Early Switch From IV To Oral Meds Is Effective For Children With Acute Bone Infectionemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
When treating children for acute osteomyelitis--a bacterial bone infection--an early changeover from intravenous (IV) antibiotic delivery to oral antibiotics is just as effective as continuing the IV therapy, according to pediatric researchers. In addition, the oral drugs are more convenient for children and families, and avoid a major drawback of IV use: increased risk of complications from using central catheters, such as infections or breaks in the catheter. Degenerative changes of the deltoid muscle have impact on clinical outcome after reversed total shoulder arthroplastyemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Conclusions  These data show that degeneration of the deltoid muscle is of impact for clinical outcome. Although other factors like disuse or pain as a reason for muscular changes cannot be fully excluded, this degeneration might occur after time due to biomechanical changes after reversed arthroplasty. However, prospective studies with longer follow-up periods and repetitive assessment of muscle quality are necessary to further support these results. Extremity soft tissue sarcomas presented as hematomasemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Conclusion  We suggest that an STS masquerading as hematoma should be suspected when the mechanism and the energy of the trauma do not justify the clinically detected severity of the injury, or the lesion does not follow the expected clinical course of resolution after initial conservative management. Bleeding predisposition does not exclude malignancy. The evacuation of hematomas should include pathologic examination of tissue. Prognosis is dismal due to early metastatic disease. We report on the case of an 81-year-old female patient who developed a squamous cell carcinoma in a long-lasting therapy-resistant crural ulcer of the lower leg due to posttraumatic chronic osteomyelitis. Eventually the lower leg had to be amputated because of massive destruction of soft tissue and the tibia bone. Based on the desolate wound conditions plastic reconstruction by a pivoted muscle flap and surgical covering with mesh graft plastic was also necessary. The amputation stump had completely healed 6 months after surgery and the patient is fitted with an artificial limb ensuring independent mobility even outside the...

Data collected from 200 patients reported for repair of their complete dentures. Data was collected from patients, aged between 30 to 80 years (mean 55 plus/minus 25 years), from both the genders. Investigations were done on factors causing the fracture. After the analysis it was observed that the ratio of fracture of upper to lowe...

Abstract  In the present report, we describe the very rare occurrence of soft tissue metastases from sarcoma at four sites in three patients, focusing on the clinical course and radiological features. All soft tissue masses occurred as a result of disseminated metastases from malignant sarcomas. The initial symptoms presented as painful masses. Magnetic resonance imaging demonstrated a non-specific appearance for the skeletal metastases derived from the sarcomas, corresponding to the signal of the primary lesion. We reviewed 25 patients who had undergone resection of a primary bone sarcoma which extended to within 5 cm of the knee with reconstruction by a combination of a free vascularised fibular graft and a massive allograft bone shell. The distal femur was affected in four patients and the proximal tibia in 21. Their mean age at the time of operation was 19.7 years (5 to 52) and the mean follow-up period 140 months (28 to 213). Three vascularised transfers failed. The mean time to union of the fibula was 5.6 months (3 to 10) and of the allograft 19.6 months (10 to 34). Full weight-bearing was allowed at a mean of 21.4 months...

Extracellular ATP activates MAP kinase cascades and increases intestinal Caco-2 cell proliferationemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
We examined the role of ATP in the activation of the MAP kinases (MAPKs) ERK1/2, JNK1/2, and p38 and their involvement in the modulation of transcription factors and proliferation of human colon cancer Caco-2 cells. ATP induces the phosphorylation of MAPKs in a time- and dose-dependent manner. Moreover, UTP and ATPgS but not ADP or ADPβS increased phosphorylation of MAPKs, indicating the involvement of, at least, P2Y2/P2Y4 purinergic receptor subtypes. RT–PCR studies and PCR product sequencing supported the expression of P2Y2 and P2Y4 receptors in this cell line. Spectrofluorimetric measurements showed that cell stimula...

Strontium ranelate for the treatment of osteoporosis: Starting at which age? Two clinical casesemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Strontium ranelate has emerged in the last few years as a first-line treatment for postmenopausal and senile osteoporosis. Its dual mechanism of action, which combines osteoblastic stimulation and osteoclastic inhibition (anabolic and antirresorptive at the same time), makes it an important element in the therapeutic armamentarium for osteoporosis. There is no published information about its use in young patients with osteopenia. We herein show the experience in two patients with secondary osteoporosis: a young male (aged 27 years) with hypopituitarism who persisted osteoporotic after replacement with testosterone and a p...

Abstract  Intermittent exogenous parathyroid hormone (PTH) is a potent osteoanabolic agent used for the treatment of severe osteoporosis. Two molecules of recombinant PTH are commercially available: the full-length PTH (PTH 1–84) and teriparatide (PTH 1–34). We present the first report of PTH-induced mild, asymptomatic, normochromic normocytic anemia in a postmenopausal woman treated sequentially with PTH 1–84 and PTH 1–34. Anemia was more pronounced with PTH 1–84 compared to PTH 1–34 and was reversed with each regimen discontinuation. We suggest monitoring of hematocrit and hemoglobin in PTH-trea...
