The objective of this study was to retrospectively investigate MGMT promoter hypermethylation status for a series of 350 human brain tumors, including 275 gliomas of different malignancy grade,
21 glioblastoma multiforme (GBM) cell lines, and 75 non-glial tumors. The analysis was performed by methylation-specific PCR
and capillary electrophoresis. MGMT expression at the protein level was also evaluated by both immunohistochemistry (IHC)
and western blotting analysis. Associations of MGMT hypermethylation with IDH1/IDH2 mutations, EGFR amplification, TP53 mutations, and 1p/19q co-deletion, and the prognostic significance of these, were investigated for the gliomas. MGMT promoter hypermethylation was identified in 37.8% of gliomas, but was not present in non-glial tumors, with the excepti...

In this study, the expression of E‐cadherin was evaluated in CRC to determine its correlation with clinico‐pathological variables, and association with disease outcome in patients with long‐term follow‐up. The present series consisted of tissue samples obtained from 230 patients with stage I, II, III, or IV CRC treated during 1981–1990 at Turku University Hospital. Archival paraffin‐embedded samples were used to build up tissue microarray blocks, and E‐cadherin expression was assessed by immunohistochemistry using an automated staining system. Different grading systems were tested for expression of E‐cadherin. Fifty‐nine percent of all tumors were positive for E‐Cadherin. There was no significant correlation between E‐cadherin expression and gender (p < 0.83), ...

Discussion:During active infection in nursing infants, the total number of white blood cells, specifically the number of macrophages, and TNFα levels increase in their mothers' breast milk. These results may support the dynamic nature of the immune defense provided by breastfeeding sick infants. Methods:Breast milk from mothers of 31 infants, up to 3 months of age, who were hospitalized with fever, was sampled during active illness and recovery. Milk from mothers of 20 healthy infants served as controls.

Conclusions This system describes discrete clinical subgroups of patients with pancreatic cancer who have similar, potentially resectable
tumor anatomy but heterogeneous physiology and cancer biology. It may be used with neoadjuvant therapy to predict outcomes,
individualize treatment algorithms, and optimize survival.

The value of frozen section evaluation in the management of borderline ovarian tumors

Conclusion: In the frozen section evaluation of BOTs, the underdiagnosis that restricts the diagnostic performance of the method seems to be a major problem. A more careful approach is therefore needed, while choosing a proper surgical technique during laparotomy for ovarian masses. In order to reduce the false diagnosis and surgical morbidity, the frozen section analysis should be applied by experienced pathologists and the possible predictive factors affecting a false diagnosis should carefully be taken into consideration.
Abstract Glioblastoma (GBM) is a morphologically heterogeneous tumor type with a median survival of only 15 months in clinical trial
populations. However, survival varies greatly among patients. As part of a central pathology review, we addressed the question
if patients with GBM displaying distinct morphologic features respond differently to combined chemo-radiotherapy with temozolomide.
Morphologic features were systematically recorded for 360 cases with particular focus on the presence of an oligodendroglioma-like
component and respective correlations with outcome and relevant molecular markers. GBM with an oligodendroglioma-like component
(GBM-O) represented 15% of all confirmed GBM (52/339) and was not associated with a more favorable outcome. GBM-O encompassed

Conclusions.-Assessment of treatment effect in pancreatic cancer is difficult. Pathologists need to be aware that some histologic features of treatment effect overlap with histologic features seen in untreated pancreatic cancer, such as tumor cell anaplasia, necrosis, and fibrosis. Careful assessment of pancreatic resections, including detailed gross examination and thorough histologic sampling, is important in accurately assessing treatment effect and improving patient outcomes.

    Abstract
The aim of this study was to investigate fascin and podoplanin expression in oral dysplasia and carcinoma in situ (CIS) immunohistochemically, and to evaluate their relationship to histopathological diagnosis based on architectural and cytological features. Fascin and podoplanin expression patterns were analyzed immunohistologically in 26 specimens of oral lesions, including benign disease (hyperplasia, papilloma, and others), intraepithelial neoplasia/borderline disease (dysplasia), and malignant disease (CIS, invasive squamous cell carcinoma). Fascin expression was scored into four original categories, and podoplanin expression was scored into five previously established categories. The relationship be...

We report the CT findings of surgically proven PPSBT in which multiple peritoneal cysts were seen. Although rare, PPSBT should be considered in the differential diagnosis of primary peritoneal tumours. Since the prognosis of the disease is good, conservation of the uterus and ovaries should be a consideration in young female patients during surgery.

