CANCELED - NIBIB 10th Anniversary Symposium
Please join us in celebrating NIBIB’s Tenth Anniversary, with events to be held in Bethesda, Maryland on Thursday evening June 21 and Friday June 22, 2012. The first NIBIB award was made in April 2002 to a team of Yale bioengineers, neurologists, neuroimaging and computer visualization scientists, biochemists, mechanical engineers and neurosurgeons. This team has since dramatically advanced the standard of diagnosis and treatment of epilepsy using on-line, real-time, updated intra-operative biochemical and functional 3D image guidance. In this spirit we have chosen as our theme, "A Decade of Innovation for Health". A d...

EUS Learning Series: Gastric cancer EUS staging
Download Video Radiology Description: Staging of gastric cancer. A 73 year old man underwent EGD for gastroesophageal reflux disease. It revealed a 3 cm antral mass; biopsies were positive for cancer. He was referred to our center for tumor staging. It consisted of a CT scan to rule out metastasis and an EUS for locoregional staging. Let us look at the CT scan first. As you can see here, there is an antral nodular mass on CT scan. On endoscopy, a nodular ulceratd mass was noted with folds converging to the mass. Biopsies were taken and subsequently an EUS was performed with a radial echo...

EUS Learning Series: Linitis Plastica
Download Video Pathology Radiology Description: In this EUS Learning Series, the next step is Linitis Plastica. A 55 year old man is referred for evaluation of gastric cancer diagnosed at an outside facility. He had undergone EGD, EUS and probe sonography; a diagnosis of Linitis Plastica was made. We would like to show stomach anatomy in health and in Linitis Plastica to appreciate the features. Anatomy of normal stomach: here you can see a nicely contracting antral wave of peristalsis going through the antrum, and a wide open antrum with normal mucosa, without any ulcer...

Multiple Sectoral Metallic Biliary Stent Placement for Hilar Tumors
Download Video Radiology Description: This is a 59 years old patient who was recently diagnosed with pancreatic adenocarcinoma metastatic to the liver. MRCP showed bilateral intrahepatic biliary dilation with abrupt central termination (these findings were compatible with Bismuth IV tumor). He underwent PTCA with right liver external-internal drain placement, and ERCP with left liver plastic stenting. He presented to us with rising LFTs due to accidental displacement of stents. The right percutaneous drain was replaced by the IR team. The scout film demonstrates a completely upmigrated p...

Sprial enteroscopy assisted pancreatoscopy for the diagnosis of obscure overt bleeding in a patient with surgically altered anatomy
We present the case of a 66 y o gentleman who had chronic pancreatitis and abdominal pain who underwent a pancreatic head resection and a Peustow procedure. The post-operative course was complicated by the development of an intra-abdominal hematoma. The hematoma is shown by the arrow here on the CT scan. An angiogram demonstrated an aneurysm of the pancreatico-duodenal artery which was treated with coil embolization. He presented 4 weeks later with melena and abdominal pain. On physical exam he was tachycardic, pale, hypotensive with epigastric tenderness and a hemoglobin of 6.9. A push enteroscopy to the entero-enteral an...

Gastroduodenal Intussusception Caused by a Gastric Hyperplastic Polypoid Mass
We present a case of 31years old female who was evaluated for intermittent episodes of nausea, non-bilious vomiting & upper abdominal discomfort for more than one year. Her past medical history was significant for resolved Parotid and Thyroid cancers. Remaining history and review of the systems were unremarkable. Her physical examination was only significant for the pallor. Laboratory data revealed severe iron deficiency anemia. Hemoglobin of 6.2 gm/dl, MCV of 68.4 fl, Ferritin of 8.2 ng/ml, normal prealbumin and stool occult was faintly positive. Other routine laboratory work up was normal. CT ABDOMEN: A CT scan of th...

Band Ligation Endoscopic Mucosal Resection of a Distal Esophageal Granular Cell Tumor
Download Video Pathology Radiology Description: Granular cell tumors (GCT) are relatively uncommon, but can occur through out the length of the gastrointestinal tract, particularly in the esophagus (1). Endoscopically, they appear as firm yellow nodules and can be mistaken for lipomas, carcinoid tumors and gastrointestinal stromal tumors. GCT generally behave in a benign manner, although malignant degeneration can occur about 3% of the time (2). Tumors that are less than 1 cm in size and that are confined to the superficial parts of the gastrointestinal tract, not involv...
