Our analysis of the data revealed that the efficacy and relapse rates of low-dose isotretinoin in mild to moderate grades of acne is comparable with the standard regimen (1 mg/kg/day), which is given in the severe grade of acne vulgaris. Although the histological findings were was consistent with a diagnosis of hidradenitis suppurativa (HS), bridged comedones, the hallmark of this disease, were absent and there were no dermal sinuses. Incision and drainage of the pustules provided only temporary improvement, which was briefly maintained with oral clindamycin and topical steroids. After the birth, a course of isotretinoin produced almost total clearance, a response not typically found in HS. Presence of documented PVCs -on electrocardiogram and Holter monitoring- and the disappearance of these PVCs after cessation of the treatment strongly suggest isotretinoin-related PVCs in our case. The impact of isotretinoin on ventricular rhythm can not be ruled out in our case as we have revealed the presence of the temporal association of isotretinoin and documented PVCs. Thus, clinicians should be aware of possible arrhythmogenic effect(s) of isotretinoin.

 additional efforts are required to improve the effectiveness of contraception while on isotretinoin treatment considering
 psycho-social aspects such as improved self-confidence, unexpected new partnership and sexual activity and incorrect perception
