Conclusion. Patients experienced substantial pain relief in months 1 and 2 following a single Botox injection into the ASM under CT guidance. Significant pain reduction was noted for 3 months after Botox injection with respect to both sensory and VAS scores, and the total and PPI scores approximated statistical significance. After 3 months, patients experienced a 29% decrease in the sensory component of their pain as well as an approximate 15% reduction in their VAS score. A single, CT-guided Botox injection into the ASM may offer an effective, minimally invasive treatment for NTOS. (Source: Pain Medicine)

She had undergone several peripheral neurectomies as well as a failed right posterior fossa exploration that resulted in a cerebrospinal fluid leak. Persistent pain over the right external nasal area and right mental region was relieved for several hours after daily injections of bupivacaine. A trial of a single dose of 100 units of botulinum toxin type A (BOTOX) diluted in 2.5 mL saline was injected into the external nasal trigger zone (60 units) and to the mental nerve region (40 units). She achieved complete pain relief in the external nasal region for 5 months. Pain recurred and the site was again injected with 100 units of botulinum t...

Abstract&nbsp;&nbsp;Chronic migraine (CM) is the leading cause of chronic daily headache, a common and debilitating headache syndrome. The management
of CM patients is challenging, with only limited benefit from available oral preventive medications. Botulinum neurotoxin
(BoNT) has been used extensively to treat disorders associated with increased muscle tone. More recent scientific data support
an analgesic effect of the toxin. The pharmacokinetic and pharmacodynamic profiles of BoNT make it an appealing candidate
for migraine prevention. Results from older clinical trials on the efficacy of the toxin in CM were inconclusive. However,
recent trials using more stringent inclusion criteria have shown positive results, supporting the use of the toxin in some
patients with this disorder...

Ultrasound-Guided Botox Injections of Salivary Glands in Children with Drooling

This study aimed to test the hypothesis that ultrasound-guided botox injection of the salivary glands in children is effective temporary palliation for excessive drooling. The study population consisted of 6 patients who underwent botox injection of salivary glands one or more times at a mid-Atlantic regional children's hospital. Parents were interviewed by telephone regarding the effects of the botox procedure on their child's drooling. The children who comprised our study population exhibited decreased drooling, less respiratory distress, and increased quality of life after the injections. Further research with a larger sample is warranted to support these findings.

CONCLUSIONS: While both treatments led to reduction in ankle clonus, phenol showed greater clinical efficacy. The difference in the neurophysiological results suggests that the two drugs have different action mechanisms with a more prevalent reduction of alpha motoneuron excitability in phenol-treated patients.

We report the first use of Xeomin((R)) in an autonomic indication. A total of 46 patients (34 females, 12 males, age 32.7 +/- 13.2 years, disease duration 14.2 +/- 12.0 years) with symmetric bilateral idiopathic axillar hyperhidrosis and a previously stable Botox((R)) treatment received 50 MU of BT in 5 ml of 0.9% NaCl/H(2)0 in each axilla. The patient, the injector and the observer were unaware of which axilla received Xeomin((R)) and which Botox((R)). The therapeutic effect as measured from the BT application to the onset of its decrease lasted 3.2 +/- 1.4 months and was excellent in 89% and good in 11% of the patients. Side-to-side differences of the therapeutic effect (onset latency, extent, duration) were neither detectable by the patient nor by the physician. Injection site pain was ...

Your facial expression may tell the world what you are thinking or feeling. But it also affects your ability to understand written language related to emotions, according to research that was presented to the Society for Personal and Social Psychology in Las Vegas, and will be published in the journal Psychological Science. The new study reported on 40 people who were treated with botulinum toxin, or Botox. Tiny applications of this powerful nerve poison were used to deactivate muscles in the forehead that cause frowning... (Source: Health News from Medical News Today)

