Chlamydia trachomatis infection is the most common sexually transmitted bacterial infection with an estimated incidence of 50 million cases in the world.1 The infection may remain asymptomatic or, if not treated, can lead to severe clinical manifestations including acute urethral syndrome, urethritis, bartholinitis, cervicitis, upper genital tract infections and perihepatitis.2 Despite the recognised need for early diagnosis and treatment of C trachomatis and other curable sexually transmitted infections, the strategies for detection of these infections are currently limited, particularly in low-resource countries.3 Migration and population mixing have been shown to increase the risk of sexually transmitted diseases;4 however, the prevalence and determinants of C trachomatis infection, and...

MedWorm Sponsor Message: Find the best January Sales in the UK.

We describe the history and background of these political, religious, and philosophical factors. Next, we present a translation of the original texts, describing the establishment of the submandibular and sublingual salivary glands. We place these translations into historical context and comment on their naming propriety. Initially we translate the works of the 15th century anatomists. Next, we look at the discovery and description of the submandibular gland's duct by Thomas Wharton (1614–1673) in his landmark book, Adenographia sive glandularum totius corporis descriptio (Adenographia or the description of the glands of the entire body) (1656). Next, the somewhat unexpected discovery of the duct of the parotid gland by Nicholas Stenson (1638–1686) and his publication, De glandulis ori...

Conclusion: An absent hymen makes renal agenesis more likely and increases the likelihood that dilator techniques will fail. This condition appears to be associated with reports of long-term problems with poor lubrication that are potentially related to the absence of the peri-hymenal Bartholin’s glands.
The Bartholin's gland (BG) may develop several tumoral lesions such as BG duct cysts, carcinomas, hyperplasias, and adenomas. BG nodular hyperplasias (NHs) are uncommon, and few cases have been reported in the literature. Specific criteria for the distinction between adenomas and hyperplasias have been recently established. Several questions on possible etiologies and appropriate medical approach still remain. Therefore, this work comprises three case reports of NH and an extensive literature reviewA glândula de Bartholin (GB) pode ser sede de várias lesões tumorais, como cistos do ducto da GB, carcinomas, hiperplasias e adenomas.
Abstract: Symptoms and signs of patients with focal vulvodynia or vulvo-vestibulitis syndrome (VVS) are variable in location and severity. It is not known whether the location of the most severe pain in the vestibulum is linked to the complaints and perhaps a different entity. A clinical gut feeling suggests that two distinct varieties of focal vulvodynia may be either focused at 2 points (5 and 7 o′ clock) or at 4 points (5, 7, 1 and 11 o′ clock).A questionnaire was filled out by 30 women with focal vulvodynia during 147 visits and checked for completeness by an independent study nurse. Another investigator to evaluated the clinical signs of VVS, blinded to the patients history or complaints. The visual analogue score (VAS) of pain experienced upon attempt of sexual contact was used a...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

We report the first case of pedunculated AMFB of the vulva occurring in a young woman of 21 years old and involving the left labia minora. After excluding the most common diseases, pedunculated AMFB should be part of differential diagnosis in the workup of any pedunculated vulvar mass even in young women with a lesion involving the labia minora. We reviewed the literature and summarized all reported cases. (Source: Clinical and Developmental Immunology)

Conclusions The results suggest that a substantial number of family caregivers of diseased palliative care patients are at risk of developing
CG and depression following their loss. While early identification of those at risk of developing CG could be helpful, the
risk assessment of professionals may lack in precision. The results indicate that bereavement services could be utilized in
a more targeted and perhaps more efficient manner. Guidelines for bereavement planning in palliative care are indicated.

Abstract: Carcinoma of the vulva is an uncommon malignant neoplasm (approximately one-fifth as frequent as cervical cancer) and represents 4% of all genital cancers in women. Approximately two-thirds of cases occur in women older than 60 years, and squamous cell carcinoma is the most common histologic type. Several different subtypes of squamous cell carcinoma have been described in the vulva; however, in terms of etiology, pathogenesis, and histologic features, most carcinomas belong to one of two categories: keratinizing squamous cell carcinomas associated with chronic inflammatory skin disorders, and basaloid or warty carcinomas related to infection with high oncogenic risk human papillomaviruses. Glandular neoplasms of the vulva arise from the vulvar apocrine sweat glands (papillary hi...

