.

Inter-rater reliability of assessment of levator ani muscle strength and attachement to the pubic bone in nulliparous women.
CONCLUSION: MOS revealed substantial agreement between the two investigators. In this group of nulliparous patients, there was good agreement between grades of LAM attachment, which correlated to TPUS. It therefore appears that these assessment tools are reproducible in nullipara, can be readily learned and reliably incorporated into clinical practice and research after appropriate training. Rate of change in cervical length in women with vaginal bleeding during pregnancy.
CONCLUSIONS: : Pregnancies without vaginal bleeding have a 20% increase in the risk of preterm birth for each additional millimeter per week increase in cervical slope. Pregnancies with vaginal bleeding are at risk for preterm birth but do not appear to undergo progressive cervical shortening. This suggests that women with vaginal bleeding undergo a different mechanism leading to preterm birth. LEVEL OF EVIDENCE: : II. PMID: 23344274 [PubMed - in process]

Preventing L5-s1 discitis associated with sacrocolpopexy.
CONCLUSION: : In the supine position, the most prominent structure in the presacral space is the L5-S1 disc, which extends approximately 1.5 cm cephalad to the "true" sacral promontory. During sacrocolpopexy, awareness of a 60-degree average drop between the anterior surfaces of L5 and S1 vertebra should assist with intraoperative localization of the sacral promontory and avoidance of the L5-S1 disc. The first sacral nerve can be expected approximately 3 cm from the upper surface of the sacrum and 1.5 cm from the midline. LEVEL OF EVIDENCE: : II. PMID: 23344278 [PubMed - in process]

Contamination rates of three urine-sampling methods to assess bacteriuria in pregnant women.
CONCLUSION: : In pregnant women, the contamination rate of midstream samples is comparable with the contamination rates of morning and clean-catch samples. The quantity of contaminants varied among the three samples collected by one woman. These results show that more complex, unpractical, and time-consuming morning and clean-catch samples are not superior. Therefore, we recommend a midstream sample to assess bacteriuria in pregnant women. LEVEL OF EVIDENCE: : II. PMID: 23344279 [PubMed - in process]

Classification of diabetes in pregnancy: time to reassess the alphabet.
Abstract The White classification is a system of alphabetically designated categories of diabetes in pregnancy based on age at onset, duration of disease, and the presence or absence of vascular complications. The original classification system underwent several revisions, each progressively increasing in detail and complexity. Individual authors and institutions have modified the classes, resulting in identically lettered classes having different definitions. Some publications make reference to the class of diabetes by letter without providing a reference defining that class. Despite a 1994 American College of Obs...

Pravastatin for the Prevention of Preeclampsia in High-Risk Pregnant Women.
Abstract Preeclampsia complicates approximately 3-5% of pregnancies and remains one of the major causes of maternal and neonatal morbidity. It shares pathogenic similarities with adult cardiovascular disease as well as many risk factors. Attempts at prevention of preeclampsia using various supplements and classes of medications have failed or had limited success, and they were not convincing enough to lead to widespread adoption of any particular strategy. Contrary to the experience with preeclampsia, prevention of cardiovascular mortality and other cardiovascular events in nonpregnant patients using 3-hydroxy-3-me...

Surgical treatment of vaginal apex prolapse.
Abstract Pelvic organ prolapse is a common problem in women that increases with age and adversely affects quality of life and sexual function. If conservative treatments fail, surgery becomes the main option for symptom abatement. For uterovaginal prolapse, treatment with or without hysterectomy can be offered, and operations must include a specific apical support procedure to be effective. Operations for apical prolapse include transvaginal, open, and laparoscopic or robotic options; few clinical trials have compared the effectiveness and risk of these various surgeries. Grafts can be used selectively for apical s...
