Total Extrusion of the Cuboid: A Case Report
The incidence of total extrusion of the cuboid is rare, without any known published data or surgical guidelines. This case report describes the management of an open extruded cuboid by staged surgical interventions. Arthrodesis of the lateral column with a structural bone graft is a viable option to address the shortening, instability, and severe bone loss caused by the total cuboid extrusion.

Pilon Fractures
The nature of the pilon fracture has caused evolution of treatment methods and its historically high rate of complication and poor outcome continue to direct choice of treatment. Attention to the delicate soft tissue envelope surrounding the ankle and recognition of the severity of the initial injury is crucial to ensure a satisfactory outcome and to minimize complications. Understanding the importance of staging surgical interventions will help to improve outcomes, but even optimal treatment may result in less than satisfactory results.

Foot and Ankle Trauma
This issue of Clinics in Podiatric Medicine and Surgery brings together experts in the field of foot and ankle trauma to provide us with the most current treatments and fixation methods available for the management of the traumatic patient. Trauma as it relates to the foot and ankle can range from commonly encountered low-energy fractures to severely deformed and mutilated injuries that can be quite challenging before deciding on an effective treatment plan. Throughout this issue, numerous fractures such as the talus, calcaneus, pilon, and Lisfranc’s injuries are reviewed in detail. In addition, the management of crush i...

Cost-effectiveness of cryotherapy versus salicylic acid for the treatment of plantar warts: economic evaluation alongside a randomised controlled trial (EVerT trial)
An economic evaluation of the EVerT randomised trial of plantar wart treatment indicates that cryotherapy costs on average GBP101 more per participant over the 12 week time-frame, while there is no additional benefit in terms of proportion of patients healed compared with salicylic acid.

Tarsometatarsal/Lisfranc Joint
Accurate early diagnosis with adequate reduction and maintenance of anatomic alignment of the dislocation or fracture within the Lisfranc joint complex have been found to be the key to successful outcomes regarding this injury. Because of the anatomic variations, the thin soft tissue envelop, and the abundance of ligamentous and capsular structures in the region, repair of these injuries can be a challenge. The classification systems used to describe these injuries aid in describing the mechanism of injury or displacement type present, which may aid in determining what treatment modality can provide the best outcome.

Letter to the Editor
Thank you, Dr Bregman, for your comments concerning our recent article, “Radiofrequency thermoneurolysis for the treatment of Morton's neuroma.” Numerous descriptions for Morton's neuroma can be identified in the literature. Definitions range from a benign growth of the sheath of a nerve that courses between the toes, as was written by Jonathan Cluett, MD, to the Mayo Clinic's definition of it as a thickening of the tissue around one of the nerves leading to the toes. In our opinion, the symptoms of a neuroma are not caused by entrapment but instead are caused by swelling, which led us to investigate whether or not rad...

Managing diabetes in dialysis patients
Burgeoning levels of diabetes are a major concern for dialysis services, as diabetes is now the most common cause of end-stage renal disease in most developed nations. With the rapid rise in diabetes prevalence in developing countries, the burden of end stage renal failure due to diabetes is also expected to rise in such countries. Diabetic patients on dialysis have a high burden of morbidity and mortality, particularly from cardiovascular disease, and a higher societal and economic cost compared to non-diabetic subjects on dialysis. Tight glycaemic and blood pressure control in diabetic patients has an important impact in...

Psychologists' medical staff membership in academic healthcare: Past, present, and future challenges.
This study surveyed 311 psychologists practicing in academic health centers about their status on the medical or professional staff, institutional privileges and governance, and other issues relevant to practice in medical settings. Data from the survey revealed that only 36.7% of respondents held full membership on the medical staff of their facility, 22.9% were classified as allied health staff, 14.5% were limited members of the medical staff, and 13.8% fell within a category of professional staff. The advent of health care reform and a call for interprofessional collaboration provides a renewed opportunity for psycholog...

The management of EGFR inhibitor adverse events: a case series and treatment paradigm.
Conclusion  The most common cutaneous adverse event in our cohort was papulopustular rash, followed by eczema and xerosis. Patients were managed with symptom target therapy, and suspension of the EGFR inhibitor was rarely required. As the use of EGFR inhibitors increases, it is important to promptly identify and treat adverse events. Further studies are necessary to develop targeted therapeutic and preventative measures.

Arthrodesis of the first metatarsophalangeal joint: comparison of three techniques.
Conclusions: Our experience corroborates the advantages of Memory staples as described in the literature, including good approximation of bone fragments, technically easy application with fewer steps than an AO-applied screw, and an adequate source of internal fixation to achieve metatarsophalangeal joint fusion. There is also a suggestion that the time to achieve fusion is shorter. PMID: 22232316 [PubMed - in process]

Effect of a metatarsal pad on the forefoot during gait.
Conclusions: The combination of increased forefoot width and the height of the second metatarsal head produced by the metatarsal pad results in an increase in space between the metatarsal heads. This extra space could play a role in pain reduction produced by a metatarsal pad. PMID: 22232317 [PubMed - in process]

Navicula drop test ad modum brody: does it show how the foot moves under dynamic conditions?
Conclusions: This study of asymptomatic individuals did not confirm that static ND can be used to individually predict dynamic ND during treadmill walking. It was demonstrated that the start position of Brody's test is not well correlated with navicular height at heel strike, with this being the main reason for the weak relationship between static and dynamic ND measures. Normal genetic variation of the human foot: part 1: the paradox of normal anatomical alignment in an evolutionary epigenetic context.
Abstract Molecular genetics is changing our understanding of the developmental translation of genotype to phenotype between and within different phylogenetic groups. Together with a growing understanding of our own evolutionary relationships to common ancestors, the epigenetic processes involved enforce a reexamination of what is regarded as a normal foot structure. A revised populationist approach is proposed and supported by paleoanthropologic evidence that reflects a picture of emerging suitability for bipedalism that is driven by natural genetic divergence. Dislocation of the fifth metatarsal base following partial fourth and fifth ray amputation: a case report.
Abstract This case report presents a rare postoperative dislocation of the fifth metatarsal base following a healed open partial fourth and fifth ray amputation of a 62-year-old male veteran with poorly controlled diabetes mellitus. The dislocated fifth metatarsal base subsequently created a chronic ulceration and an inhibition of normal gait. The patient was taken to the operating room where the fifth metatarsal base was resected with transfer of the peroneus brevis tendon to the cuboid to maintain biomechanical stability. Lisfranc Arthrodesis
Lisfranc fracture-dislocations are complex injuries that require a high skill set from foot and ankle surgeons to diagnose and treat. Conservative treatment is seldom an option for treatment of Lisfranc injuries. The authors believe that even subtle injuries require surgical intervention.