Squamous cell carcinoma of the vaginal Bartholin’s glands is a rare vulvar neoplasm affecting predominantly postmenopausal women. This case report presents a woman in her early 50s with a firm mass in the posterior vagina. Surgical biopsy revealed the mass to be a poorly differentiated squamous cell carcinoma with probable direct invasion of the rectum. (Source: Journal of Diagnostic Medical Sonography)

The de novo designed heterodimeric E/K coiled-coil system has been previously demonstrated to be an excellent capture/dimerization system applicable to various needs in both biotechnology and pharmaceutical fields. Those include controlled protein dimerization, capture, purification and Western blot detection. We here report the development of a new generation of ELISA test based on coiled-coil interactions for the direct quantitation of coil-tagged epidermal growth factor (EGF). The new approach was evaluated for its specificity, plate storability and reusability as well as for convenience when compared to commercially available systems. Our results show a similar affinity/sen...

Conclusion In conclusion, these small molecules, built on a 1,3-dihydroindole-2-one scaffold, elicit strong anti-proliferative and cytotoxic
activity, and importantly, a strong anti-tumorigenicity is observed in in vivo xenograft models of human breast, ovary, prostate
and pancreatic cancers encouraging the translation of this class of compounds into the clinic.

Abstract In humans, exogenous retroviruses are known to cause immunodeficiency and neurological disease. While endogenous retroviruses
are firmly established pathogens in other species, the human endogenous retroviruses (HERVs) may well be considered as emerging
pathogens. HERVs also exhibit complex interactions with exogenous retroviruses and herpesviruses. Two neurological disorders
in particular are associated with HERVs: multiple sclerosis (MS) and schizophrenia. HERV-H/F and HERV-W are specifically activated
both in the circulation and the central nervous system (CNS) in a majority of MS patients, and particularly, the envelopes
(env transcription and Env proteins) appear strongly associated with disease activity. Interferon beta (IFN-β) therapy is well-established

Conclusions: In cases clinically suspicious for urethral diverticulum magnetic resonance imaging had a 24.4% error rate. Serious consequences are failure to detect cancer and suboptimal treatment for urethral diverticulum. The reason for the high magnetic resonance imaging accuracy rate in other series may be that in the absence of radiological confirmation some surgeons may choose not to perform surgery. Magnetic resonance imaging is useful to assess urethral diverticula but physicians should be aware of its limitations. (Source: The Journal of Urology)

A Bartholin's cyst, also called a Bartholin's duct cyst, is a small growth just inside the opening of a woman's vagina. The Bartholin's glands secrete fluid that helps lubricate the vagina. They are located on each side of the vaginal opening. Sometimes the openings of these glands become obstructed, causing fluid to back up into the gland. This causes the swelling called a Bartholin cyst...
Authors: Jakobsen J, Mikkelsen J, Nielsen A
For cellular and animal transgenesis, FLP- and Cre-recombinase gene capture systems are highly effective to provide stable integration of a donor plasmid carrying the transgene cassette of interest into an engineered genomic locus in a given cell line. However, in many protocols, the entire plasmid bacterial backbone is integrated along with the transgene cassette. Here, we present a very simple yet highly efficient method for excluding plasmid bacterial backbone integration. The transgene cassette, including a single FLP recognition target site, is specifically amplified by PCR, and the resulting DNA ligated into minicircles can serve as donor DNA in FLP-mediated recombination. Interestingly, the elimination of the bacterial backbone increas...