    Abstract
Worldwide, hepatocellular carcinoma (HCC) is the fifth most common malignant neoplasm. The development of HCC in cirrhotic liver is described either as de novo hepatocarcinogenesis or as a multistep progression, from low-grade dysplastic nodule (DN) to high-grade DN, then to DN with microscopic foci of HCC, then to small HCC, and finally to overt carcinoma. Therefore, early detection and accurate assessment of small HCC is of great importance as the therapeutic efficacy of various treatments and the survival of patients with small HCC are much more successful than those for patients with larger tumors. The hallmark of HCC during computed tomography scan or magnetic resonance (MR) imaging is the presence of arterial enhancement followed by washout. The most ...

Abstract MicroRNAs are small non-coding RNAs, which inhibit expression of specific target genes at the post-transcriptional level and
are often misregulated in human cancer. Among them, miR-34a is considered a tumor suppressor with a hypothetical role in melanoma tumorigenesis. In this work, 62 Italian index patients
with familial melanoma and negative for CDKN2A/CDK4 screening were investigated for miR-34a germline mutations. Eight novel miR-34a sequence variants were identified at both the heterozygous (c.+259G>A, c.+424G>A, c.+1465C>T, c.+1769C>T, c.+2456T>G, c.+2603C>T,
c.+2972T>A, c.+3069T>C) and homozygous (c.+424G>A, c.+1465C>T, c.+1769C>T) states. Molecular screening identified all nucleotide

In conclusion, Brenner ovarian tumors are unilateral, small
and benign neoplasms in their majority and present specific histopathological and immunopathological characteristics and mixed
forms with other epithelial and germ cell neoplasms. This could be explained as a form of metaplasia or a diverse histogenesis
from surface epithelium and/or the germ cell ovarian component.

Conclusion The main concern for laparoscopic management of ovarian masses is unexpected malignancy. However, with careful patient selection,
proper technique and an experienced operator, laparoscopic treatment of ovarian masses is the best approach and should be
implemented, with low risk of unexpected malignancy and recurrence.

AbstractAngiogenesis plays a key role in the development and treatment response of various tumors. The signaling transductions mediated by the binding of VEGF to its receptor KDR is the most important pathway in tumor angiogenesis. Single nucleotide polymorphisms (SNPs) in VEGF have been extensively implicated in the etiology and treatment outcome of colorectal cancer (CRC). However, no study has been reported evaluating the role of KDR SNPs in CRC prognosis. We herein assessed the association between four potentially functional KDR SNPs and tumor recurrence in a Chinese population with 408 surgically resected CRC patients. The most significant SNP was for rs10013228 located in KDR gene promoter. Compared to the homozygous wild‐type genotype, the variant‐containing genotypes of this SN...

Conclusion
This analysis of a relatively new screening programme demonstrates that the programme has been effective in detecting bowel cancers at an early stage. This is extremely important in this disease, as in the absence of an effective screening programme most cases would not be diagnosed until they have progressed considerably. At this point treatment becomes more difficult, and survival is less likely than those cases diagnosed at an early stage.
The researchers say that uptake of the programme outside of London was very good, and high by international standards. The study also revealed unexpected variations in participation across regions and socioeconomic levels. Knowledge of such variation is essential, as it allows programme administrators to alter their recruitment and invitat...

    Abstract
Investigate FasL and survivin expression in a series of primary ovarian surface epithelial tumors, correlate their expression with each other, and characterize the presence of CD3+ T-lymphocytes in studied tumors and determine whether their presence correlates with FasL or survivin expression in malignant cases. FasL and survivin expression was assessed in 54 ovarian epithelial tumors. The results were compared between different tumor types and grades. Correlation between both markers' expression in all studied tumors was done. Either marker's expression was compared to the mean CD3+ T-lymphocytes per HPF in the studied malignant tumors. Either FasL or survivin expression was significantly higher in malignant versus benign ovarian epithelial tumors (p &...

We analyzed histopathological features of core needle biopsy (CNB) and surgical excision specimen comparatively in 129 patients with surgically proven phyllodes tumor (PT). Stromal characteristics including cellularity, atypia, mitosis, overgrowth, tissue fragmentation, and the tumor margin were assessed. Benign/borderline/malignant phyllodes tumor (PT) were diagnosed in 90 (69.8%)/30 (23.3%)/9 (7.0%) patients. Among the 90 cases of benign PTs, 67 cases (74.4%) were diagnosed concordantly in CNB. For borderline and malignant PTs, three out of eight (26.6%) and four out of nine (44.4%) cases were diagnosed concordantly in CNBs. All 50 cases of discordant diagnosis were underestimated in matched CNBs, especially in their stromal cellularity and mitosis. The size of tumor is larger in discord...

