For example, the FDA recently approved Botox for preventing chronic migraines.

Conclusions:This work stresses the importance of considering synkinesis and hyperkinesis separately because they are different conditions. These two symptoms showed improvement after botulinum toxin treatment, but only hyperkinesis showed a positive correlation when objective and subjective evaluations were performed. This treatment is effective in the management of facial synkinesis and hyperkinesis due to facial palsy, thus improving quality of life. It is a safe, minimally invasive treatment that can be repeated. (Source: The Laryngoscope)

They're disappearing. From where? Restaurant menus? No, from people's faces.Don't. I have a phobia about birds being near me. Her eerily smooth visage cropped up both on The Graham Norton Show and the Golden Globes at the weekend.I'm sure that's down to a good moisturiser and her famously placid temperament.
AbstractMany papers report the clinical success of botulinum toxin A as a method of management of various bladder dysfunctions. The rationale was that botulinum toxin A was able to block the presynaptic release of acetylcholine from the parasympathetic efferent nerve. The efficacy might result not only from an inhibitory effect on detrusor muscle, but also some effects might be mediated by altering the afferent nerve input. This systematic literature review discusses the efficacy and safety of botulinum toxin A therapy for idiopathic detrusor overactivity, neurogenic detrusor overactivity, interstitial cystitis/painful bladder syndrome and benign prostatic hyperplasia. The information was gathered from a PubMed literature research for abstracts from recent urological meetings. Inject...

According to an investigation published in the January issue of Anesthesia & Analgesia, official journal of the International Anesthesia Research Society (IARS), researchers have discovered that injecting Botox (botulinum neurotoxin type A) affects muscles other than those it's injected into. The study, led by Dr Christiane G. Frick of Massachusetts General Hospital, Boston, demonstrates that rats injected with Botox display lasting effects on muscles after injection, even in muscles far from where Botox was injected...
Botulinum neurotoxin type A better known as Botox has previously unsuspected 'systemic' effects on muscles other than the ones it's injected into, reports a study in the January issue of Anesthesia & Analgesia, official journal of the International Anesthesia Research Society (IARS). Experiments in rats show that Botox has lasting effects on muscles after injection even muscles distant from the injection site. In addition, Botox seems to have "unique" effects on muscle responses to a widely used muscle relaxant, which could affect patient monitoring during surgery or mechanical ventilation...
Authors: Dolly JO, O'Connell MA
    Abstract
There is a pressing unmet need for long-acting and effective therapeutics to alleviate symptoms of the varied forms of chronic pain. As many sufferers do not respond satisfactorily to non-addictive anti-nociceptives, a new treatment has emerged using inhibitors for the release of pain mediators from peripheral sensory nerves to give prolonged benefit. This strategy relies on proteolytically inactivating intra-neuronal SNARE (soluble N-ethylmaleimide-sensitive-factor attachment protein receptors) proteins which are essential for regulated exocytosis of transmitters, peptides and other pain signalling molecules. Success has been achieved with botulinum neurotoxin A (BoNT/A) which targets neuronal acceptors via its heavy chain, becomes endocyto...

CONCLUSIONS: Despite that a full success was obtained in 46% of the cases, BTX-A injection therapy failed to treat refractory NDO in 23% of patients suffering from MS. Duration of the disease was a predictive factor for an inefficient treatment. The injection therapy should be considered as soon as oral anticholinergic drugs fail to reduce NDO.
Randomized controlled study of the efficacy of the injection of botulinum toxin type A versus corticosteroids in chronic plantar fasciitis: results at one and six months.

Conclusion: Botulinum toxin type A should be considered for the treatment of chronic plantar fasciitis in view of the improvement found at one month, and particularly at six months, when this treatment clearly has better results than corticosteroid injections. Further studies with larger samples are necessary to confirm these results.

Planta MedDOI: 10.1055/s-0031-1280385AbstractMost cosmetic and therapeutic applications of Clostridium botulinum neurotoxin (BoNT) are related to muscle paralysis caused by the blocking of neurotransmitter release at the neuromuscular junction. BoNT specifically cleaves SNARE proteins at the nerve terminal and impairs neuroexocytosis. Recently, we have shown that several polyphenols inhibit neurotransmitter release from neuronal PC12 cells by interfering with SNARE complex formation. Based on our previous result, we report here that myricetin, delphinidin, and cyanidin indeed paralyze muscle by inhibiting acetylcholine release at the neuromuscular junction. While the effect of myricetin on muscle paralysis was modest compared to BoNT/A, myricetin exhibited a shorter response time than BoNT...