Abstract Alternative allocations of a fixed bundle of healthcare resources often involve significantly different indirect, non-health
effects. The question arises whether these effects must figure in accounts of the conditions under which a distribution of
healthcare resources is morally justifiable. In this article we defend a Scanlonian, affirmative answer to this question:
healthcare resource managers should sometimes select an allocation which has worse direct, health-related effects but better
indirect, nonhealth effects; they should do this when the interests served by such a policy are more urgent than the healthcare
interests better served by an alternative allocation. We note that there is a prima facie case for the claim that such benefits
(and costs) are relevan...

A woman's Bartholin glands are located at the entrance to her vagina, one on each side. The glands are small, and may not be seen or felt when they are healthy. The function of the glands is to secrete fluid onto the mucosal, or inner, surface of the labia surrounding the vagina. (Source: Disabled World)

Streptococcus thermophilus is perceived as a recently emerged food bacterium that evolved from a commensal ancestor by loss and gain of functions. Here, we provide data allowing a better understanding of this evolutionary scheme. A MLST approach that we developed showed that S. thermophilus diverges significantly from its potential ancestors of the salivarius group and displays a low allelic variability, confirming its likely recent emergence. An analysis of the origin and dissemination of prtS gene was carried out within this evolutionary scheme. This gene encodes protease that allows a better growth in milk, by facilitating the casein breakdown for amino acid supply. S. thermophilus protease exhibits 95% identity to ...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Conclusions: Vulvar lipomas in children tend to occur on the right side, especially the right anterolateral side. It is necessary to differentiate this tumor from an inguinal hernia. Radiologic studies such as ultrasonography, computed tomography, and magnetic resonance imaging are useful for making the proper diagnosis. Surgical excision is the treatment of choice. Typically, we use a softer catheter for the ring, but we've also used butterfly tubing in a pinch. (Source: The Journal of Emergency Medicine)

CONCLUSION. Restricted diffusion is generally considered to be
associated with malignant tumors because of the high cellularity of these
tumors. However, in interpretation of diffusion-weighted images, it should be
kept in mind that a number of benign lesions, as many as 22% in our cohort,
can exhibit restricted diffusion on images with high b values, thus mimicking
malignant lesions. (Source: American Journal of Roentgenology)

We describe the case of a 52-year-old woman who was admitted to our hospital for the local excision of a 4cm vulvar mass, originally thought to be a Bartholin's gland cyst. It was subsequently found to consist of small round cells positive for anti-CD99 antibody, thus suggesting a diagnosis of ES/pPNET. The demonstration of EWSR1 gene translocations by means of fluorescent in situ hybridization excluded small-cell carcinoma, squamous cell carcinoma of the small type, Merkel cell carcinoma, and lymphoblastic lymphoma. After surgery, the patient received six cycles of polychemotherapy and radiotherapy; she is still alive and well after 1 year of follow-up. Our findings underline the crucial role of molecular biology techniques in the differential diagnosis of small round cell tumors in these...

MedWorm Sponsor Message: Find the best January Sales in the UK.

Abstract Whether or not breast cancer can be a feature of the hereditary nonpolyposis colorectal cancer (HNPCC) syndrome has been debated.
In order to clarify if defective mismatch repair (MMR) may indeed play a role in breast cancer, we used the Danish HNPCC register
to identify all breast cancers that occurred in MMR gene mutation carriers. In total, 20 female mutation carriers were diagnosed
with breast cancer at mean 50 years of age. These tumors were predominantly ductal carcinomas with extensive lymphocytic reactions
in 8/14 evaluated tumors. MMR protein immunostaining showed loss of expression of MLH1, MSH2 or MSH6 corresponding to the
mutations identified in 7 of the 16 cases investigated, and these tumors were diagnosed at mean 50 (33–66) years of age. ...

