[Comment] How the Health and Social Care Bill 2011 would end entitlement to comprehensive health care in Englandemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
The National Health Service (NHS) in England has been a leading international model of tax-financed, universal health care. Legal analysis shows that the Health and Social Care Bill currently making its way through the UK Parliament would abolish that model and pave the way for the introduction of a US-style health system by eroding entitlement to equality of health-care provision. The Bill severs the duty of the Secretary of State for Health to secure comprehensive health care throughout England and introduces competitive markets and structures consistent with greater inequality of provision, mixed funding, and widespread...

An independent advisory group has urged the UK Government to integrate genomic medicine into the National Health Service (NHS). First, the group proposed that the government outline a policy for expansion of genomic technology in the NHS; HGSG emphasised that commissioning of cost-effectiveness studies will be a necessary step. "Revolution begins with a transformation of consciousness". Innovation, doing things differently, is becoming a high priority activity in the NHS. Conclusion The main finding of this study was that being admitted to hospital at the weekend (Saturday or Sunday) is associated with a significant increased risk of death over the following 30-days. This study has strengths in that it has used an extremely large and reliable data set representative of almost all hospital admissions within the NHS in England during one financial year. The researchers’ model also accounted for a wide range of medical and sociodemographic factors and characteristics of admission that could have influenced the risk of death. While the researchers’ models adjusted for a variety of important...

Local formularies provide a list of selected or preferred drugs available to local prescribers and have an important role in underpinning safe and effective use of medicines. However, there is currently no standard process or advice for putting together a local formulary which has led to variations across the country. A recent report into innovation in healthcare by the Department of Health has highlighted that not all local formularies are including all of NICE's technology appraisals. This can lead to a postcode lottery where patients miss out on drugs approved by NICE. (Source: NHS Networks)

Patients referred by GPs or attending the emergency department with minor emergency surgical conditions are assessed against certain criteria. Those with abscesses requiring drainage or hernias that are painful but not obstructed or strangulated are not admitted but instead sent home and allocated to unplanned gaps in elective surgery lists. Patients are notified of their surgery slot once known and usually attend the following day in a day surgery unit (weekdays) or short-stay ward (weekends), to fast before undergoing surgery. More ….. We were interested to read the paper by Morris et al demonstrating significant variation in 30-day postoperative mortality following major colorectal cancer surgery in National Health Service hospitals in England.1 While we agree that understanding the underlying causes of this variation will be invaluable to inform best practice, we think that it is necessary to choose a definition of postoperative mortality that provides information relevant to patients as well as clinicians. In cardiothoracic surgery, this issue has been debated for some years, resulting in a measure of ‘operative mortality’ which encompasse...
