MicroRNAs and lung development
Abstract MicroRNAs (miRNAs) constitute a large group of small (∼22 nucleotides), non‐coding RNA sequences that are highly conserved among animals, plants and microorganisms, suggesting that microRNAs represent a highly conserved and important regulatory mechanism. They have been demonstrated to play an important role in gene regulation. Recently, miRNAs have become a major focus of interest for research in organ development. Research focusing on the potential role of microRNAs during lung development is slowly starting to emerge. A number of miRNAs have been demonstrated to play important roles during early and late lu...

Current Guideline For The Neurodiagnostic Evaluation Of The Child With A Simple Febrile Seizure
In this issue of EM Practice Guidelines Update, the 2011 American Academy of Pediatrics (AAP) update regarding neurodiagnostic evaluation of pediatric simple febrile seizures is reviewed. Simple febrile seizures are largely benign events with good prognoses, and they often do not require laboratory studies or neurodiagnostic imaging to be performed in the emergency department (ED). This update of the 1996 AAP guideline focuses on the use of lumbar puncture in children who present to the ED with a simple febrile seizure.

[Cerebral near-infrared spectroscopy (NIRS) in paediatric anaesthesia.]
Abstract Cerebral oximetry allows continuous real-time and non-invasive monitoring of cerebral oxygen saturation (cSO(2)), by measuring oxyhaemoglobin and deoxyhaemoglobin near infrared light absorption, similarly to pulse oximetry. cSO(2) measurement predominantly reflects brain venous compartment, and is correlated with jugular venous saturation. As jugular venous saturation, cSO(2) must therefore be interpreted as a measure of balance between transport and consumption of O(2) in the brain. Cerebral oximetry should be used as a trend monitoring, because its accuracy is insufficient to be considered as reliable me...

A worldwide collaboration to harmonize guidelines for the long‐term follow‐up of childhood and young adult cancer survivors: A report from the international late effects of Childhood Cancer Guideline Harmonization Group
Abstract Childhood and young adult cancer survivors should receive optimum care to reduce the consequences of late effects and improve quality of life. We can facilitate achieving this goal by international collaboration in guideline development. In 2010, the International Late Effects of Childhood Cancer Guideline Harmonization Group was initiated. The aim of the harmonization endeavor is to establish a common vision and integrated strategy for the surveillance of late effects in childhood and young adult cancer survivors. With the implementation of our evidence‐based methods, we provide a framework for the harmonizatio...

Decision making in pediatric oncology: Evaluation and incorporation of patient and parent preferences
Abstract Decision making in pediatric cancer is particularly difficult. There may be uncertainty about prognosis, and long‐term survival estimates from trials may not be applicable to current patients. There are many motivations to conduct research into patient and provider decision making. This review discusses three approaches to understanding decision making, namely decision analysis, the threshold technique and discrete choice experiment. These techniques are applied to situations in pediatric oncology to illustrate how the results may be useful for patient care. Future work in pediatric oncology decision making shou...

Risks of Noncardiac Operations and Other Procedures in Children With Complex Congenital Heart Disease [ORIGINAL ARTICLES: PEDIATRIC CARDIAC]
Conclusions Within this population, we have identified independent risk factors for specific clinical outcomes. Patients before stage II palliation, undergoing more invasive procedures, and receiving inotropes, angiotensin-converting enzyme inhibitors, or digoxin appear to be at risk for intraoperative hemodynamic instability. Patients with preoperative hospital LOS exceeding 14 days appear to be at greater risk for requiring postoperative mechanical ventilation. Patients with preoperative LOS exceeding 14 days, with ventricular dysfunction, receiving inotropes, and not receiving digoxin appear to be at risk for protracted...

Gastrointestinal Complications After Stage I Norwood Versus Hybrid Procedures [ORIGINAL ARTICLES: PEDIATRIC CARDIAC]
Conclusions Gastrointestinal complications are common after initial palliation of patients with hypoplastic left heart syndrome. Preoperative and perioperative factors play a significant role. Despite a more "limited" intervention, patients undergoing hybrid procedures remain at high risk for feeding issues, commonly leading to a prolonged hospital course. Close attention and proactive management are essential to optimize the nutritional status in these patients.

Soluble CD30 and ELISA‐detected human leukocyte antigen antibodies for the prediction of acute rejection in pediatric renal transplant recipients
Summary Biomarker‐based post‐transplant immune monitoring for the prediction of impending graft rejection requires validation in specific patient populations. Serum of 28 pediatric renal transplant recipients within the framework of a well‐controlled prospective randomized trial was analyzed pre‐ and post‐transplant for soluble CD30 (sCD30), a biomarker reflecting mainly T‐cell reactivity, and anti‐human leukocyte antigen (anti‐HLA) antibody reactivity, a biomarker for B‐cell activation. A sCD30 concentration ≥40.3 U/ml on day 14 was able to discriminate between patients with or without biopsy‐proven...

Fibular Stress Fracture in a High School Athlete
Elliot M. Greenberg, Nicholas Gohn, Matthew GradyThe patient was a 15-year-old adolescent male who was referred to a physical therapist for a chief complaint of bilateral posterolateral lower-leg pain, which was worse in the right lower extremity than in the left. Due to findings that were concerning for a stress fracture, the patient was referred to a pediatric sports medicine physician. Subsequent radiographs revealed findings that were concerning for a stress fracture along the medial aspect of the midshaft of the right fibula.J Orthop Sports Phys Ther 2013;43(1):29. doi:10.2519/jospt.2013.0401KEY WORDS: lower leg, radiography

Collaboration with medical professionals in clinical practice: Pediatric abdominal pain as a case example.
Pediatric abdominal pain is a common childhood complaint. Available conceptualizations of the condition point toward an interdisciplinary approach to care as having the highest utility. The current study sets out to describe practice patterns among psychologists treating pediatric abdominal pain, the degree to which such practitioners engage in collaborative behaviors, barriers they experience to collaboration, and their overall satisfaction with their collaborative efforts. A survey of American Psychological Association members revealed that psychologists are engaging in the treatment of pediatric abdominal pain in high n...

The lung is involved in juvenile dermatomyositis
ConclusionThe frequency of lung involvement in children with JDM justifies systematic respiratory assessment with PFT including measures of respiratory muscle strength. Endobronchial nocardiosis in an 11‐year‐old child
Abstract Pulmonary nocardiosis is a rare infection and overwhelmingly limited to immunocompromised individuals. Endobronchial nocardiosis is even more rare. Nocardiosis is rarely seen in immunocompetent hosts mostly as cutaneous infection. Here we report an immunocompetent child with endobronchial nocardiosis presenting as nonresolving pneumonia and lung abscess. To our knowledge, this is the first case reported in the literature of endobronchial nocardiosis in a child. Pediatr Pulmonol. © 2012 Wiley Periodicals, Inc.
