Conclusions.  PDE5 represent first line ED therapy only for SCI patients, though treatment results through meta‐analysis were not possible. Encouraging results are reported for Parkinson's and MS patients. PDE5 use for other CND patients is limited for various reasons, such as ED and concomitant libido impairment caused by depression and/or sexual endocrinology dysfunctions, and because PDE5 may cause a worsening of neurological illness. Medical centers staffed by health professionals able to counsel patients on the possible use of PDE5 are needed. Lombardi G, Nelli F, Celso M, Mencarini M, and Del Popolo G.
AbstractThe present study was carried out to investigate the effects of long‐acting phosphodiesterase five inhibitor, tadalafil, on pulmonary hypertension induced by chronic hypobaric hypoxia in rats. Adult Albino Wistar rats were exposed to 2 weeks of hypobaric hypoxia for 8 h daily and treated with tadalafil or tempol, a standard antioxidant agent. Right ventricular systolic pressure (RVSP) was taken as an index for pulmonary arterial pressure; malondialdehyde, reduced glutathione and superoxide dismutase were chosen as the markers of oxidative stress; serum tumour necrosis factor alpha (TNF‐α) levels and inflammatory changes in lungs were assessed for inflammation. Chronic hypobaric hypoxia was found to induce pulmonary hypertension, as it significantly (P &lt; 0.001) incre...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Please support the <a href="http://www.doctorsinchains.org/" target="_blank">Doctors In Chains</a> campaign for the <a href="http://www.doctorsinchains.org/">medics</a> tortured and sentenced for up to 15 years in <a href="http://www.doctorsinchains.org/">Bahrain</a>. <a href="https://twitter.com/#!/search/%23FreeDoctors">#FreeDoctors</a></p></div>

Conclusion&nbsp;&nbsp;Tadalafil 20&nbsp;mg three times/week shows better results than 10&nbsp;mg atorvastatin daily. Atorvastatin alone seems to improve EF
compared to not using any medication, and this significance is more prominent in patients with supranormal serum lipid levels.
Further studies with subgroups of different serum lipid levels should be conducted.

CONCLUSIONS: Based on current studies, sildenafil appears to be well tolerated and can improve markers of cardiovascular and pulmonary function in patients with HF. PDE5 inhibitors may be a therapeutic option for patients who cannot tolerate standard therapy for HF or who remain symptomatic with standard therapy. Larger long-term trials are necessary to better understand the role of PDE5 inhibitors in HF.

In this study, we established a detailed procedure for simultaneous analysis of typical ED drugs (sildenafil,
vardenafil, tadalafil) and their metabolites in human blood and urine by isotope dilution liquid chromatography-tandem mass
spectrometry (LC–MS–MS). Each sample of whole blood and urine containing the three ED treatment drugs, their metabolites,
and deuterated internal standards (ISs) was diluted with alkalinized water, loaded onto an Oasis HLB cartridge, washed with
dilute ammonium hydroxide solution, and eluted with chloroform. The eluate was acidified with methanol and concentrated HCl
and evaporated to dryness. The resulting residue was reconstituted with methanol and mobile phase solution, and 5&nbsp;μl of the
solution was injected into an LC–MS–MS instrument. T...

    Abstract
We characterized the nitric oxide (NO)-cyclic GMP-phosphodiesterase-5 (PDE5) pathway in Crotalus durissus terrificus aorta. Concentration responses curves to acetylcholine (ACh), sodium nitroprusside (SNP), BAY41-2272 (soluble guanylyl cyclase [sGC] stimulator), BAY60-2770 (sGC activator) and tadalafil (PDE5 inhibitor) were constructed in phenylephrine (10μM)-precontracted tissues with intact (E(+)) or denuded (E(-)) endothelium. ACh (0.001-10μM) and SNP (0.0001-10μM) relaxed aorta, which were reduced by the NO synthase (L-NAME,100μM) or the sGC inhibitors (ODQ, 10μM). Tadalafil (0.0001-10μM) relaxed E(+) rings with potency (pEC(50)) and maximal response (E(max)) values of 7.34±...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Find the best <a href="http://www.januarysales.org/" target="_blank">January Sales</a> in the UK.</p></div>

In conclusion, PDE5 inhibitors applied luminally had minor contractile effect, whereas abluminal sildenafil induced middle cerebral artery dilatation above therapeutic levels. In vivo, sildenafil dilated middle meningeal artery concomitant with a reduction in blood pressure. Tadalafil had no dilatory effects. PDE5 inhibitors show differential vascular activity in cerebral arteries from healthy animals; arterial dilatation is seen primarily above therapeutic levels. Such findings support clinical studies showing no vasodilator effects of sildenafil on cerebral arteries in healthy subjects.