Conclusion: Botox appears to provide improvement in digital perfusion and pain reduction in patients with Raynaud's syndrome when conservative management fails. Summary: Digital ulceration associated with Raynaud's syndrome is painful, difficult to treat, and frequently results in patient debilitation and chronic depression. Reports by Sycha and Van Beek have indicated potential benefit for patients with ischemic digits with local injection of botulinum toxin (Euro J Clin Invest 2004;34:312-3; Plast Reconstr Surg 2007;119:217-26). This was a retrospective observational study on outcomes of 19 patients with Raynaud's syndrome treated with botulinum toxin. All patients had chronic ischemic hand pain, and vascular studies had ruled out proximal occlusive disease and underlying disorders, inc...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

Botulinum toxin A is a highly efficacious and cost-effective, nonsurgical option for reducing the width and shape of the lower face and jawline. The results can vary from the subtlest thinning of the face to an extremely thin, cachectic appearance. Many nuances can be achieved. The administration is simple, and the process takes barely 5 minutes in an office setting. Botulinum toxin A can also be effectively used to reduce the bulk of an enlarged parotid gland without affecting saliva production. We investigated antinociceptive activity of botulinum toxin type A (BTX-A) in a model of diabetic neuropathic pain in rats. Male Wistar rats were made diabetic by a single intraperitoneal injection of streptozotocin (80mg/kg). Sensitivity to mechanical and thermal stimuli was measured with the paw-pressure and hot-plate test, respectively. The formalin test was used to measure sensitivity to chemical stimuli. Diabetic animals with pain thresholds lower for at least 25% compared to the non-diabetic group were considered neuropathic and were injected with BTX-A either subcutaneously (3, 5 and 7U/kg) or intrathecally (1U/kg). Mechanical and thermal sensitivity was measured at several time-points. After periph...

Doctors are worried that as the New Year begins, many Americans will resolve to be thinner in 2010 by using over-the-counter supplements. According to a survey conducted by ADSAM and SenseUS polling companies, about 60 percent of physicians feel troubled about the safety of taking diet pills. Not surprisingly, while physicians feel negatively about the safety of over-the-counter diet pills, they are much more comfortable about the use of injections like Botox and Restylane or even breast implants...
We present evidence of prolonged denervation following BoNT injection of the vastus lateralis (VL) muscle to correct quadriceps muscle imbalance in patients with chronic anterior knee pain. Needle electromyography data were obtained from 10 subjects who had received a single BoNT treatment 5 to 19 months earlier as part of a clinical trial. Insertional and spontaneous activity, recruitment, and motor unit action potentials were examined. Clear differences between the injected and non-injected VL muscles, which correlated with the time since injection, were identified in all subjects. All 10 subjects studied with needle EMG showed evidence of persisting denervation in the BoNT-A injected VL muscle beyond the period of neuromotor recovery expected from animal experimental studies. © 2010 Mo...

Objective: To evaluate published evidence of efficacy and safety of pharmacologic treatments for childhood spasticity due to cerebral palsy.
Methods: A multidisciplinary panel systematically reviewed relevant literature from 1966 to July 2008.
Results: For localized/segmental spasticity, botulinum toxin type A is established as an effective treatment to reduce spasticity in the upper and lower extremities. There is conflicting evidence regarding functional improvement. Botulinum toxin type A was found to be generally safe in children with cerebral palsy; however, the Food and Drug Administration is presently investigating isolated cases of generalized weakness resulting in poor outcomes. No studies that met criteria are available on the use of phenol, alcohol, or botulinum toxin type B inj...

To the Editor: The use of botulinum neurotoxin A is the most frequent intervention in aesthetic medicine. Sales of the two main preparations—Botox (Allergan Inc, Irvine, CA) and Dysport (Ipsen Ltd, Slough, Berkshire, UK)—amounted to USD $1 billion in 2007, and the trend is still growing.
The Lubs X-linked mental retardation syndrome (MRXSL) is caused by small interstitial duplications at distal Xq28 including the MECP2 gene. Here we report on four novel male patients with MRXSL and different Xq28 duplications delineated by microarray-based chromosome analysis. All mothers were healthy carriers of the duplications. Consistent with an earlier report [Bauters et al. (2008); Genome Res 18: 847-858], the distal breakpoints of all four Xq28 duplications were located in regions containing low-copy repeats (LCRs; J, K, and L groups), which may facilitate chromosome breakage and reunion events. The proximal breakpoint regions did not contain known LCRs. Interestingly, we identified apparent recurrent breakage sites in the proximal and distal breakpoint regions. Two of the four pati...