Conclusion: With the development of epithelial ovarian tumors from benign to malignant, IEX-1 expression is decreased, concomitant with a decreased rate of cell apoptosis. Thus, IEX-1 is pro-apoptotic in the development of epithelial ovarian cancer. The pro-apoptotic activity may take part in restraining tumor growth at the early stage of ovarian epithelial cancer, whereas its decreased expression probably contributes to the abnormal survival advantage for malignant cancer. Altered IEX-1 expression can potentially be a new predictor of the malignant transformation and a prognostic indicator for cancer therapy.

AbstractIt has been well established that ovarian low‐grade and high‐grade serous carcinomas are fundamentally different types of tumours. While the molecular genetic features of ovarian high‐grade serous carcinomas are now well known, the pathogenesis of low‐grade serous carcinomas, apart from the recognition of frequent somatic mutations involving KRAS and BRAF, is largely unknown. In order to comprehensively analyze somatic mutations in low‐grade serous carcinomas, we applied exome sequencing to the DNA of eight samples of affinity‐purified low‐grade serous carcinomas. A remarkably small number of mutations were identified in seven of these tumours: a total of 70 somatic mutations in 64 genes. The eighth case displayed mixed serous and endometrioid features and a mutator p...

Conclusions The majority of pediatric ovarian tumors were benign disease, and the patients with malignant lesions had a good prognosis.
In neonatal cases, an umbilical crease incision approach is feasible and provides excellent cosmesis. We recommend tumor resection
with ovarian preservation through a minimally invasive approach (modified Rocky Davis incision) as the first line treatment
for older pediatric patients with ovarian tumors other than those preoperatively diagnosed as malignant.

The experience with laparoscopic DP for malignant disease remains limited, and long-term follow-up data are required to clearly
define this role.

Area: News
NHS Choices has assessed a report in the Daily Telegraph that in vitro fertilisation (IVF) doubles the risk of non-fatal ovarian cancer.

The report is based on a large, long-term Dutch cohort study that examined the risk of ovarian cancer associated with IVF treatment. Its findings were published in 'Human Reproduction.' The study compared the risk of developing ovarian cancer in the 15 years following treatment with the risks of cancer in the general population and in women who had difficulty conceiving but did not undergo IVF treatment. The researchers found that the IVF group was over four-times as likely to have developed borderline ovarian tumours (not yet cancerous) compared to the non-IVF group. However, they were at no greater risk of deve...

Conclusion
This was a large, long-term cohort study that examined the association between ovarian stimulation during fertility treatment and the subsequent risk of developing ovarian cancer. The study was well designed, especially in terms of its selection of an appropriate comparator group and its attempts to account for potential confounders.
In their attempt to control potential confounders the researchers collected information on baseline ovarian cancer risk through a mailed questionnaire, and on treatment factors through medical record examination.
However, data on factors such as family history of cancer, number of pregnancies, use of birth control and lifestyle characteristics were available in only 65.2% of the total cohort. This may bias the results, as there is no way of telling...

A Dutch study suggests that stimulating the ovaries to produce eggs for in vitro fertilization or IVF, may increase the risk of developing ovarian tumors, some of which may become cancerous, later in life. Overall, the researchers found rates of non-fatal "borderline ovarian tumors" were increased four-fold and rates of malignant tumors were increased two-fold compared to other women seeking fertility treatment who did not undergo IVF...
AbstractCXCL12 is a chemotactic cytokine that has pro‐metastatic functions in several malignancies through interactions with its receptor, CXCR4. CXCL12 is an estrogen‐regulated gene, and notably, estrogen is a major risk factor for endometrial cancer (EC) development. As few studies examine concurrent CXCL12, CXCR4, and estrogen receptor (ER) expression in EC patients, we examined this pathway in 199 EC patients with data from the University of Pittsburgh Medical Center Cancer Registry. Immunohistochemistry (IHC) was used to detect CXCR4, CXCL12, and ER protein expression. As CXCR4 expression was positive in all cases, this investigation focused on associations between CXCL12 and ER expression, clinicopathologic factors, and survival outcomes using chi‐square tests, Kaplan‐Meier g...

We report a case of a 12-year-old premenarcheal girl who developed ovarian mucinous adenocarcinoma. The tumor, which originated
from the right ovary, weighed 1400 g, and had a smooth surface and thin capsule, was extirpated. The cut surface showed multilocular
cysts of various sizes and yellowish gelatinous to solid parts that accounted for approximately 40% of the cut surface. The
histological findings showed a continuous mixture of mucinous adenoma, mucinous borderline tumor, and mucinous adenocarcinoma
with an expansile invasive pattern. It was assumed that an ovarian mass developed in an early period after birth or infancy,
and the mutation of certain oncogens occurred in an early period, leading to cancer. While some etiological hypotheses concerning
ovarian carcinogenesis ...

