Increasing the Elution of Vancomycin from High-Dose Antibiotic-Loaded Bone Cement: A Novel Preparation Technique.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
CONCLUSIONS: High-dose-antibiotic bone cement prepared with delayed antibiotic addition increased vancomycin elution compared with the standard surgical preparation. Incorporating additional liquid monomer decreased vancomycin elution from high-dose-antibiotic cement. We recommend preparing high-dose-antibiotic bone cement with the delayed-antibiotic-addition technique and not incorporating additional liquid monomer. CLINICAL RELEVANCE: Both the relative volume of liquid monomer and the timing of antibiotic addition have substantial effects on the elution of antibiotic from bone cement. Abstract Fractures of the lateral clavicle and the acromion are uncommon and represent a separate entity. Fractures of the lateral clavicle demonstrate a high rate of problems, such as non-union, malunion and functional impairment when treated nonoperatively. The aim of any treatment option is full restoration of shoulder function by achieving fracture healing without significant malpositioning. Unstable fracture patterns have to be identified and should be fixed using an appropriate technique, which include plating, K-wire fixation and arthroscopic techniques which bring the dislocated fragments into contact. Abstract Failed conservative treatment or surgical procedures for acute AC joint seperations create the demand for revision or salvage procedures to successfully treat persistent AC joint instability. Besides the classic Weaver-Dunn procedure and its modifications lately new procedures combining a more or less rigid implant for primary stability with a biologic augmentation using an autologous tendon graft have been developed. These procedures are performed arthroscopically or arthroscopically assisted. PMID: 23007125 [PubMed - as supplied by publisher]

Abstract Regarding imaging diagnostics in case of posttraumatic or postoperative chronic osteomyelitis, x-ray should be the first choice to obtain basic information of the affected bone. Because knowledge of the exact dimensions of the inflammatory changes is essential for adequate therapy planning, diagnostics via cross-sectional imaging is indicated in most cases. In order to adequately estimate the lesion's dimensions, magnetic resonance imaging (MRI) is the method of choice in these cases. Nuclear medicine examination techniques such as bone scintigraphy using specially marked granolocyte antibodies are alterna...

Abstract Treatment for rib fracture pain can be broadly divided into pharmacologic approaches with oral and/or parenteral medication and interventional approaches utilizing neuraxial analgesia or peripheral nerve blocks to provide pain relief. Both approaches attempt to control nociceptive and neuropathic pain secondary to osseous injury and nerve insult, respectively. Success of treatment is ultimately measured by the ability of the selected modality to decrease pain, chest splinting, and to prevent sequelae of injury, such as pneumonia. Typically, opioids and NSAIDs are the drugs of first choice for acute pain because of...

Abstract  Fibrodysplasia ossificans progressiva (FOP) is a rare autosomal dominant disorder characterized by progressive heterotopic ossification. FOP is caused by a gain-of-function mutation in ACVR1 encoding the bone morphogenetic protein type II receptor, ACVR1/ALK2. The mutant receptor causes upregulation of a transcriptional factor, Id1. No therapy is available to prevent the progressive heterotopic ossification in FOP. In an effort to search for clinically applicable drugs for FOP, we screened 1,040 FDA-approved drugs for suppression of the Id1 promoter activated by the mutant ACVR1/ALK2 in C2C12 cells. ...

Conclusions  During the past 25 years there has been a trend towards the use of more comprehensive and effective methods of evaluating patients with ONFH. However, during the past five years, approximately half of the published studies continued to use limited, non-quantitative methods of evaluation. This should be brought to the attention of the orthopaedic community. Future investigations and publications on ONFH should employ comprehensive methods of classification which include not only the stage of disease but also measurements of the size of the necrotic segment and the extent of femoral head ...

Conclusions  According to our results, the TAD should not exceed 25 mm in stable (AO/OTA A1) as well as unstable (AO/OTA A2) fractures. An increased anterior hip screw placement should be avoided while fracture reduction with a slight valgus Neck Shaft seems favorable. November 2010 Cognitive changes after surgery have long been noted, especially in the geriatric population. In this comparison study, the researchers investigated long-term (ie, 13-month) effects of general or regional anesthesia on the cognitive ability of 270 patients older than 65 years who had undergone orthopedic surgery between October 1998 and January 2002. The control group consisted of 310 elderly people from a longitudinal study on cognitive aging who had not had surgery and who had experienced stable cognitive functioning for the past year. The researchers investigated the effects of anesthesia over time using ...

In 2003, my colleagues and I in the anesthesiology department of a university hospital in Patras, Greece, decided to investigate the incidence of hypothermia in postoperative patients who had undergone orthopedic surgical procedures. Orthopedic procedures are of relatively short duration and do not involve open body cavities, and warming devices were used during the surgeries. Thus, we were surprised to find that as many as 74% of the patients arrived hypothermic (ie, with a core temperature

Soy protein with or without isoflavones failed to preserve bone density in gonadal hormone–deficient male rat model of osteoporosisemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Abstract: Soy with its isoflavones has been shown to positively influence bone mineral density in female ovariectomized rats; hence, we hypothesized a similar effect in orchidectomized (ORX) male rats. Forty male Sprague-Dawley rats, aged 95 days, were divided into 4 groups and were either sham operated (Sham) or ORX. The ORX groups were fed a soy protein–based diet (SOY), an isoflavone-depleted soy protein diet (SOY−), or a casein based diet for 65 days after surgery. Orchidectomy increased the rate of bone turnover, resulting in reduced bone mineral density and bone mineral content by 3.5% and 14%, respectively, and ...

CONCLUSIONS: Clinicians should be aware that hindfoot tumors can be initially misdiagnosed as soft tissue injuries and suspicion of a tumor should be raised in the absence of trauma or persistent symptoms. Lesions affecting the calcaneus are more likely to be malignant. Early diagnosis and adjuvant therapy are important. LEVEL OF EVIDENCE: Level IV, therapeutic study.
