Cilostazol prevents foot ulcers in diabetic patients with peripheral vascular disease
Abstract
Diabetic patients are at the high risk of foot ulcerations that may lead to limb amputations with important socio economic impact. Peripheral vascular disease may be frequently associated in diabetes mellitus type II with its main symptom, intermittent claudication. Many studies reported the known efficacy of cilostazol in treating vascular claudication. Metalloproteinase‐9 (MMP‐9) seems to be a biochemical marker implicated in chronic wounds and in particular in diabetic foot ulcers. Adherence to general surgery prophylaxis guidelines at Alder Hey Children's Hospital
Conclusions
This audit showed that overall adherence with the Trust guideline was low. Problems highlighted were:Prophylaxis was indicated but not prescribed in a number of cases.
Duration of antibiotics was often longer than recommended.
Antibiotic doses selected were frequently lower than that required.
Patients were not re-dosed with antibiotics during prolonged procedures.
MRSA positive patient did not receive appropriate prophylactic antibiotics.
These problems could increase the risk of surgical site infections and encourage the development of antibiotic-resistant organisms.

Cortical plasticity induced by different degrees of peripheral nerve injuries: a rat functional magnetic resonance imaging study under 9.4 Tesla
Conclusions:
After major peripheral nerve injury, an adjacent nerve, with similar function to the injured nerve, may become significantly over-activated in the cortex when stimulated. This results in intra-hemispheric cortical expansion as the only component of cortical plasticity. When all nerves responsible for a certain function are injured, the same nerves on the contralateral side of the body are affected and become significantly over-activated during a task. Hepatectomy and liver regeneration: from experimental research to clinical application
ConclusionsOur results support the view that human liver regeneration commences early, is fast, non‐anatomical and functionally complete 5 days after hepatectomy. Workplace‐based assessment in surgical training: experiences from the Intercollegiate Surgical Curriculum Programme
ConclusionsAs a consequence, there has been an increased focus upon faculty development, while some of the assessments have been redesigned in line with the PBA. A global rating scale has been introduced that uses clinical anchors. The rating scales have also been altered with a reduction in the number of ratings while an enhanced description of the complexity of the case has been introduced within the Case‐based Discussion and the Clinical Evaluation Exercise. Liver surgery in the multidisciplinary management of gastrointestinal stromal tumour
In this study, we evaluated the outcome of patients treated with TKIs followed by surgery for metastatic GIST.
MethodsEleven patients underwent liver resection after downsizing TKIs therapy for metastatic GIST from 2006 until 2010 were reviewed.

Simultaneous ventral hernia repair in bariatric surgery
This study demonstrated a low rate of mesh infection (4.44%) at a median follow‐up of 13 months, even when a resectional procedure was performed (5.56%). These results suggest the possible viability and reasonable short‐/long‐term outcomes of simultaneous laparoscopic abdominal wall hernia repair during bariatric surgical procedures, even if the surgery involved division or resection of part of the gastrointestinal tract.