Conclusions. Combining BtxA and mCIMT is an effective and safe intervention for improving spasticity and motor function in chronic stroke patients. The results are promising enough to justify further studies. We recommend future research to address the likely need for including rehabilitation with BtxA to improve function in patients with poststroke spasticity.
CONCLUSIONS:: When counselling prenatal patients regarding sonographically diagnosed isolated clubfoot, it is important to convey that approximately 10% of individuals will have a normal foot or positional foot deformity requiring minimal treatment. Conversely, 10-13% of prenatally diagnosed cases of isolated clubfoot will have complex clubfoot postnatally, based on the finding of additional structural or neurodevelopmental abnormalities. Although this study did not identify an increased risk of fetal aneuploidy associated with isolated clubfoot, a review of the literature indicates a risk of 1.7-3.6% with predominance of sex chromosome aneuploidy.
CONCLUSIONS: Injecting 300 units of BTX-A into 30 sites in the muscle located in the body of the bladder region is effective for neurogenic bladder patients with intermittent catheterization who have urge and reflective types of incontinence. Injec-tions of 100 and 200 units of BTX-A to treat non-neurogenic overactive bladder with urinary incontinence provided together without retention. The optimal dose of BTX-A requires further investigation. Injection with 200 units of BTX-A was not useful against interstitial cystitis. None of the patients developed any adverse effects after injecting the bladder wall with BTX-A.

What matters, however, is a person's organizational rank. Generally, a handshake should be offered by the person of higher authority to the person of lower status. Examples?
This is the first well-controlled study of the use of botulinum toxin type A (BoNTA) for glabellar lines in China. To evaluate the safety and efficacy of BoNTA in the treatment of glabellar lines in Chinese subjects. A total of 227 subjects received a single treatment in a 3:1 randomization ratio of BoNTA (20 U):placebo and were observed for 120 days after injection. Effective outcome measures included investigator's rating of wrinkle severity at maximum frown and rest and subjects' global assessment and self-perception of age. A significantly higher responder rate at maximum frown, ranging from 94.1% at day 30 to 52.9% at day 120, was noted in the BoNTA group. The proportion of subjects with none or mild glabellar lines at rest was 66.7% in the BoNTA group at day 30. Most (95.3%) of the s...

This study sought to determine whether patients receiving BTX-A have a preference for 30-gauge or 32-gauge needles based on the amount of pain and bruising experienced. Thirty-seven subjects received BTX-A on the right side of the face using a 30-gauge needle and on the left side using a 32-gauge needle. Subjects were masked to needle size. They were then asked to rate injection pain on an 11-point numerical rating scale and to note any bruising. Physician preference was also evaluated. There were no statistically significant differences in the amount of intra-procedural pain (p=.37) or the level of post-procedural pain and discomfort (p=.76) experienced. Twenty-seven percent of subjects reported greater bruising with the 32-gauge needle, versus 29.7% with the 30-gauge needle. The physicia...

