Prostate Brachytherapy: The “Can't Miss” Approach for Patients Undergoing Definitive Radiation for Prostate Cancer?
In this issue, Williams et al publish their single-institution experience comparing biochemical outcomes in intermediate- and high-risk prostate cancer patients based on whether they received 3-dimensional conformal external beam radiation therapy (EBRT) alone versus a combination of external beam radiation therapy to the prostate and seminal vesicles together with a high-dose-rate (HDR) brachytherapy boost. With a median follow-up time of just over 5 years, they were able to demonstrate improved biochemical control for those patients who received HDR brachytherapy as a component of their treatment (5-year rates: 79.8% vs...

Oncology Scan—Thoracic Cancers
We have decided to focus on 1 topic for the thoracic cancers-themed “Oncology Scan,” specifically the role (or lack thereof) of thoracic radiation therapy (RT) for operable, pathologically proven stage IIIA/N2 non-small cell lung cancer (NSCLC). This encompasses 2 scenarios: (1) microscopic IIIA/N2 NSCLC found after resection of NSCLC: in this setting, there is now agreement that fit patients should receive some type of adjuvant chemotherapy, but little consensus otherwise; and (2) clinically diagnosed stage IIIA/N2 NSCLC, in which there is general agreement that patients should not proceed to surgery immediately, but ...

In Regard to Mast et al
To the Editor: Mast et al suggest that patients with breast cancer might be at higher risk for coronary artery disease (CAD) as shown by their higher coronary artery calcium (CAC) scores . Their findings, however, may be due to chance. We would also be interested in the authors' opinion of what might be the mechanism behind their finding. Artifactual aortic coarctation: a processing error in aortic magnetic resonance angiography.
Conclusion Sculpted removal of the heart and left pulmonary veins can improve the accuracy of the MIP generation process. Physicians should be alerted to the possibility of a postprocessing error when interpreting MRA datasets. PMID: 23322860 [PubMed - in process] (Source: Radiologic Technology)

In Reply to Nieder
To the Editor: We thank Nieder for his thoughtful comments on our recent article that showed an inverse relationship between acute toxicity and biochemical control in prostate cancer patients treated with radical radiation therapy. In Reply to Gyenes and Butler
To the Editor: We thank Gyenes and Butler for their interest in our recent publication, and we thank them for confirming the importance of an evidence-based approach to estimate pre-radiation cardiovascular risks and to attempt to reduce the long-term complications of patients who received radiation therapy for breast cancer . They raise the question as to which mechanism could be pointed out with respect to the observed higher risk for coronary artery disease in breast cancer. In the literature, a high body mass index (BMI) and a low level of physical activity were defined for the risk of developing breast cancer as well ...

In Regard to Vesprini et al
To the Editor: Vesprini et al recently analyzed biochemical failure-free rate (BFFR; American Society for Radiation Oncology definition) and acute toxicity in 362 patients with localized prostate cancer treated with image guided radiation therapy. At 5 years, 76% of patients who reported acute toxicity scores (Source: International Journal of Radiation Oncology * Biology * Physics)

Spinal Cord Hypothermia without Systemic Hypothermia [SPINE]
CONCLUSIONS: While the data are preliminary in a small group of animals, the ability to rapidly create a wide range of controlled spinal cord hypothermia while preserving normal body temperature warrants wider exploration. The study also indicates that further investigation of the hypothesis that CSF temperature monitoring may be an acceptable surrogate for direct spinal cord temperature monitoring should be pursued. Immediate Pain Response to Interlaminar Lumbar Epidural Steroid Administration: Response Characteristics and Effects of Anesthetic Concentration [SPINE]
CONCLUSIONS: An immediate pain-extinction response is identified after LESI, which appears independent of anesthetic strength. This observation may relate to pain origin and/or pain nociceptor afferent pathway in an individual patient and potentially relate to treatment response. (Source: American Journal of Neuroradiology)

Trends in Spinal Pain Management Injections in Academic Radiology Departments [SPINE]
CONCLUSIONS: Most responding academic radiology departments perform SPMIs. Most fellows and just more than half of residents at SPMI-performing departments achieve SPMI proficiency. For the most part, the number of SPMIs performed in responding departments has been stable during the past 5 years. (Source: American Journal of Neuroradiology)

Head and Neck Arteriovenous Malformations: Results of Ethanol Sclerotherapy [HEAD & NECK]
CONCLUSIONS: Ethanol sclerotherapy has potential for complete eradication of head and neck AVM with low recurrence within the first year after completion of treatment. MRI of carriers of the apolipoprotein E e4 allele–evidence for structural differences in normal‐appearing brain tissue in e4+ relative to e4– young adults
We present the first study to show subtle differences in the brains of young healthy adult e4+ and e4– subpopulations. These differences were detected in normal‐appearing brain tissue using a range of sophisticated and novel structural imaging techniques. We suggest that such subtle differences in white matter integrity may confer neural efficiency advantages, and hence give rise to the performance differences in tests of verbal fluency. Comparison of Carotid Plaque Ulcer Detection Using Contrast-Enhanced and Time-of-Flight MRA Techniques [EXTRACRANIAL VASCULAR]
