Teaching NeuroImages: MRI appearances of Lhermitte-Duclos disease
A 48-year-old woman was admitted with a 3-year history of intermittent dizziness and unstable gait. Cranial MRI demonstrated a laminated lesion of T2 hyperintensity and T1 hypointensity involving the left cerebellar hemisphere, which appeared enlarged (figure, A and B). Secondary hydrocephalus and Chiari I malformation were observed. Because the striated appearance on MRI was characteristic of Lhermitte-Duclos disease,1 presurgical diagnosis was made. Total resection of this lesion was performed, and histopathologic evaluation confirmed the diagnosis (figure, C and D). Follow-up neuroradiologic studies revealed no recurren...

Development of Simultaneous Pitch Encoding: Infants Show a High Voice Superiority Effect
Infants must learn to make sense of real-world auditory environments containing simultaneous and overlapping sounds. In adults, event-related potential studies have demonstrated the existence of separate preattentive memory traces for concurrent note sequences and revealed perceptual dominance for encoding of the voice with higher fundamental frequency of 2 simultaneous tones or melodies. Here, we presented 2 simultaneous streams of notes (15 semitones apart) to 7-month-old infants. On 50% of trials, either the higher or the lower note was modified by one semitone, up or down, leaving 50% standard trials. Infants showed mi...

Is insufficient knowledge of epilepsy the reason for low levels of healthcare in the Lao PDR?
Conclusions: The low level of knowledge of epilepsy on the part of health workers may be contributing to the wide treatment gap in the Lao PDR. Improving knowledge of this disease and increasing the availability of antiepileptic drugs will reduce misconceptions about epilepsy, thus encouraging more PWE to seek treatment. Community-based educational programs and extensive advocacy for people with epilepsy only began in 2009.

Factors affecting unmet need for family planning in eastern Sudan
Conclusions: Unmet need for family planning in Eastern Sudan was significantly higher among women with less than secondary education. Also; it is influenced by couple's educational status and woman's occupation. The results of this study necessitate the need for the programme managers to take into account the concept of reproductive health education.

Exploring the perspectives and experiences of health workers at primary health facilities in Kenya following training.
Conclusion: Enhanced health worker skills conferred by the mental health training programme may be responsible for the significant improvement in outcome of patients in the intervention clinics found in the randomised controlled trial, despite the general shortage of medicines and other health system weaknesses. These findings suggest that strengthening mental health training for primary care staff is worthwhile even where health systems are not strong and where the medicine supply cannot be guaranteed.Trial registrationISRCTN 53515024

Influence of personal and environmental factors on mental health in a sample of Austrian survivors of World War II with regard to PTSD: is it resilience?
Conclusions: Our results show no clear picture of factors constituting resilience. Instead, most aspects of resilience rather appeared to be concomitants or consequences of PTSD and non-PTSD. However, special attention should be placed on a challenge-oriented and humorous attitude towards stress in future definitions of resilience.

Validation of an information--motivation--behavioral skills model of self-care among Chinese adults with type 2 diabetes
Conclusions: Consistent with the IMB model, having better provider-patient communication, having social support, and having higher self-efficacy was associated with performing diabetes self-care behaviors; and these behaviors were directly linked to lipid control. The findings indicate that diabetes education programs should including strategies enhancing patients' knowledge, motivation and behavioral skills to effect behavior change.

Cross-protective immune responses elicited by live attenuated influenza vaccines.
Abstract The desired effect of vaccination is to elicit protective immune responses against infection with pathogenic agents. An inactivated influenza vaccine is able to induce the neutralizing antibodies directed primarily against two surface antigens, hemagglutinin and neuraminidase. These two antigens undergo frequent antigenic drift and hence necessitate the annual update of a new vaccine strain. Besides the antigenic drift, the unpredictable emergence of the pandemic influenza strain, as seen in the 2009 pandemic H1N1, underscores the development of a new influenza vaccine that elicits broadly protective immun...

Acute Modulations in Stratum Corneum Permeability Barrier Function Affect Claudin Expression and Epidermal Tight Junction Function via Changes of Epidermal Calcium Gradient.
Abstract Tight junction (TJ) is recognized as a second barrier of the skin. Altered expression of TJ proteins in various skin diseases characterized by the abnormal permeability barrier such as psoriasis suggests that TJ could be affected by stratum corneum (SC) barrier status. However, the physiological relationship between SC and TJ barrier remains to be investigated. Therefore, we examined the effect of SC barrier disruption on the expression of TJ proteins, claudin (Cldn)-1 and Cldn-4, and TJ barrier function in hairless mouse skin. We also investigated whether the alterations in epidermal Ca(2+) affected TJ pr...

A Case of Long QT Syndrome Type 3 Aggravated by Beta-Blockers and Alleviated by Mexiletine: The Role of Epinephrine Provocation Test.
Abstract Long QT syndrome (LQTs) is an uncommon genetic disease causing sudden cardiac death with Torsade de Pointes (TdP). The first line drug treatment has been known to be β-blocker. We encountered a 15-year-old female student with LQTs who had prolonged QTc and multiple episodes of syncope or agonal respiration during sleep. Although her T wave morphology in surface electrocardiography resembled LQTs type 1, her clinical presentation was unusual. During the epinephrine test, TdP was aggravated during β-blocker medication, but alleviated by sodium channel blocker (mexiletine). Therefore, she underwent implanta...

In this report, however, we describe a case in which dorsal-plantar loop technique with only one antegrade approach, using chronic total occlusion devices via anterior tibial artery, was used to successfully recanalize BTK arteries. We think that this new technique, which may represent a safe and feasible endovascular option to avoid more invasive, time-consuming, and riskier surgical procedures, especially in end-stage renal disease and diabetes, should be considered whenever the foot is at risk, and results of above-the-ankle percutaneous transluminal angioplasty remain unsatisfactory or insufficient to achieve limb salv...

Cloning and characterization of two Argonaute genes in wheat (Triticum aestivum L.)
Conclusions: Two putative wheat Argonaute genes, TaAGO1b and TaAGO4, were cloned. Phylogenetic analysis, prediction of conserved domains and catalytic motifs, and modelling of their protein structures suggested that they encode functional Argonaute proteins. Temporal and spatial expression analyses indicated that these genes are potentially involved in developmental regulation of wheat plants.
