[Case Report] A purple rash

In July, 2010, a 60-year-old white woman with uncontrolled type 2 diabetes mellitus presented with pain and swelling of the hands, wrists, and feet, and a purpuric rash on both heels. She had a 10-year history of diabetes, hypertension, and hyperlipidaemia. Before admission, her baseline creatinine concentration was 126 μmol/L (normal <88 μmol/L for women), GFR was 40 mL/min, 2+ urine protein (1·0 g/L), and haemoglobin A11c (HbA1c) was 68 mmol/mol. Her medications included: atorvastatin, metoprolol, benazepril, furosemide, glargine, and glipizide. (Source: LANCET)

In a prespecified secondary analysis of the ACCOMPLISH (Avoiding Cardiovascular Events Through Combination Therapy in Patients Living With Systolic Hypertension) trial, Bakris et al conclude that benazepril/amlodipine therapy reduces chronic kidney disease (CKD) progression more effectively than benazepril/hydrochlorothiazide therapy in hypertensive patients at high cardiovascular risk. Effects of Add-on Fluvastatin Therapy in Patients with Chronic Proteinuric Nephropathy on Dual Renin-Angiotensin System Blockade: The ESPLANADE Trial.

CONCLUSIONS: In chronic kidney disease patients with residual proteinuria despite combined angiotensin-converting enzyme inhibitor and angiotensin receptor blockade therapy, add-on fluvastatin does not affect urinary proteins, but further reduces serum lipids and is safe. Whether combined angiotensin-converting enzyme inhibitor, angiotensin receptor blockade, and statin therapy may improve cardiovascular outcomes in this high-risk population is worth investigating. We examined the interdependency of hemodynamics and levels of Cu/Zn-SOD and oxidized low-density lipoprotein (oxLDL) in HF patients, using a randomized, double-blinded, crossover design to compare (i) the outcomes of single-agent therapy with either benazepril or valsartan alone vs. the combination thereof and (ii) the outcome of single-agent treatment with benazepril vs. single-agent treatment with valsartan. After each treatment, arterial (ART) and coronary sinus (CS) blood samples were collected. Cu/Zn-SOD and oxLDL levels were higher in CS samples than in ART samples.