    Abstract
Phosphodiesterases (PDEs) are a family of enzymes that hydrolyze cyclic nucleotides, thereby modulating cell functions. Three highly selective PDE5 inhibitors (PDE5i), sildenafil, vardenafil and tadalafil, have been developed for treatment of erectile dysfunction (ED). Experimental evidence showed that chronic treatment with sildenafil PDE5i in a mouse model of diet-induced obesity and insulin resistance improved insulin action and decreased circulating fatty acid levels. It has recently been shown that healthy athletes use PDE5i as performance enhancers, hence in the present study we investigated whether the long-lasting PDE5i tadalafil influences energy metabolism in C2C12 skeletal muscle cells by evaluating lacta...

The FDA announced that it has approved Eli Lily's answer to Viagra, known as Cialis, for the treatment of enlarged prostate. This should prove to be a boon for Eli Lily's blockbuster impotence medication. The FDA stated that Cialis can now be labelled for treatment of a non cancerous enlarged prostate, (called benign prostatic hyperplasia (BPH) and be used for people who have both BPH and erectile dysfunction. Nearly half of men over fifty suffer from enlarged prostate problems that can cause difficulty urinating... (Source: Health News from Medical News Today)

Authors: Francis SH, Corbin JD
    Abstract
Erectile dysfunction (ED) is strongly linked to cardiovascular disease (CVD), especially in diabetics. ED is associated with deleterious changes in the overall vasculature and is recognized as an indicator of higher risk for adverse cardiovascular events. Endothelial dysfunction, vascular smooth muscle changes and increased fibrosis are indicated as major players in both ED and CVD. ED in diabetics is more refractory to acute treatment with phosphodiesterase-5 (PDE5) inhibitors (Viagra, Cialis, Levitra, Zydena) than in non-diabetics, but recent studies indicate that chronic administration of these drugs improves endothelial function, preserves vascular smooth muscle and decreases fibrotic changes. Use of PDE5 inhibitors in pre-diabetic and di...

HIV-positive men are commonly prescribed phosphodiesterase-5 (PDE5) inhibitors for erectile dysfunction (ED). When prescribing, consideration should be given to possible onward transmission of HIV, which is enhanced by the presence of other sexually transmitted infections (STIs).1&ndash;3 We were unable to find any reports of STI acquisition in men with HIV using PDE5 inhibitors. The aim was to audit the management of ED within our HIV cohort in Belfast according to British Society of Sexual Medicine guidelines.4 Documentation of STI diagnoses and safer sex discussion was also reviewed in those who had received PDE5 inhibitors (sildenafil, tadalafil and vardenafil) between 2002 and 2010. Data were obtained from case notes and pharmacy records. Fifty HIV-positive men received PDE5 inhibitor...

In this study, impurity profiles were treated like fingerprints representing the quality of the samples. A total of 73 samples of counterfeit and imitations of Viagra(®) and 44 samples of counterfeit and imitations of Cialis(®) were analysed on a HPLC-UV system. A clear distinction has been obtained between genuine and illegal tablets by the mean of a discriminant partial least squares analysis of the log transformed chromatograms. Following exploratory analysis of the data, two classification algorithms were applied and compared. In our study, the k-nearest neighbour classifier offered the best performance in terms of correct classification rate obtained with cross-validation and during external validation. For Viagra(®), both cross-validation and external validation sets returned a 10...