Conclusions: Because the radicality of the first surgical resection is of essential importance for long-time local control of chest desmoids, accurate preoperative diagnostics and well-planned aggressive surgical resection of the primary tumor is recommended. Due to the low incidence of desmoids, multicentric randomized investigations would be mandatory to establish evidence-based protocol for desmoid tumors.
Conclusion: Overall, cancer detection with digital or film-screen mammography is similar in U.S. women aged 50 to 79 years undergoing screening mammography. Women aged 40 to 49 years are more likely to have extremely dense breasts and estrogen receptor-negative tumors; if they are offered mammography screening, they may choose to undergo digital mammography to optimize cancer detection.
CONCLUSIONS: Adrenocortical tumors are rare in children. Adrenocortical tumors include adenomas and carcinomas, and in the absence of vascular infiltration and/or metastases it is difficult to differentiate between the two types by imaging and histology. The combination of an adrenal mass and clinical signs of adrenocortical hyperfunction in a child is virtually diagnostic of an adrenocortical tumor.

Abstract: Phyllodes tumors of the breast are rare tumors, accounting for less than 0.5% of all breast tumors. These tumors are comprised of both stromal and epithelial elements; and traditionally they are graded by the use of a set of histologic features into benign, borderline, and malignant subtypes. Unfortunately, the histologic classification of phyllodes tumors does not reliably predict clinical behavior. The mainstay of treatment of non metastatic phyllodes tumors of the breast is complete surgical resection with wide resection margins. Lumpectomy or partial mastectomy is the preferred surgical therapy. However, despite the complete surgical resection, local failure rate may be high; and 22% of malignant tumors may give rise to haematogenous metastases.
CONCLUSIONS: EGFR inhibitors, including pertuzumab, may add activity to conventional chemotherapy for treatment of platinum-resistant ovarian cancer. Certain subsets of women with particularly aggressive tumours resistant to conventional chemotherapy may benefit from EGFR inhibitor treatment. Further RCTs are necessary before EGFR inhibitors are introduced as first- or second-line treatment of ovarian cancer.

CONCLUSIONS: Chromosome 12 trisomy has been previously identified in pure fibromas, supporting the concept that a significant proportion of benign serous tumors are primary fibromas with an associated cystic mass. This is the first high resolution genomic analysis of benign serous ovarian tumors and demonstrates not only that the majority of benign serous tumors have no genetic evidence of epithelial neoplasia but that a significant proportion may be more accurately classified as primary fibromas.

Conclusions: 1) There is an overlap in using the terminologies as STUMP, AL, AL with low risk of recurrence, AL with low malignant potential. A designation of STUMP does convey a category of borderline malignancy to the gynecological surgeons. Most behave in a benign fashion and follow-up without adjuvant therapy is currently recommended. Critical evaluation of coagulative tumor necrosis is essential. Follow-up remains a challenge in our setting.

Conclusions: A relatively high frequency of epithelial ovarian neoplasms among all ovarian tumors in a purely adolescent population was found in our study. Age-related selection bias may account at least in part for the discrepancy between our data and most previous reports. The most common subtype of epithelial ovarian tumor in our series was the benign serous cystadenoma. Abstract
Prostatic stromal tumours are rare neoplasias that include benign, malignant and borderline lesions. Stromal tumour of uncertain malignant potential (STUMP) has been recently described and only a few reports exist in the literature. As a rare and distinct neoplasia, to date, there is no description of MRI findings of prostate STUMP. In this article, we describe the clinical and MRI features with histopathological correlation of a patient with prostate STUMP.

Conclusion
This research has illustrated that fluorescent tagging of ovarian cancer cells during diagnostic keyhole surgery is not only possible, but it can also help surgeons identify small areas of cancerous tissue that they cannot see by regular visual inspection alone. This potentially allows surgeons to better identify cancerous tissue when assessing the stage of the cancer by laparoscopy, a technique which is often used alongside other diagnostic imaging procedures such as CT and MRI scanning. It may also help surgeons ensure that they remove all cancerous tissue during therapeutic surgery, which will usually be quite a major operation. In particular, the authors consider that it may guide surgeons when performing debulking surgery, and so improve the likely efficiency of the chemoth...

