I’m tired of stress, I prefer fatigue
As a medical student, I was taught about an entity known as a “march fracture”. This term was coined, not because it occurred in Spring, or was described by a Dr March, but because new army recruits, sent on long and punishing route marches burdened by heavy packs, would develop unusual fractures of their metatarsals. Plus ça change, plus ćest la même chose. Such a lesion would often present with chronic pain or aching in the forefoot, and, when eventually obtained, radiographs would reveal a healing metatarsal shaft fracture, usually undisplaced and with obvious callus formation. Since my undergraduate days, the so...

Neurological examination of the peripheral nervous system to diagnose lumbar spinal disc herniation with suspected radiculopathy: a systematic review and meta-analysis
Conclusions: This systematic review and meta-analysis demonstrate that neurological testing procedures have limited overall diagnostic accuracy in detecting disc herniation with suspected radiculopathy. Pooled diagnostic accuracy values of the tests were poor, whereby all tests demonstrated low sensitivity, moderate specificity, and limited diagnostic accuracy independent of the disc herniation reference standard or the specific level of herniation. The lack of a standardized classification criterion for disc herniation, the variable psychometric properties of the testing procedures, and the complex pathoetiology of lumbar...

Optimism lowers pain: evidence of the causal status and underlying mechanisms. Hanssen MM, Peters ML, Vlaeyen JW, Meevissen YM, Vancleef LM. Pain 2013;154(1):53–8. Epub 2012 Oct 18
This study sought to test the causal status by experimentally inducing a temporary optimistic state by means of writing about and visualizing a future best possible self. In addition, we explored pain expectations and (situational) pain catastrophizing as possible underlying mechanisms of the link between optimism and pain. Seventy-nine university students participated in a cold pressor task (CPT). Before the CPT, half of them received the optimism manipulation and the other half a control manipulation. Induced optimism was related to lower pain intensity ratings during the CPT compared to the control group, thereby experi...

Recombinant human bone morphogenetic protein‐type 2 (rhBMP‐2) enhances local bone formation in the lumbar spine of osteoporotic sheep
ABSTRACT The failure of orthopedic implants in osteoporotic patients is attributed to the lack of sufficient bone stock and regenerative capacity but most treatments for osteoporosis fail to address this issue. rhBMP‐2 is known to promote bone formation under normal conditions but has not been used clinically in the osteoporotic condition. Osteoporosis was induced in 19 ewes using ovariectomy, low calcium diet, and steroid injection. After induction, the steroid was withdrawn and pellets containing inert carrier with rhBMP‐2 in either slow or fast‐release formulation were implanted into the lumbar vertebrae of each a...

US spending on complementary and alternative medicine during 2002–08 plateaued, suggesting role in reformed health system. Davis MA, Martin BI, Coulter ID, Weeks WB. Health Aff (Millwood) 2013;32(1):45–52
Complementary and alternative medicine services in the United States are an approximately $9 billion market each year, equal to 3 percent of national ambulatory health care expenditures. Unlike conventional allopathic health care, complementary and alternative medicine is primarily paid for out of pocket, although some services are covered by most health insurance. Examining trends in demand for complementary and alternative medicine services in the United States reported in the Medical Expenditure Panel Survey during 2002–08, we found that use of and spending on these services, previously on the rise, have largely plate...

The role of fear of movement in subacute whiplash-associated disorders grades I and II. Robinson JP, Theodore BR, Dansie EJ, Wilson HD, Turk DC. Pain 2013;154(3):393–401. Epub 2012 Dec 1
This study examined the role of fear after WAD and assessed the effectiveness of 3 treatments targeting fear. People still symptomatic from WAD grade I-II injuries approximately 3months previously (n=191) completed questionnaires (eg, Neck Disability Index [NDI]) and were randomized to 1 of the treatments: (1) informational booklet (IB) describing WAD and the importance of resuming activities, (2) IB+didactic discussions (DD) with clinicians reinforcing the booklet, and (3) IB+imaginal and direct exposure desensitization (ET) to feared activities. DD and ET participants received three 2-hour treatment sessions. Absolute im...

Clearance of the thoracolumbar spine—A clinical decision rule is needed
The initial assessment and imaging of severely-injured patients is controversial. Most hospitals have developed protocols for the reception of potential major trauma victims based largely on experts’ opinion with limited rigorous prospective outcome studies to support the varied approaches. Over the past decade computerised tomography (CT) scanning has been increasingly used in most hospitals to rapidly diagnose injuries to all parts of the body, especially the head, neck and torso. Many hospitals have designed their emergency and trauma facility to enable rapid access to a CT scanner for even unstable patients who may n...
