Recurrent upper gastrointestinal bleeding masquerading as gastric variceal bleeding on endoscopy after liver transplantation
Abstract
Splenic artery ligation is commonly performed today, especially during live donor liver transplantation to prevent portal hyper‐perfusion and small‐for‐size syndrome. The current case describes submucosal gastric arterial lesions that developed following splenic artery ligation during live donor liver transplantation. Those arterial lesions manifested with massive gastrointestinal bleeding four years following liver transplant and they had an endoscopic appearance similar to that of gastric varices. Cyanoacrylate, the therapeutic modality of choice for gastric varices, is associated with a significant risk o...

Intra‐operative hypercoagulability during liver transplantation as demonstrated by thromboelastography
Abstract
Thrombotic complications are more common in liver disease than might be expected given the coagulopathy described by conventional coagulation tests. Some of these complications may be life‐threatening. The phenomenon of hypercoagulation is associated with complications in many populations, but the incidence in liver transplant recipients is unclear.
A retrospective database review of intra‐operative thromboelastography (TEG) for 124 liver transplant recipients. We assessed the prevalence of hypercoagulation in this group, and investigated the relative frequency of both shortened TEG r‐time and increased G va...

Expedited liver allocation in the U.S.: A critical analysis
Conclusions: Transplantation of livers allocated by means of an expedited refusal code is asymmetric across regions and in some instances results in bypass of patients with higher waitlist priority but without notification of the bypassed center. Short‐term graft function after EP allocation was excellent. Policies governing EP allocation should be created in order to improve access to available organs. Liver Transpl, 2013. © 2013 AASLD. Selection and outcome of living donors with a remnant volume less than 30% after right hepatectomy
Abstract
Insufficient is the evidence defining the safe minimal remnant volume after living donor hepatectomy. The aim of this study was to evaluate the outcomes of living donors with a remnant‐to‐total volume ratio (RTVR) less than 30% after right hepatectomy by the selection criteria: preservation of middle hepatic vein (MHV), age<50 years, and no or mild fatty change in healthy adults. All living donors who underwent right hepatectomy saving MHV at the authors' institution between January 2005 and September 2011 were divided into 2 groups: group A with an RTVR < 30% and group B with an RTVR ≥ 30%. Chronic lithium treatment protects against liver ischemia‐reperfusion injury in rats
In this study, we aimed to evaluate the effects of lithium in a model of liver ischemia/reperfusion injury (I/R) in rats. Chronic treatment with lithium (2 mmol/kg, for 3 days before ischemia) decreased I/R injury, whereas acute treatment with a single dose of lithium (2 mmol/kg, 1 h before ischemia) did not confer any protection in partial hepatic I/R model. Furthermore, chronic lithium‐treated rats had a significantly better survival rate (60%) as compared to saline (27%) in total hepatic I/R survival model. Chronic lithium treatment protected against liver I/R injury, as indicated by lower serum aminotransferase level...

Early use of renal‐sparing agents in liver transplantation: A closer look
Abstract
Renal dysfunction is a critical issue for liver transplant candidates and recipients. Acute and chronic nephrotoxicity, however, are the compromise for the potent immunosuppression provided by calcineurin inhibitors (CNIs). To maintain graft and patient survival afforded by CNIs while minimizing renal dysfunction in liver transplant patients, the reduction, delay, or elimination of CNIs in immunosuppression regimens is being implemented more frequently by clinicians. The void left by standard‐dose CNIs is being filled by nonnephrotoxic immunosuppressants such as the mycophenolates and mammalian target of rapamyc...

Donor risk index for african american liver transplant recipients with hepatitis C virus
Abstract
African‐American (AA) liver transplant (LT) recipients with hepatitis C virus (HCV) have higher rates of graft loss than other racial/ethnic groups. The Donor Risk Index (DRI) predicts graft loss but is neither race nor disease‐specific and may not be optimal for assessing donor risk for AA HCV‐positive LT recipients. We developed a DRI for AA with HCV with the goal of enhancing graft loss predictions. All U.S. HCV‐positive adult AA first deceased donor LTs surviving ≥30 days from 3/2002 to 12/2009 were included. A total of 1766 AA LT recipients were followed for median 2.8 (IQR 1.3‐4.9) years. Indepen...

Markers of operational immune tolerance after pediatric liver transplantation in patients under immunosuppression
Abstract
A prospective identification of the estimated 20–50% of pediatric LTX recipients developing operational tolerance would be of great clinical advantage. So far markers of immune tolerance – T‐cell subpopulations or gene expression profiles – have been investigated only retrospectively in successfully weaned patients. Fifty children aged 8–265 months (median 89) were investigated 1–180 months (median 44) after LTX under ongoing immunosuppression. T‐cell subpopulations were measured during regular post‐transplant visits using FACS (Vδ1‐ vs. Vδ2‐γδ‐T cells and Tregs). A Vδ1/Vδ2‐γδ‐T...

Re‐evaluation of the indications for liver transplantation in Wilson's disease based on the outcomes of patients referred to a transplant center
Abstract
The aim of this study was to re‐evaluate the indications and timing of LT for WD. From 2000 to 2009, eight patients with WD who had been referred to our institution for LT were enrolled in this study. The mean patient age was 15.9 yr (range, 7–37 yr). Four patients could not receive LT, because there were no available donors. All four patients were treated with chelating agent medication. Three of them (two of two patients with fulminant WD and one of two with cirrhotic WD) who did not undergo LT are still alive and doing well with stable liver functional tests. Only one of the patients with cirrhotic WD who...

Hematopoietic stem cell transplantation for children with primary immunodeficiency diseases: Single center experience in Jordan
Abstract
HSCT can be curative for many PID. Little is known about the outcome of HSCT for patients with PID in the developing countries. We retrospectively reviewed all children with PID who received HSCT at KHCC in Jordan between August 2003 and October 2011. Twenty‐eight patients were identified. The median age was 16 months (3 months–17 yr). Patients' diagnoses were SCID (n = 16), CHS (n = 3), HLH (n = 3), WAS (n = 2), Griscelli syndrome (n = 1), ALPS (n = 1), Omenn's syndrome (n = 1), and DiGeorge syndrome (n = 1). Seventeen patients received HLA‐matched related HSCT, eight received maternal un...

Clinical and microbial impact of screening kidney allograft preservative solution for bacterial contamination with high sensitivity methods
In conclusion, the use of more sensitive culture methods increases the rate of bacterial contamination of PS and is associated with an increased prescription of antibiotics and increased ESBL‐PE carriage and related infections. Therefore, the systematic use of PS blood bottle cultures in kidney transplantation may have no benefit and might increase the rate of ESBL‐PE emergence.

The effect of rabbit antithymocyte globulin on human mesenchymal stem cells
Summary
Mesenchymal stem cells (MSCs) possess immunomodulatory properties which are of key interest for their application in autoimmunity and transplantation. In transplantation, administration of MSCs has shown promising results in preclinical models and has recently moved to clinical trials. Therefore, it is important to study the interactions between MSCs and immunosuppressive drugs currently used in transplantation. We aimed to analyze the effect of rabbit antithymocyte globulin (rATG) MSCs. MSCs were obtained from perirenal fat of kidney donors and exposed to ranging doses of rATG (Thymoglobulin®, Genzyme; 0.5–100...
