A comparison of different assessment techniques for measuring foot and ankle volume in healthy adults.

Conclusions: Although any of these techniques should be acceptable for monitoring foot and ankle volume in normal limbs, the figure-of-eight method comes closest to reproducing the results of water volumetry. We believe that this technique would also be best in the presence of foot deformities, but this remains to be studied.

We report a comparison of orthotic materials to manage foot pain in a 59-year-old man with type 1 diabetes mellitus, peripheral neuropathy, peripheral arterial disease, and a history of foot ulceration. We investigated a range of in-shoe foot orthoses for comfort and plantar pressure reduction in a cross-sectional study. The most comfortable and most effective pressure-reducing orthoses were subsequently evaluated for pain relief in a single system alternating-treatment design. After 9 weeks, foot pain was completely resolved with customized multidensity foot orthoses. The outcome of this case study suggests that customize...

Hyperhidrosis is defined as excessive and uncontrollable sweating due to overactivity of the eccrine sweat glands. The first line of treatment for plantar hyperhidrosis consists of conservative therapies such as topical solutions (ie, antiperspirant applications and aluminum chloride preparations) and iontophoresis. When the patient has failed these standard treatments, the other available medical options are rather limited and not well tolerated. Botulinum toxin type A (Botox, Allergan Inc, Irvine, California) is a purified neurotoxin complex approved by the US Food and Drug Administration in 2004 for multiple medical...

Conclusions: Transfer of the flexor digitorum brevis tendon to the dorsum of the proximal phalanx can be performed for correction of claw or hammer toe deformities, especially in the second, third, and fourth toes. The transverse aponeurotic fibers originating from the extensor digitorum longus impede the transfer of the flexor digitorum brevis tendon, and meticulous excision of these fibers is essential to the success of the procedure.

The Charcot foot (osteoarthropathy) is a significant lower extremity complication of diabetes mellitus that can result in significant deformity, ulceration, and subsequent limb loss. A result of even unrecognized trauma to an insensitive foot, continued weight bearing on the injured foot promotes the evolution of the disorder that is often diagnosed only after significant deformity has occurred. Although this entity is considered a rare complication of the diabetic population, it has profound implications for those persons affected. Greater awareness of the frequency, distribution, determinants, and natural history of ...

The Charcot syndrome is a rare complication of neuropathy in diabetes and is characterized by an acute inflammatory episode of the foot that is associated with variable degrees of dislocation, fracture, and deformity. It has no single cause but represents the final common pathway in people who are predisposed to its development by the varying overlap of several different factors. The association of the active phase of the disease with inflammation, increasing osteopenia, and increasing calcification of the arterial walls strongly suggests, however, the involvement of the receptor activator of nuclear factor-kappaB liga...

We present the hallmarks for diagnosis of Charcot foot based on the clinical examination and imaging studies. We provide a simple algorithm based on evidence and experience for the investigation of the foot when Charcot arthropathy is suspected. Additionally, we propose a new classification that accounts for the degree of complications in the Charcot joint. This new system considers deformity, ulceration, and osteomyelitis, which may help to predict amputation.

The goal of this article is to outline the progression of Charcot's neuroarthropathy. A historical background is detailed, with an emphasis on the current trends in the literature. Acute Charcot's neuroarthropathy requires a prompt diagnosis with the proper practical offloading methods of the affected limb. This, coupled with directed medical and surgical approaches designed to prevent or mitigate deformities, may subsequently reduce the risk of amputation in this high-risk population.

The cornerstone of treatment of acute Charcot neuroosteoarthropathy is immediate effective offloading, typically with total contact casting, and reduction in weight bearing. The targets of pharmacological intervention are inhibition of excess osteoclast activation and suppression of an excess proinflammatory cytokine response. Antiresorptive therapy, especially with bisphosphonates, has been used in randomized trials. While evidence of an ideal dosage regime and significant differences in long-term outcome are lacking and should be evaluated in future studies, the trials so far demonstrated improved symptom control, a ...

Charcot arthropathy places individuals at risk of developing diabetic foot ulcers and potentially subsequent limb amputation by means of altering the anatomy of the foot and ankle. Physical trauma is an important component to the etiology of the condition. The physical management of the Charcot foot is concerned with minimizing the stress applied to the affected foot and ankle skeletal structure. The most appropriate device is temporally dependent on the progression of the disease. At the initiation of Charcot arthropathy, care by total contact cast is recommended. As the affected bones begin to heal, use of a removabl...
