Caring for children with physical disability in Kenya: potential links between caregiving and carers' physical healthemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Conclusions  Carers of children with moderate‐severe motor impairments live with their own physical health challenges. While routine assessments lead to diagnosis of simple musculoskeletal pain syndromes, the overall health status and situation of carers may be more complex. As a consequence, the role of rehabilitation therapists may need to be expanded to effectively evaluate and support carers' health needs. The provision of equipment to improve their child's mobility, respite care or transport to enable school attendance is likely to be helpful to carers and children alike.

In conclusion, our results suggest that CCF may be a viable method to enhance the effectiveness of swallowing‐related muscles by changing neck position. When CCF is applied, the stability of the deep flexor muscles must be secured first after which superficially located muscles may better assist swallowing with less effort.

Rehabilitation and treatment perspectives and interventions have concentrated efforts on areas where perpetrators of sexual abuse are deficient, neglecting those where offenders actively seek and strategically plan sexual offence situations and scenarios. Abstract The United Nation's Millennium Development Goals do not explicitly articulate a focus on disability; similar failures in the past resulted in research, policy, and practice that are not generalizable and did not meet the needs of persons with disabilities since they were developed for an "average" population. Academics and professionals in health and other disciplines should have a knowledge base in evidence-based practices that improve well-being and participation of people with disabilities through effective service delivery of assistive technology. Grounded by a theoretical framework that incorporates a...

Abstract The ability of the human brain to learn is exceptional. Yet, learning is typically quite specific to the exact task used during training, a limiting factor for practical applications such as rehabilitation, workforce training, or education. The possibility of identifying training regimens that have a broad enough impact to transfer to a variety of tasks is thus highly appealing. This work reviews how complex training environments such as action video game play may actually foster brain plasticity and learning. This enhanced learning capacity, termed learning to learn, is considered in light of its computat...

Abstract  In this observational study, we analyzed the long-term neuromuscular deficits of survivors of critical illness. Intensive care unit-acquired muscular weakness (ICU-AW) is a very common complication of critical illness. Critical illness polyneuropathy (CIP) and critical illness myopathy (CIM) are two main contributors to ICU-AW. ICU-AW is associated with an increased mortality and leads to rehabilitation problems. However, the long-term outcome of ICU-AW and factors influencing it are not well known. We analyzed the medical records of 490 survivors of critical illness, aged 18–75 years and loc...

ConclusionsComprehensive stroke units are associated with reductions in length of stay and combined death and dependency and improved functional outcomes compared to other stroke unit models. A comprehensive stroke unit model is worth considering as the preferred model of stroke unit care in the planning and delivery of metropolitan and rural stroke services.

Relationship of ultrasonographic findings with synovial angiogenesis modulators in different forms of knee arthritidesemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Abstract  Angiogenesis is controlled by a variety of angiogenesis stimulators and inhibitors. The increased power Doppler (PD) signals determined by ultrasonography is an indirect marker of synovial vascularity in arthritis. We aimed to investigate relationship between ultrasonographic findings and synovial angiogenesis modulators. Thirteen Behcet’s disease (BD), 15 spondyloarthropathy, 21 rheumatoid arthritis (RA), and 15 osteoarthritis (OA) patients with knee arthritis were included. Cumulative effusion, synovial hypertrophy, and PD signal scores were calculated in arthritic joints. In synovial fluid samp...

Lesional infiltration of receptor activator of nuclear factor-κB ligand+ cells in experimental autoimmune neuritis ratsemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Abstract  Experimental autoimmune neuritis (EAN) is a T cell-mediated autoimmune demyelinating inflammatory disease of the peripheral nervous system. Receptor activator of NF-κB ligand (RANKL), a member of the tumor necrosis factor family, regulates proliferation of mature T cells. Here, we have studied the expression of RANKL in sciatic nerves of EAN rats. EAN was induced in male Lewis rats. The spatiotemporal expression of RANKL in sciatic nerves of EAN rats was investigated using immunohistochemistry. In sciatic nerves of normal rats RANKL+ cells were rarely seen. EAN induced a significant accumulation of...

“Madam, Methinks I See Him Living Yet” (John Milton, 1644) Predicting Full Recovery After Out-of-Hospital Cardiac Arrest⁎email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Doing and saying nothing are great powers but they should not be abused. —Charles Maurice de Talleyrand-Périgord, First Prince de Bénévent () Management of patients presenting with out-of-hospital cardiac arrest is a perennially vexing problem because no categorical predictors of neurological recovery exist. Predicting the extent of brain damage permits more sensible management efforts. Adding to the complexity of clinical decision making by both family and medical staff is a stew of ethical, economic, and emotional factors admixed with legal issues. Almost 80% of patients with return of spontaneous circulation after...

Effect of transient occlusal loading on the threshold of tooth tactile sensation perception for tapping like the impulsive stimulationemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
The objectives of this study were (1) to establish a reliable method for detecting the force threshold of the tooth tactile sensation while avoiding experimenter bias and (2) to examine the effect of occlusal force loading on the threshold for impulsive force stimulation in subjects with normal dentition. Twenty volunteers participated in this study (10 males and 10 females; mean age, 26.6 ± 2.9 years). To simulate the bite force during occlusal tapping, a force-loading device was designed to exert impulsive force to the occlusal surface in the direction parallel to the tooth axis. The impulsive force d...

This article considers the late 19th-century medical invention of the category of the homosexual in relation to homosexuality’s moment of deliverance from medicine in the 1970s, when it was removed as a category of mental aberration in the Diagnostic and Statistical Manual (DSM). With the rise of the AIDS pandemic in gay communities in the early 1980s, I argue that homosexuals were forcibly returned to the medical sphere, a process I call “the painful reunion.” Reading a collection of queer narratives across the 20th century, I show that historical and contemporaneous medical events prompted the mobilization of se...
