Are prenatal, obstetric, and infant complications associated with postpartum psychosis among women with pre‐conception psychiatric hospitalisations?
ConclusionsWe found no prenatal indicators of postpartum risk for psychiatric hospitalisation among high‐risk women, but they had higher odds of postpartum pregnancy‐related medical problems and, rarely, offspring death.

We thank Dr Kreitzer for his continuing communications on shoulder dystocia and comments on our work. We agree that subjective definitions and imperfect record keeping are limitations with most studies on shoulder dystocia; however, we would like to point out an incorrect assumption in the letter. In our study, incidences of shoulder dystocia were not determined by retrospective chart review, which would potentially add recorder bias issues. The diagnosis was recorded prospectively in the delivery record by the delivering clinicians at the time of the birth.

Shoulder dystocia outcomes associated with structured prenatal counseling
I commend the efforts of my New Jersey colleagues undertaking quantitative risk assessment for shoulder dystocia, and applying it prospectively. It is likely, however, that the commonly used but somewhat restrictive definition of shoulder dystocia chosen by the authors (use of maneuvers following a failure of downward traction) is, to a great extent, responsible for the decline in shoulder dystocia incidence each year, and when compared to the group without enhanced assessment and counseling. Pregnancy Outcomes of Women With Physical Disabilities: A Matched Cohort Study
Conclusion: Our study describes a cohort of women who had moderate-to-severe physical disabilities who historically have been discouraged from pregnancy. Women with physical disabilities experienced higher rates of preterm deliveries, low birthweight infants, and pregnancy complications. The pregnancy complications in most cases were not severe and were readily managed. Although it was common to experience functional changes during pregnancy, these changes had largely resolved by 6 weeks postpartum. Maternal psychosocial stress during pregnancy is associated with newborn leukocyte telomere length
Conclusion: Our finding provides the first preliminary evidence in human beings that maternal psychological stress during pregnancy may exert a “programming” effect on the developing telomere biology system that is already apparent at birth, as reflected by the setting of newborn LTL.

Managing persistent ovarian follicles in premenopausal women fitted with the levonorgestrel-releasing intrauterine system

A population-based survey on the influence of the menstrual cycle and the use of hormonal contraceptives on sleep patterns in São Paulo, Brazil
Conclusion: The use of hormonal contraceptives was associated with a lower apnea–hypopnea index and a trend toward improved sleep efficiency. The current findings suggest that the use of hormonal contraceptives has a stronger association with sleep duration compared with menstrual cycle phase.

Non‐obstetric vulval trauma
ConclusionsNon‐obstetric vulval injuries are uncommon (incidence 3.7%). All cases require assessment for vaginal, urethral, anal and bony pelvis injuries. This might require examination under anaesthesia. Conservative management of haematomas in the absence of acute haematoma expansion is favoured. The need to screen for sexually transmissible infections and pregnancy is important. Social worker and psychological support is important to reduce the incidence of long‐term psychological problems.

Primary Leydig cell tumour of epididymis: a rare case report with review of literature
Summary Leydig cell tumour (LCT) is an uncommon tumour that typically occurs in the testis. Primary epididymal LCT is extremely rare. ConclusionsWhilst previous work has shown that gain correlates well with placental tissue VOCAL indices, the correlation between gain level and VOCAL indices in an area of 100% vascularity at the cord insertion is poor. Regions of 100% vascularity appear to be artificially assigned a value approaching 100% for all VOCAL indices irrespective of gain level. This precludes using the technique of VOCAL indices from large vessels to standardize power Doppler measurements and the FMBV index is therefore not applicable to image analysis using VOCAL.

Abstract This review highlights the progress made in food allergy (FA) and anaphylaxis research in pediatrics published in the journal Pediatric Allergy and Immunology since 2010. Putative risk factors for FA are as follows: a family history of allergic disease, particularly in the mother, low birth order, season of birth, and severe atopic eczema. Primary anti-phospholipid syndrome: any role for serum complement levels in predicting pregnancy complications?
Conclusion. No association was found between hypocomplementaemia and obstetric complications in PAPS. However, both cases of pre-eclampsia were characterized by low C3 throughout pregnancy. There is evidence that the complement system is a contributor to the mechanisms of aPL-mediated damage, but its predictive role on the final pregnancy outcome does not seem to be of major impact.

Self‐hypnosis for coping with labour pain: a randomised controlled trial
ConclusionIn this large randomised controlled trial of a brief course in self‐hypnosis to ease childbirth, no differences in use of epidural analgesia or pain experience were found across study groups. Before turning down self‐hypnosis as a method for pain relief, further studies are warranted with focus on specific subgroups.