Perineal infections are a common presenting complaint in women attending acutely to a gynaecology department. Specifically, Bartholin's abscesses can occur in approximately 2% of women. Conservative versus surgical approaches to manage these infections aims to reduce the need for hospital admission. We summarise the literature on the diagnosis and management of Bartholin's and other types of perineal infections and abscesses specific to gynaecology. (Source: Best Practice & Research.
Abstract Atopic dermatitis (AD) is a common, itchy skin disease of complex inheritance characterized by dermal and epidermal inflammation.
The heritability is considerable and well documented. To date, four genome scans have examined the AD phenotype, showing replicated
linkage at 3p26-22, 3q13-21 and 18q11-21. Our previous AD scan showed evidence of linkage to loci at 3p and 18q, and furthermore
at 4p15-14. In order to further investigate the genetic basis of AD, we collected and analysed a new Danish family sample
consisting of 130 AD sib pair families (555 individuals including 295 children with AD). AD was diagnosed after clinical examination,
AD severity was scored and specific IgE was determined. A linkage scan of chromosome 3, 4 and 18 was performed using 91 microsat...

Invariant natural killer T (iNKT) cells constitute a distinct subset of T lymphocytes exhibiting important immune-regulatory functions. Although various steps of their differentiation have been well characterized, the factors controlling their development remain poorly documented. Here, we show that TGF-β controls the differentiation program of iNKT cells. We demonstrate that TGF-β signaling carefully and specifically orchestrates several steps of iNKT cell development. In vivo, this multifaceted role of TGF-β involves the concerted action of different pathways of TGF-β signaling. Whereas the Tif-1 branch controls lineage expansion, the Smad4 branch maintains the maturation stage that is initially repressed by a Tif-1/Smad4-independent branch. Thus, these three differen...

Most of the Bartholin's gland abscesses have been thought to be caused by colonizing micro-organisms of the perineal region. We encountered an interesting case of acute Bartholins abscess caused by Streptococcus pneumoniae in a primigravida. The abscess was incised and drained. The patient was treated with Cefuroxime. This case is presented for its rarity.

Discussion There is no consensus on the treatment of adenoid cystic carcinoma of the Bartholin’s gland. Wide local excision and/or vulvectomy
and radiotherapy are advocated for treatment of this cancer. There is not much data on treatment of metastasis and it varies
according to site.

We describe the case of a 63-year-old woman with a large Bartholin's duct cyst that showed high signal intensity on T(1)- and T(2)-weighted magnetic resonance (MR) images. Although Bartholin's duct cyst is the most frequent cystic lesion of the vulva, such a giant cyst is rare.

Conclusion Any vulvar lesion with unusual characteristics or insidious evolution in labia majora or Bartholin’s glands area should be
carefully and promptly studied. This is particularly important in order to perform an effective surgical treatment in cases
of leiomyosarcoma.

This study highlights the importance of diagnosing non-venereal dermatoses and refutes the general misconception that all vulval itching is the result of fungal infection. The two most common causes of vulval itching observed in the study were lichen sclerosus and lichen simplex chronicus.

Conclusions. Our present study provides novel insights into the function of the nuclear form of the FLRG protein, which is revealed as a novel regulator of transcription. The nuclear isoform of FLRG lacks an intrinsic transactivation domain, but enhances AF10-mediated transcription, probably through promoting the homo-oligomerization of AF10, thus facilitating the recruitment of co-activators.

Abstract Endometriosis is a common gynecologic disease characterized by growth of endometrial glands and stroma outside the endometrium.
Implants are most commonly found in the pelvis but can occur in many other sites. Vulvar involvement of endometriosis is extremely
rare. A case of vulvar endometriosis is presented. Endometriosis was infiltrating the subcutaneous scar tissue of left labia
minora after cystectomy of Bartholin gland for abscess that has recurred several times despite office-based treatment.

Activin, a member of the transforming growth factor ß (TGFß) superfamily, regulates diverse processes, such as cellular growth and differentiation. There is increasing evidence that TGFß and its signaling effectors are key determinants of tumor cell behavior. Loss of sensitivity to TGFß-induced growth arrest is an important step toward malignancy. We previously characterized FLRG as an extracellular antagonist of activin. Here, we show that activin-induced growth inhibition is altered in FLRG-expressing breast cancer lines. Silencing FLRG induced growth inhibition, which is reversible upon addition of exogenous FLRG. We showed that FLRG silencing effects resulted from restoration of endogenous activin functions as shown by increased levels of phosphorylated smad2 an...