The NHS North East Treatment Advisory Group (NETAG) has issued a recommendation and associated appraisal report on the use of botulinum toxin type A (Botox®) for the treatment of chronic migraine. The Group did not recommend its use as it was concerned about the clinical efficacy of treatment and important uncertainties remained regarding interpretation of cost effectiveness.
CONCLUSION: NGF appears to play a decisive role in OAB. Its use as a biomarker in both the diagnostics and follow-up after therapy seems promising. To what extent sNGF can be useful as a biomarker or in therapy monitoring needs to be examined prospectively in a larger population.

The occurrence of hyperesthesia, flushing and sweating on cheek and pre-auricular area are signs and symptoms of Frey's syndrome. This syndrome was firstly described in 1757 but only in 1923 Lucie Frey, a Frenchwoman medical, described this occurrence after neural damage. The incidence of this syndrome is rare and can be associated to direct or indirect damage of greater auricular nerve and/or auriculotemporal nerve; the spontaneous occurrence is extremely rare and can be associated with normality variation. Other rare case is the bilateral trifid mandibular condyle, usually this modification of the normal anatomy can be associated with fracture, but the association with embryological changes is rare. The aim of this case report is to show a rare case of spontaneous Frey's syndrome associa...

The effect of surgical and chemical denervation on ischaemia/reperfusion injury of skeletal muscle

Conclusion: Both surgical and chemical denervation applied before muscle transfer increased muscle ischaemia tolerance. With similar experimental outcomes, denervation with BoNT-A can be preferred to surgical denervation because of its abundant clinical availability and it can be applied without any secondary surgery.
Conclusions Long-term outcomes after laparoscopic esophagomyotomy were excellent across a wide spectrum of disease severity and presentations. Previous treatments, such as balloon dilation or botulinum toxin (Botox) injection, did not portend worse outcomes. When the myotomy was extended 2 cm onto the stomach and a Dor fundoplication was performed, severe heartburn was rare. (Source: Archives of Surgery)

There often comes a time when IT's forward march brings a medical practice to a crossroads, a fork in the road where both directions lead to the same destination though not equally efficiently or cost effectively. Such was the case for Flower Mound Dermatology, an 11-employee suburban Dallas practice whose administrators seven years ago recognized the importance of adding Electronic Medical Records (EMR) capabilities to its existing UNIX-based Practice Management (PM) system.

In conclusion, BoNT/A alleviates lower urinary tract symptoms due to HBP, but different studies present considerable variations regarding the dose administered, inclusion criteria and follow‐up time, as well as poorly defined retreatment, losses to follow up and, above all, a high degree of variability in the communication of results (with large standard deviations). In consequence, further clinical trials are needed. © 2011 Wiley‐Liss, Inc.
Conclusions & Implications: In the clinical setting of the Drooling Treatment Project, the results indicated that the context in which drooling occurs is an important factor and suggested the value of considering the situational context when making drooling judgments. Further, there was a difference in the pattern of response between the children with cerebral palsy and those with operculum syndrome, suggesting that aetiology may be involved in the response to Botox®.

Authors: Smith KC
    Abstract
Now that several formulations of botulinum toxin type-A (BoNT-A) are on the market in North America and throughout the world, it is of great practical importance for those who administer these medications to be familiar with the differences between the formulations. These differences pertain to the stability of the medications, in particular during the period of time between reconstitution and administration, which may affect the degree of diffusion through various tissues after injection. A variety of relatively new uses of BoNT-A for dermatological conditions will be discussed.

Abstract In their systematic review, Zhang et al. evaluate randomized controlled trials on the efficacy of Botulinum toxin type A (BoNTA)
in helping patients with musculoskeletal disorders. Scoring each trial according to the Jadad scale they note that the studies
range from 1 to 5 with an average of 4.1, which they interpret as a testimony of the high quality of the studies included.
Unfortunately, we cannot share the author’s enthusiasm, as a perfect Jadad score of 5 is no guarantee that a study is of good
quality.

Studies from the University of Toronto's psychology department show that people who use more invasive anti-aging methods such as Botox injections or surgery are viewed more negatively than those who use milder techniques such as sun-avoidance and facial creams and younger adults are more negative about using anti-aging methods than older adults. "These results suggest that despite the rapid growth of the anti-aging cosmetic industry, age concealment has not yet become universally accepted," said lead author and associate professor, Alison Chasteen...
Source: FDA
Area: News
The FDA has approved botulinum toxin A injection (Botox®) to treat urinary incontinence in people with neurological conditions such as spinal cord injury and multiple sclerosis who have an overactive bladder. The approval was based on data from two clinical studies in 691 patients which showed statistically significant decreases in the weekly frequency of incontinence episodes in the Botox group compared with placebo. The most common adverse reactions observed following injection were urinary tract infection and urinary retention. Those who develop urinary retention after Botox treatment may require self-catheterisation to empty the bladder.