Abstract: Background: Intraductal papillary neoplasms of the bile duct have been applied to certain types of papillary tumours occurring in the biliary tract. Although many cases have been sporadically reported, there remain controversies.Aims: To analyze the clinicopathologic characteristics and long-term survival of intraductal papillary neoplasms of the bile duct. Methods: The clinicopathologic data of 52 patients who underwent surgery for intraductal papillary neoplasms of the bile duct were retrospectively evaluated. Results: In our series, tumours located in intrahepatic and hilar bile duct, rather than in extrahepatic bile duct, were more commonly diagnosed as adenomas or borderline tumours (12/19 and 7/13 vs 6/20; P=0.046). And the gastric type was more commonly associated with adeno...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Doctors believe use of fluorescent dye could boost survival rates by guiding them to tiny clusters of malignant cellsA group of women with ovarian tumours have become the first in the world to have surgery using a procedure that makes cancer light up in the body.Operations were performed on 10 women as part of the first phase of a clinical trial to evaluate the technique, called fluorescence-guided surgery.Doctors developed the procedure to help surgeons remove malignant tissue from ovarian cancer patients and reduce the risk of the disease returning months and years later. The method illuminates cancer cells inside a patient's body to make it easier for surgeons to spot smaller cancers that can be easily missed, and to help them identify the borders between tumours and the healthy tissues ...

CONCLUSIONS: Because micropapillary SBOT was significantly associated with peritoneal implants, especially invasive implants, and lymph node involvement, complete staging procedures, including lymph node dissection, are recommended. However, micropapillary SBOT itself was not significantly associated with DFS. Peritoneal implant was the only factor independently associated with tumor recurrence.

    Abstract
Pregabalin is gaining popularity in the perioperative period for its usefulness in treating neuropathic pain and its apparent opioid- sparing effect. The present report describes the perioperative course of three patients who received pregabalin and experienced significant respiratory depression in the postoperative period. All three patients consented to the report and publication of the present case series. The first patient was elderly with borderline renal dysfunction. She experienced respiratory arrest in the immediate postoperative period following a craniotomy for tumour excision. The second patient presented with severe respiratory depression 12 h after receiving a spinal anesthetic for joint replacement, and was later found to have clini...

    Abstract
Epithelioid hemangioendothelioma (EHE) is a rare tumor of the vascular origin. It was first described in its pulmonary form by Dail and Leibow in 1975 and named "intravascular bronchioalveolar tumor" (IVBAT). Since then, reports of occurrences of the tumor have been made for number of locations, but most often tumor can be found in soft tissues, liver, lungs, bone and skin. It is considered to be a low or borderline malignant tumor with, usually, slow progression, but aggressive forms have been described. We here report a case of a 46-year old female patient with multifocal malignant tumor spreading to lungs, liver, spleen and with synchronous involvement of lumbal vertebrae, illiac bone...

Conclusions Patients with MP-SBOT represent a heterogeneous population in terms of presence of invasive peritoneal implants. Conservative
surgery could be a suitable option for MP-SBOT patients without implants and who wish to conserve childbearing potential,
without increasing the risk of recurrence.

Conclusion: Neoadjuvant treatment seems to have some activity in patients with borderline/unresectable pancreatic adenocarcinoma. Nearly one third of tumors initially deemed marginal for operative intervention were able to be ultimately resected after treatment. Until more effective targeted chemotherapeutics are developed, the only group of patients with pancreatic cancer that may benefit from neoadjuvant treatment are those with locally advanced disease.
Conclusion: Early diagnosis and appropriate treatment are crucial for patients with ovarian cancer diagnosed during pregnancy. Tumor staging is possible during pregnancy, but the appropriateness of surgery needs to be considered carefully. Ideally, the treatment strategy should be discussed and structured on an individual basis.
CONCLUSIONS: During primary surgery for advanced stage epithelial ovarian cancer all attempts should be made to achieve complete cytoreduction. When this is not achievable, the surgical goal should be optimal (< 1 cm) residual disease. Due to the high risk of bias in the current evidence, randomised controlled trials should be performed to determine whether it is the surgical intervention or patient-related and disease-related factors that are associated with the improved survival in these groups of women. The findings of this review that women with residual disease < 1 cm still do better than women with residual disease > 1 cm should prompt the surgical community to retain this category and consider re-defining it as 'near optimal' cytoreduction, reserving the term 'suboptimal' c...

Abstract Both narrow band imaging (NBI) and autofluorescence imaging (AFI) are new techniques for the assessment of lung cancer. The
major aim of this study was to investigate whether the combination of these two techniques improve sensitivity and specificity
in the assessment of lung cancer extension. The study prospectively evaluated 118 patients with suspected lung cancer. All
of the patients were examined using EVIS LUCERA SPECTRUM videobronchoscopy system. The narrow band imaging preceded autofluorescence
imaging examination. In every patient, at least 1 but no more than 4 biopsies were taken from places visualized as pathologic,
surrounding primary tumor, and at least 1 biopsy from places that appeared visually normal. Sensitivity, specificity, positive,
and negative...