Follistatin related gene (FLRG) has been previously identified from a chromosomal translocation observed in a B-cell chronic lymphocytic leukemia (B-CLL). FLRG (alternative names: follistatin-related protein, FSRP/follistatin-like-3, FSTL3) is a secreted glycoprotein highly similar to follistatin. Like follistatin, FLRG is involved in the regulation of various biological effects through its binding to members of the transforming growth factor beta (TGFbeta) superfamily such as activin A and myostatin. We have previously shown that TGFbeta and activin A are potent inducers of FLRG transcriptional activation through the Smad proteins. Using a biochemical approach, we investigated whether tumor necrosis factor alpha (TNFalpha) c...

Conclusions: Our study provides novel insights into the function of the nuclear form of FLRG protein which is revealed as a novel regulator of transcription. The nuclear isoform of FLRG lacks an intrinsic transactivation domain, but enhances AF10-mediated transcription probably through promoting the homo-oligomerization of AF10, thus facilitating the recruitment of coactivators.

Mycoplasma hominis was isolated from a Bartholin's gland abscess 70 years ago, and ureaplasmas were isolated from the genital tract about 20 years later. Subsequently, reports incriminating mycoplasmas in the known adverse outcomes of pregnancy have been legion. Without doubt these genital mycoplasmas are able to invoke an inflammatory response and take part in the cascade of events that culminates in preterm birth. Their role in this and other conditions is becoming clearer, but controversy remains due mainly to investigators often ignoring bacterial vaginosis, with its complex of bacteria, and failing to disentangle their role from that of the genital mycoplasmas. This is a theme that will be highlighted in this chapter, in which an attempt is made to indic...

MedWorm Sponsor Message: Find the best January Sales in the UK.

Modern immunology has been notably free of public disputes over credit for major discoveries in this discipline. But the early recognition of the lymphatic system witnessed two examples of heated priority feuds. The first in the 17th-century concerned the greater anatomical organization of the system, while the second in the 18th-century concerned its function. This essay reviews the earlier of the two disputes, in which a Swedish medical student (Ole Rudbeck) charged a respected Danish Professor (T. Bartholin) with plagiarism and antedating his observations. Thus ethical issues in immunology predate modern times. How this discipline reached this point in its history is another focus of this essay and also an excuse to review briefly the anatomy of the lymphatic sys...

During the last decades there has been a continuing evolution in the surgical approach of squamous cell carcinoma of the vulva that has been traditionally treated with radical vulvectomy and bilateral inguinal-femoral lymphadenectomy. Patients with T1 tumour are usually treated with radical local excision, if the lesion is unifocal and the remainder of the vulva is normal. Patients with T1a disease have no risk of groin metastases and do not need lymphadenectomy, whereas those with T1b disease need ipsilateral inguinal-femoral lymphadenectomy if the lesion is lateral, and bilateral lymphadenectomy if the lesion is midline. Modifications of the surgical technique of deep femoral lymphadenectomy and the mapping of senti...

Between 1585 and 1738, four members of the celebrated Bartholin family made significant contributions to anatomical science and medicine. Caspar Bartholin (the elder), two of his sons (Thomas and Rasmus), and his grandson (Caspar the younger) all served on the medical faculty of the University of Copenhagen, and helped to gain international acclaim for the institution. Over three generations, the Bartholins challenged traditional ideas about science and the human body, and discovered anatomical structures and phenomena that would prove crucial to the practice of modern medicine.
We describe a case in a 51-year-old woman whose preoperative diagnosis was confounded with Bartholin's glandular cyst. The neoplasia was well delimited and made up of three characteristic components: fusiform cells forming small fascicles, numerous blood vessels and adipose tissue interspersed between the fusiform cells. The stroma cells were positive for vimentin and negative for CD34, protein S-100, actin and desmin. The differential diagnoses for this tumor include aggressive angiomyxoma, angiomyofibroblastoma, lipoma of fusiform cells, solitary fibrous tumor, perineurioma and leiomyoma.