Botox is currently not approved for this indication in the UK. (Source: NeLM - News)

Botox (onabotulinumtoxinA) injection for individuals with urinary incontinence (bladder overactivity) resulting from spinal cord injury, multiple sclerosis and other neurologic conditions, has been approved by the FDA (Food and Drug Administration).
Conclusions: BTX-B prevented or reduced the incidence of thrombosis after acute vessel anastomosis in this rat model at all time points less than 120 hours compared with placebo. The average final vessel diameter throughout the series of BTX-B–treated vessels was significantly larger than in the control group.Clinical relevance: The use of BTX-B may improve the success rate of microvascular anastomoses by being protective against vasospastic stress and subsequent thrombosis.
This article provides an overview of how the constitutional protections for commercial speech affect the Food and Drug Administration's (FDA) regulation of drugs, and the emerging issues about the scope of these protections. A federal district court has already found that commercial speech allows manufacturers to distribute reprints of medical articles about a new off-label use of a drug as long as it contains disclosures to prevent deception and to inform readers about the lack of FDA review. This paper summarizes the current agency guidance that accepts the manufacturer's distribution of reprints with disclosures. Allergan, the maker of Botox, recently maintained in a lawsuit that the First Amendment permits drug companies to provide "truthful information" to doctors about "widely accept...

MedWorm Sponsor Message: Find the best January Sales in the UK.

This study assessed treatment consistency of botulinum toxin administration in spastic upper limbs under pragmatic conditions, as derived through stability of dosages and between injections intervals. Over a period of 8 years, 153 children (81 with bilateral spastic cerebral palsy, 72 with unilateral) were treated according to accepted, experience-based guidelines with Botox and Dysport. Treatment response was based on assessment of spasticity and attainment of pre-determined goals at 3, 6 and 12 months post each treatment. Mean age at treatment onset was 6y 4mo (SD: 4y 10mo), median F/U, 2.5 years (4 months-6 8/12 years). Number of injection sessions was 1-10; few had more than 6 sessions. In 106 (69.28%) children, more than one anatomic regions of the limb were injected. Most (56.2%), ha...

Conclusions Laparoscopic Heller myotomy for achalasia is associated with a high success rate as the primary therapeutic option and after
failure of endoscopic therapy. It can be performed safely and with favorable outcomes also in patients >40 years. However,
the long-term durability of the procedure remains to be established.

AbstractExercise can improve and maintain neural or muscular function, but the effects of exercise in physiological adaptation to paralysis caused by botulinum toxin A has not been well studied. Twenty‐four rats were randomly assigned into control and treadmill groups. The rats assigned to the treadmill group were trained on a treadmill three times per week with the running speed set at 15 m/min. The duration of training was 20 min/session. Muscle strength, nerve conduction study and sciatic functional index (SFI) were used for functional analysis. Treadmill training improved the SFI at 2, 3, and 4 weeks (p = 0.01, 0.004, and 0.01, respectively). The maximal contraction force of the gastrocnemius muscle in the treadmill group was greater than in the control group (p < 0.05...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Botulinum Toxin Decreases Hyperalgesia and Inhibits P2X3 Receptor Over‐Expression in Sensory Neurons Induced by Ventral Root Transection in Rats

Conclusions.  L5 VRT led to over‐expression of P2X3 receptors in the L5 DRG and bilateral mechanical allodynia in rats. Subcutaneous injection of BTX‐A significantly reversed the neuropathic pain behavior and the over‐expression of P2X3 receptor in nociceptive neurons. These data not only show over‐expression of purinergic receptors in the VRT model of neuropathic pain but also reveal a novel mechanism of botulinum toxin action on nociceptive neurons.
Intradermal injection of Botulinum toxin type A alleviates infraorbital nerve constriction‐induced thermal hyperalgesia in an operant assay

Summary  Recent studies have shown that infraorbital nerve constriction (IoNC)‐induced mechanical allodynia has been attenuated by administration of highly purified 150‐kDa Botulinum neurotoxin type A (BoNT/A). Here, we extend these studies to determine whether BoNT/A could attenuate IoNC‐induced symptoms of thermal hyperalgesia. Instead of testing head withdrawal thresholds, a thermal operant assay was used to evaluate cortical processing of sensory input following IoNC. In this assay, a fasted rat’s desire to obtain a food reward (sweetened condensed milk) is coupled to its ability to tolerate facial contact with a warm (45 °C) thermode. Bilateral IoNC decreased the ratio of thermode contact duration/event, which is an indicative of thermal hyperalgesia. BoNT/A injection int...