MedWorm Sponsor Message: Find the best January Sales in the UK.

Conclusions:
Postoperative mortality and morbidity do not significantly increase after neoadjuvant treatment. Neoadjuvant treatment in locally advanced pancreatic cancer can lead to an objective pathologic response, but this does not significantly improve survival after resection.
CONCLUSION: Most adnexal masses can be classified according to the ultrasound simple rules of the score of Timmerman with a good specificity to eliminate their malignant or borderline characteristics. Tumors which cannot be classified according to these rules must be referred to an expert ultrasonographist.

This regimen appears effective and has acceptable toxicity. The primary end point (1-year overall survival rate > 45%) was met, with encouraging survival duration. Smad4(Dpc4) immunostaining correlated with the pattern of disease progression. Prospective validation of Smad4(Dpc4) expression in cytology specimens as a predictive biomarker is warranted and may lead to personalized treatment strategies for patients with localized pancreatic cancer.
AbstractA 52‐year‐old woman had undergone a partial cystectomy for a cystic mass at the dome of the urinary bladder in 1997. The pathological diagnosis was a urachal mucinous cystadenoma with borderline malignancy. Twelve years later, multiple masses were noted on the uterus, ovaries and abdominal wall. She underwent debulking surgery, including a total hysterectomy, bilateral salpingo‐oophorectomy, pelvic lymph node dissection, infracolic omentectomy, appendectomy, colon mass removal and abdominal wall mass removal. Remnants of the tumor were seeded throughout the abdominal cavity. The pathological evaluation confirmed metastatic urachal mucinous adenocarcinoma in both ovaries, the abdominal wall, the colon mass, the uterosacral ligament and the bladder.
Conclusion. The IMPT technique produced the most conform plans. For tumors located in the one of the cerebral hemispheres, the non-coplanar RA and the IMPT techniques were able to reduce doses to the contralateral hippocampus. The IMPT technique offered the largest sparing of the brain and fiber tracts. RA techniques tended to produce more conform target doses than IMRT.

Epiregulin (Ep) was found to be produced in non-cancer ovarian cells in response to gonadotropin stimulation as well in ovarian cancer cells in an autonomous manner. However, there were no systematic follow-up studies of Ep expression in the development of different stages of ovarian cancer. Using specific antibodies to Ep and the indirect immunocytochemistry methods, we found that in normal ovary the staining for Ep was mainly confined to the epithelial cells, while the stromal cells were only occasionally and moderately stained. In contrast in benign serous and mucinous tumors most of the tumor cells showed a clear staining in the cytoplasm. In borderline serous and mucinous tumors the staining was mu...

Acute right lower quadrant pain is a common emergency department presentation. Medical imaging has a main role to rule out appendicitis. Distinguishing between appendicitis and the other two major appendix pathologies, mucocele and carcinoid tumors, is not easy, but it is important for medical and surgical management planning. Appendix mobility is not usually assessed during sonography, but is it helpful? When comparing sonography and computerized tomography with histopathology findings to distinguish appendiceal pathologies, appendix mobility was found to be a key component. Appendix diameter, wall thickness, hyperemia, and surrounding echogenic fat are signs of an inflammatory process that will fix the appendix. Appendiceal carcinoids and mucoceles, on the other hand, will not initially ...

Conclusions This is the first study to compare 3T with 1.5T MRI for T-staging of rectal cancer within the same patients. Our results showed
no difference between 3T and 1.5T-MRI for the distinction between T1-2 and borderline T3 tumors, regardless of expertise.
The higher resolution at 3T-MRI did not aid in the distinction between desmoplasia in T1-2-tumors and tumor stranding in T3-tumors.
Larger studies are needed to acknowledge these findings.

The aim of the study was to assess the relationship between the expression of COX-2 and p53, hormone receptors and HER-2 in the in situ (DCIS) and invasive components of ductal carcinomas (IDC) of the same breast. The expression of COX-2, p53, and hormone receptors was assessed in 87 cases of IDC with contiguous areas of DCIS. Results showed that there was no difference in COX-2 expression comparing the in situ and invasive components of the tumors. In the in situ component, there was a statistically borderline increase in p53 expression in tumors that also expressed COX-2. ER-positive specimens were more common in the group of tumors that expressed COX-2 in the inva...

