uncertain diagnosis/ investigation for marathon runner
hiya,
in my last exam there was a question that came up which I have been unable to find the answer for. It was something along these lines:
35yo marathon runner presents with severe RUQ pain. Urinalysis shows protein + blood +++ glucose -ve. What test would most likely confirm the diagnosis?

any ideas/ answers would be greatly appreciated!

extraordinary match lists for certain schools
Why do some schools perform much better than other schools within the same "tier"? For example:
- Tufts match extensively to Harvard (about 12% of class) as well as other top hospitals like Yale
- Dartmouth match almost 15% to Harvard and a good amount to top hospitals on both coasts
- Case Western matched 9 derm, 2 plastics, 8 ophtho, 15 ortho, 4 ENT, 5 urology, most of them at top hospitals.

Pigment gallstones
I did a question recently about a patient with a history of aortic valve replacement presenting with indigestion with fatty foods. Xray of abdomen shows nodular densities in upper right quadrant. Lab studies show increased indirect bilirubin.
Question was what constitues the nodular densities in the upper right quadrant?
I always thought since it's the liver excreting bile into the gall bladder, it must be conjugated bile.. so I chose 'conjugated bilirubin' making up the pigment stone. Got it wrong.
The answer was unconjugated bilirubin. Can anyone explain this? I feel this is simple, but somehow cant get my head aroun...

How are TGL measured in the blood stream?
It seems that the intestine releases chylomicrons into the blood stream and these are converted into TGL from the fatty acids in the chylomicron in adipose tissue and the glyerol goes to the liver to be converted into TGL and then released as VLDL. NBME2 spoiler - renal - please help
I have no idea what this question is even remotely asking. All the choices seem wrong.
Answer key = D, but that doesn't make sense because it's well-known that ACE-inhibitors are contraindicated in RAS, no questions asked.
That leaves choices A and C.
I could see that choice D is the only one that doesn't require an imaginative stretch in terms of a diuretic's effects on renin, but you clearly wouldn't give an ACEi with RAS.
I went with choice A because Loops are known to have positive prostaglandin effects on the afferent arterioles, which could have possibly explained the decreased renin despite the proximal arterial...

Does anyone know the most common outcome of HepD infection?
HepA+B, most commonly = subclinical disease followed by complete recovery.
HepC, most commonly = chronic, asymptomatic hepatitis
Wikipedia says that HepD has the highest mortality rate (20%) when associated with infection, but what about the other 80%? Is it asymptomatic chronic hepatitis or complete resolution, etc.?

Step 2 timing question
I am finishing up my M3 Medicine clerkship, and will be done 9/28/12. Step 1 211.
I had to postpone it until this summer semester because I had to take some time off last fall due to a complicated pregnancy and subsequent preterm delivery.
My re-worked schedule had me taking Step 2 in December (anytime, because my school is a testing spot just for our students), but now I'm wondering if I should try to take it sooner.

<p>Hello, I found out about a month ago that I have a 9mm kidney stone that is still giving me some pain in my back. I have not seen a urlolgist yet but I have an appointment this monday. I have DD508 and am 26 years old. I really think this has something to do with the cf. Staph A now in urine
Has anyone else also cultured Staph A in their urine? Jasey has cultured it twice in her throat swabs but now she has cultured it in her urine. I don't know if this is normal or should I be concerned?
