Inter‐rater reliability of assessment of levator ani muscle strength and attachement to the pubic bone in nulliparous women
ConclusionMOS revealed substantial agreement between the two investigators. In this group of nulliparous patients, there was good agreement between grades of LAM attachment, which correlated to TPUS. It therefore appears that these assessment tools are reproducible in nullipara, can be readily learned and reliably incorporated into clinical practice and research after appropriate training. Further evaluation to establish its clinical utility in postpartum women and women with (symptomatic) pelvic organ prolapse is awaited.

Predicting successful vaginal birth after cesarean section using a model based on cesarean scar features examined using transvaginal sonography

Severe second trimester ventriculomegaly revealing obstruction related to pathologies of diencephalic, mesencephalic and rhombencephalic differentiation
ConclusionWhen facing severe isolated ventriculomegaly, of which sonographic features are suggestive of an obstructive mechanism, close examination of the third ventricle, thalami, Sylvius aqueduct, and cerebellum may reveal pathological differentiation of the diencephalon, mesencephalon or rhombencephalon, often in combination.

Unplanned Extubation in the NICU
ABSTRACT
Unplanned extubation (UE) in the neonatal intensive care unit (NICU) is a significant patient safety and quality control issue. I describe the implementation of a quality improvement program using multifactorial prevention strategies, including staff education, identification of neonates at risk for UE, extubation and weaning, standardization of procedures, and comprehensive documentation. Additional research on quality improvement with strategies for neonates may prove beneficial in reducing UE rates in neonates.

Clinical Determinants of Parents’ Emotional Reactions to the Disclosure of a Diagnosis of Congenital Anomaly
ConclusionBoth parents’ emotional experiences should be acknowledged at the disclosure. Clinical variables were found to define the stressful situation (the diagnosis). When the diagnosis was perceived as more threatening (i.e., more unexpected, less controllable, and predictable), parents presented a pattern of high‐intensity emotional reactions.

A Rounding System to Enhance Patient, Parent, and Neonatal Nurse Interactions and Promote Patient Safety
ABSTRACT
Comfort rounds, an approach to assessing and meeting the needs of patients and families in a neonatal intensive care unit (NICU), was created based on nursing rounds. The purpose of comfort rounds is to provide information to parents and assess and enhance the neonate's comfort and safety. I describe a comfort rounds process that was implemented through presentations, huddles, and discussions and was validated through parent comments, employee documentation, and an increase in patient satisfaction scores.

Room Air Resuscitation and Targeted Oxygenation for Infants at Birth in the Delivery Room
ABSTRACT
The results of several clinical trials suggest that infants born depressed can be successfully resuscitated with room air. In 2010, the American Heart Association, American Academy of Pediatrics, Neonatal Resuscitation Program, and the International Liaison Committee published new guidelines to initiate the resuscitation of the term neonate with 21% oxygen. Although this recommendation cannot be extrapolated to the preterm neonate, the use of oxygen for resuscitation in this population can be used cautiously.

Treatment‐Seeking Behaviors and Attitudes Survey among Women at Risk for Perinatal Depression or Anxiety
ConclusionFindings suggest screening and referral can be enhanced through expanding perinatal mental health education to target social support persons as they are often consulted by women when determining whether depression exists or should be treated. Further, a more active, personalized referral processes should be used routinely in addition to screening and education to facilitate appropriate treatment.

Understanding the Moral Nature of Intrapartum Nursing
ConclusionThese findings suggest a need to challenge assumptions related to the provision of choice and family centered care to create environments that can support and sustain understanding and trust between nurses and women giving birth. In addition, given the lack of shared understandings of what constitutes best care, there is a need to develop collaborative models of care that include the voices of women as a central component.

Helping Babies Breathe Around the World
ABSTRACT
Helping Babies Breathe (HBB) is an educational curriculum using the train‐the‐trainer model to teach neonatal resuscitation in resource limited areas. The purpose of this evidence‐based program is to reduce global neonatal mortality by educating birth attendants to provide basic neonatal resuscitation. This program directly addresses the Millennium Developmental Goal #4 of reducing child mortality.

Certain Markers For Down's Syndrome More Significant
Certain second trimester markers for Down's syndrome that are identified in an ultrasound are more significant than others. This study compares the efficacy of a lympho-hemostatic combipatch TachoSil(®) versus conventional implementation of a suction drain on the incidence and severity of lymphoceles. This is a case (group TachoSil(®)n=20) control (drainage group) n=40 study. In our study, we demonstrated an increased rate of postoperative complications (<3weeks) in the group TachoSil(®) (P=0.0265) explained by a lymphocele rate significantly higher (P=0.0194). However, no significant difference was demonstrated on the severity of these lymphoceles. Beyond 3weeks postoperatively, the two groups were comparable in terms of postoperative co...

Misdiagnosed ectopic pregnancy mimicking adnexal malignancy: a report of two cases
We present two cases of nulliparous women with no history of amenorrhea who were referred to our unit following finding of a hypervascularized solid adnexal mass. In both cases a malignant tumor of tubal origin was suspected at transvaginal sonography. However, following laparoscopy, histological examination revealed that the masses were the result of an undetected tubal ectopic pregnancy that had failed spontaneously, with massive vasodilatation of pelvic blood vessels surrounding the trophoblastic tissue. We recommend consideration of the potential diagnosis of a previously undetected, failed tubal ectopic pregnancy in f...

Three‐dimensional ultrasound of pelvic floor: is there a correlation with delivery mode and persisting pelvic floor disorders 18–24 months after first delivery?
Conclusions3D transperineal ultrasound, which is easily accessible, can provide useful information on morphological changes of the female pelvic floor. Women with a spontaneous or operative vaginal first delivery had a significantly larger hiatal area and axial distension than did women whose first delivery was by Cesarean section, even 2 years after delivery. Performing 3D ultrasound after the first delivery may help to identify women at high risk for persisting pelvic floor disorders.

Reliability of fetal thymus measurement in prediction of 22q11.2 deletion: a retrospective study using four‐dimensional spatiotemporal image correlation volumes
ConclusionThe TT‐ratio method is a feasible and potentially useful tool during detailed fetal heart assessment. Fetal karyotyping should be recommended in cases with conotruncal heart abnormalities, irrespective of the TT‐ratio.

Review article: extubation of the difficult airway and extubation failure.
Abstract
Respiratory complications after tracheal extubation are associated with significant morbidity and mortality, suggesting that process improvements in this clinical area are needed. The decreased rate of respiratory adverse events occurring during tracheal intubation since the implementation of guidelines for difficult airway management supports the value of education and guidelines in advancing clinical practice. Accurate use of terms in defining concepts and describing distinct clinical conditions is paramount to facilitating understanding and fostering education in the treatment of tracheal extubation-rel...