AbstractDiagnosis of Borderline papillary serous tumor of the fallopian tube was comprehensively established by Zheng in 1996 supported mostly by a histological similarity to its ovarian counterpart. It is a very rare entity with eight cases published so far and the ninth case described here as a 41‐year‐old woman presented with non‐specific lower abdominal pain, dyspareunia and dysuria. Left adnexal mass was identified and she was operated on. It turned out the tumor was attached exclusively to the left tube, with no connection to any of the surrounding structures and with histology of borderline serous tumor with non‐invasive implants in the left and right ovary and visceral peritoneum. Reviewing available data on genetics of these tumors there was diploid status in one examined ...

Conclusion(s) Mature oocytes were successfully retrieved ex-vivo from the hyperstimulated ovary recovered via laparoscopy. The procedure can be performed in a quick manner, with standard
equipment, without damaging the ovary, the follicles or the oocytes, and without the risk of cancer cell spillage associated
with the standard transvaginal oocyte retrieval if there is concern of ovarian surface/peritoneal metastatic disease.

Abstract Alpha-methylacyl-CoA racemase (AMACR) is involved in the cellular metabolism of fatty acids. It is a prognostic factor in
prostate and colorectal cancer. So far, little is known about its expression and prognostic role in ovarian cancer. We investigated
the expression of AMACR in a total of 420 ovarian tumors (388 carcinomas, 32 borderline tumors) by immunohistochemistry on
tissue microarrays of two independent patient cohorts. In both cohorts, cytoplasmic AMACR expression was identified in 11.8%
(16/136) and 5.4% (13/239), respectively, of the ovarian carcinomas. In contrast, borderline tumors did not show any AMACR
expression. AMACR expression was significantly associated with histological subtype, FIGO stage, and grade in one cohort and
low estrogen receptor le...

Photodynamic therapy is a method for achieving selective tissue destruction of neoplastic and atypical proliferative cells using a chemical photosensitizer, which accumulates within the tumor, and a light source to activate a free radical cascade. This method has been used for a variety of malignant and premalignant conditions. We have been using photodynamic therapy in the larynx for intractable and difficult keratosis with atypia using a nebulized formulation of aminolevulinic acid activated with the 585-nm pulse dye laser (Cynosure, Chelmsford, MA). We can perform this procedure in the office under local anesthesia using a flexible nasopharyngoscope with a 2.5-mm working channel. We have found it to be a valuable adjunct for patients with widespread keratosis and dysplasia. In this arti...

CONCLUSION: Based on findings indicated in a primary investigation set and confirmed in the large secondary validation set, we report for the first time plasma GDF-15 as a biomarker for endometrial cancer phenotype, including presence of lymph node metastasis and reduced survival. Its applicability as predictor of metastatic nodes and in monitoring treatment of endometrial cancer needs to be further studied.

Abstract Breast phyllodes tumors are rare neoplasms which present challenges for histological classification. Microscopic features
are not always predictive of clinical behavior, and scarce data exist on the prognostic role of biological markers. Our study
evaluated a series of 145 phyllodes tumors diagnosed at the Department of Pathology, Singapore General Hospital between 2006
and 2009, incorporating 91 (62.8%) benign, 40 (27.6%) borderline, and 14 (9.7%) malignant phyllodes tumors. Antibodies to
keratin 15 (KRT15), transcobalamin I (TCN1), and homeobox gene Hox-B13 (HOXB13) were applied to sections cut from tissue microarray
blocks. KRT15 and TCN1 positivity was defined when there was reactivity of 1% or more stromal cells, while HOXB13 positivity

The World Health Organization classification of lymphoid neoplasms updated in 2008 represents a worldwide consensus on the diagnosis of these tumors and is based on the recognition of distinct diseases, using a multidisciplinary approach. The updated classification refined the definitions of well-recognized diseases, identified new entities and variants, and incorporated emerging concepts in the understanding of lymphoid neoplasms. However, some questions were unresolved, such as the extent to which specific genetic or molecular alterations define certain tumors, and the status of provisional entities, categories for which the World Health Organization working groups felt there was insufficient evidence to recognize as distinct diseases at this time. In addition, since its publication, new...

Ovarian epithelial carcinoma (OEC), the most common ovarian malignancy, is a heterogeneous disease with several histologic subtypes that show characteristic cytogenetic features, molecular signatures, oncologic signaling pathways, and clinical-biologic behavior. Recent advances in histopathology and cytogenetics have provided insights into pathophysiologic features and natural history of OECs. Several studies have shown that high- or low-grade serous, endometrioid, and clear cell carcinomas are characterized by mutations involving the TP53, K-ras/BRAF, CTNNB1, and PIK3CA genes, respectively. High-grade serous carcinomas, the most common subtype, often manifest with early transcoelomic spread of disease beyond the ovaries, whereas low-grade serous and mucinous carcinomas commonly manifest w...