Can't get enough Policy & Practice? Check out our new podcast each Monday. egmnblog.wordpress.com Allergan just reported its third-quarter earnings, and although Botox (onabotulinumtoxinA) sales increased a scant 3%, the company said that the injectable contributed to a net profit rise of 8.3%. The company predicted that its total Botox sales for the year would be almost $1.3 billion. Also in the third quarter, Allergan filed for Food and Drug Administration approval to promote the use of Botox for migraine headaches and submitted additional data to the agency on the drug's use for upper limb spasticity. Allergan also stated that it expects sales of the eyelash lengthener Latisse (bimatoprost) to hit $70 million for the year. (Source: Skin and Allergy News)

Conclusions: In patients with BPS, the electrically induced muscle activation neither increases the effectiveness of BonT-A nor produces larger electrophysiological peripheral effects. The lack of BonT-A-induced changes in the blink reflex recovery cycle provides evidence that BonT-A therapy is safe in patients with BPS. (Source: European Journal of Neurology)

Treatment success for overactive bladder with urinary urge incontinence refractory to oral antimuscarinics: a review of published evidence

Conclusions:
A treatment success definition that encompasses the many components of underlying OAB with UUI symptoms is currently not practical as a consequence of difficulties in measuring urgency. The treatment success outcome of >=50% improvement in incontinence episodes was associated with a clinically meaningful improvement in disease-specific HRQoL for those with neurogenic OAB with UUI. The recommended success definition is less restrictive than a measure such as complete continence but includes patients who are satisfied with treatment and experience meaningful improvement in symptoms. A standardized measure of treatment success will be useful in clinical and health economic applications. (Source: BioMed Central)

The license for Xeomin® (botulinum toxin type A) has been extended in the EU to include the management of post-stroke spasticity of the upper limb presenting with flexed wrist and clenched fist in adults. Xeomin® is already licensed for the symptomatic management of blepharospasm and cervical dystonia of a predominantly rotational form (spasmodic torticollis) in adults. The current license extension is based on the Kanovsky study, the largest randomised, placebo-controlled, double-blind trial with a botulinum toxin in upper limb post-stroke spasticity to date.
Several newspapers have reported the dangers associated with cosmetic surgery, including a lack of regulation in some areas of the industry. The reports also feature warnings from Nigel Mercer, the president of the British Association of Aesthetic Plastic Surgeons, who voiced his views as part of a series of medical articles on cosmetic surgery. Mr Mercer called for tighter regulation and testing of drugs, procedures and implants to offer more protection to patients.
The Times reported that the number of cosmetic surgical operations conducted by “audited members of the profession” has more than tripled to 34,000 since 2003, but that many additional procedures are being carried out illegally on the black market. The newspaper says that these are “fuelled by internet promotions, magazi...

Allergan, the manufacturer of Botox (onabotulinumtoxinA), has sued the Food and Drug Administration, claiming that the agency's restrictions on off-label promotions are inhibiting the company's ability to “proactively share truthful and relevant information with the medical community,” according to a company statement. FDA's constraint that Allergan discuss off-label uses for Botox only in general terms violates the first amendment and hurts patients, Allergan claimed. To fully inform physicians, the company said it needs to describe dosing guidelines, patient selection criteria, and proper injection technique. “We believe that the inability to share such important information proactively with the medical community violates the First Amendment and potentially diminishes the quality o...

The clinical hallmark of asymmetric crying facies (ACF) is a symmetric appearance of the oral aperture and lips at rest, but significant depression of one side of the lower lip with animation (crying or smiling). ACF can resolve spontaneously in the first year of life, but surgical intervention may be required at some point to ensure a good cosmetic outcome. The authors report on the successful use of botulinum toxin type A to achieve temporary facial symmetry in two children with ACF with results lasting up to six months and suggest that such treatments may be helpful in providing more time to consider and/or plan surgical intervention.
Authors: Panicker J, Haslam C
Lower urinary tract dysfunction in multiple sclerosis is common and is highly amenable to treatment. Individuals may have bladder storage and/or voiding dysfunction. The risk of progression to renal failure is low and hence lower urinary tract dysfunction should be considered medically manageable in most individuals. Evaluation begins with history taking and is supplemented by using a bladder diary. Ultrasonography is used to assess the degree of incomplete bladder emptying, and for assessing upper urinary tract damage. Incomplete bladder emptying is most often managed by clean intermittent self-catheterization and should be initiated if post-void residual urine is greater than 100 mls. Storage symptoms are most often managed using antimuscarinic medicatio...