SummaryAnimal‐type melanoma (ATM) represents a rare subtype within the wide spectrum of melanocytic tumors. Clinically, ATM lesions appear as sharply demarcated, brown, black and dark blue pigmented nodules, which show grey‐white surface elements on dermatoscopy. The tumor is restricted to the dermis and arranged in irregular fascicles, which are composed of spindle‐shaped and epithelioid melanocytes. Moderate tumor cell pleomorphism, mitoses and apoptotic cells all suggest a malignant process. Abundant, finely dispersed melanin pigment within tumor cells as well as numerous melanophages are strongly suggestive of ATM. Even though locoregional lymph node metastases are frequently found at diagnosis, the course of ATM is generally benign. Specific molecular changes may be detected in ...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

The diagnosis and early treatment of ovarian cancer is in the news today, with the publication of new NICE guidance for doctors on recognising ovarian cancer. NICE calls for more initial investigations (such as a blood test) to take place in GP surgeries - this is so that more women are referred to hospital specialists and begin treatment sooner, greatly increasing their chances of survival.
The guidance focusses on areas where there is current uncertainty or wide variation in clinical practice among doctors with regard to the detection of ovarian cancer, particularly in primary care where most women will present. In particular, the guidance gives clear advice on when to carry out a specific blood test when cancer is suspected.

Conclusions Advanced primary tumor stage and age were associated with inferior OS after PD for AC. Adjuvant therapy did not impact survival.
Patients with advanced tumor stage should be considered for clinical trials of adjuvant therapy after PD with novel compounds
and optimized radiation therapy strategies.

Abstract Overexpression of topoisomerase II protein (topo 2α) is postulated to be more closely associated with responsiveness to anthracycline-containing
chemotherapy than human epidermal growth factor receptor type 2 (HER2) gene amplification or alterations in the topoisomerase

randomized to CEF or CMF adjuvant chemotherapy in the NCIC Clinical Trials Group Mammary 5 (MA.5) trial. No significant interaction
was found between treatment and continuous topo 2α level in either relapse-free (RFS) or overall survival (OS). In 136 patients

Spleen-preservation has recently been emphasized in benign and borderline malignant pancreatic diseases requiring distal pancreatectomy. Reports to suggest that laparoscopic distal pancreatectomy is feasible and safe have been increasingly published. Robotic surgical system has been introduced and is expected to provide unique advantages in laparoscopic surgery. However, robot-assisted pancreatic surgery has not yet been performed by many surgeons. A 45-year-old female patient with abdominal discomfort was found to have pancreatic cyst in the body of the pancreas. Mucinous cystic tumor of the pancreas was the most favourable preoperative diagnosis. She underwent spleen-preserving laparoscopic distal pancreatectomy by using da Vinci surgical robo...

Opinion statement Since the introduction of laparoscopy and robotic surgery in gynecologic practice in the last several decades, use of these
minimally invasive surgical techniques has increased dramatically. The role of minimally invasive surgical techniques continues
to expand because they offer reduced intraoperative and postoperative complications, less intraoperative blood loss, and a
shorter postoperative recovery. Despite initial concerns about the use of minimally invasive surgery in gynecologic oncology,
this approach has been shown to be safe and effective in the management of uterine and cervical cancer, and minimally invasive
surgical management of these malignancies is now commonplace. Concerns remain regarding the use of minimally invasive surgery

Summary: Sonic hedgehog signaling is important for human development, and aberrant regulation of this pathway can result in the development of tumors. The aim of this study was to examine the expression of sonic hedgehog signaling molecules in oral squamous cell carcinoma. By quantitative real-time polymerase chain reaction, the expression of SHH, SMO, PTCH-1, and GLI-1 was analyzed in 30 oral squamous cell carcinoma cases and 8 samples of nonneoplastic oral mucosa and associated to clinical pathologic features. The expression of β-catenin, cyclin D1, Wnt-1, and Egfr was evaluated by immunohistochemistry in 26 available cases of oral squamous cell carcinoma. Normal oral mucosa from healthy individuals was negative for all genes that were evaluated. SHH, PTCH-1, SMO, and GLI-1 were not exp...

CONCLUSIONS: Frozen section analysis of BOTs has low accuracy, sensitivity, and positive predictive value, and underdiagnosis and overdiagnosis are frequent. Therefore, surgical decision-making for BOTs based on frozen section diagnosis should be done carefully, especially in tumors with mucinous histology.

Conclusion: Systematic histologic analysis of glandular mammary tissue sampled after reduction mammoplasty in the particular context of breast reconstruction after breast cancer makes it possible to discover lesions that were not seen in presurgical evaluation. The early management of these borderline occult lesions could reduce the incidence of breast cancer in these at-risk patients.