Abstract: Following a health complaint a food supplement was brought in for analysis on the suspicion of being adulterated with a synthetic drug substance. When the capsule content did not show evidence of adulteration, the capsule shell was investigated. Using HPLC-DAD and HPLC–MS the capsule shell was found to contain 2.85mg of the erectile dysfunction drug tadalafil. Using microscopy and RAMAN spectroscopy the presence of tadalafil was shown throughout the gelatine matrix as particles and dissolved into the matrix. The adulteration is probably carried out by adding tadalafil powder to a gelatine jelly in the manufacturing of the capsules shells. Because this technique may also be used for other drug substances, capsules shells should be considered a vehicle for hiding drug substances ...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Find the best <a href="http://www.januarysales.org/" target="_blank">January Sales</a> in the UK.</p></div>

Conclusion.  Human vesicular‐deferential artery is characterized by a high expression and activity of PDE5, which was inhibited by tadalafil in vitro. In SHR, tadalafil increases prostate tissue oxygenation, thus suggesting a possible mechanism through which PDE5i exert beneficial effects on LUT symptoms.
Conclusions.  While it is evident that PDE5is are the first choice in patients with comorbid ED and PE (where one may be secondary to the other), well‐designed studies on the possible use of PDE5is in PE patients without ED are still limited. The issue will be less controversial when further evidence on the role of NO and PDE5 in the mechanism of ejaculation is available. Jannini EA, McMahon C, Chen J, Aversa A, and Perelman M.
This study included 50 patients with arterial ED. EMPa and VR levels were measured by using flow cytometry. The CD45neg‐CD144pos‐annexin VPos events were defined as EMPa, and the CD51pos–CD61pos events were defined as VR. Patients with ED were evaluated before and after daily treatment with 5 mg tadalafil for 90 days. Patients with arterial ED had serum concentrations of EMPa and VR significantly higher than the control group at baseline. After tadalafil, the serum concentrations of EMPa and VR of the patients with arterial ED were significantly lower than before treatment, but significantly higher than controls. Patients with arterial ED expressed higher levels of both EMPa that are associated with dysfunction of the arterial endothelial pathway and VR‐expressing platelet‐en...

Conclusion.  There is lack of conclusive evidence to indicate a direct cause–effect relationship between PDE5 inhibitor use and vision‐threatening ocular events. Men who use PDE5 inhibitors appear to suffer vision‐threatening complications at the same frequency as the general population. However, minor visual adverse effects occur in 3–11% of users and they are transient and reversible. Azzouni F and Abu samra K.
This study aimed at exploring whether the chronic use of tadalafil has an adverse effect on the testes and semen of old albino rats. Sixty male albino rats were divided into three groups. Group 1: given 2 ml saline orally for 90 days. Group 2: received tadalafil orally (1.8 mg kg−1 day−1 for 3 months equivalent to 20 mg day−1 for 3 months as in human dose) and Group 3: received tadalafil orally (1.8 mg kg−1 day−1 for 6 months equivalent to 20 mg day−1 for 6 months as in human dose). Animals were observed daily for signs of toxicity and mortality. Body weight and food consumption were recorded once a week. After sacrificing the animals, gross examination of the testes was performed in situ and then the epididymis was processed for the evaluation o...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Please support the <a href="http://www.doctorsinchains.org/" target="_blank">Doctors In Chains</a> campaign for the <a href="http://www.doctorsinchains.org/">medics</a> tortured and sentenced for up to 15 years in <a href="http://www.doctorsinchains.org/">Bahrain</a>. <a href="https://twitter.com/#!/search/%23FreeDoctors">#FreeDoctors</a></p></div>

CONCLUSIONS: In the small number of studies available, tadalafil was effective and well tolerated when used to treat patients with PAH. Compared with placebo, tadalafil was associated with significant improvements in exercise capacity and reduced time to clinical worsening (68% relative risk reduction; P = 0.038). There is limited evidence comparing tadalafil with sildenafil and vardenafil, and the studies are limited by short treatment durations.

Predicting off-targets by computational methods is gaining increasing interest in early-stage drug discovery. Here, we present a computational method based on full 3D comparisons of 3D structures. When a similar binding site is detected in the Protein Data Bank (PDB) (or any protein structure database), it is possible that the corresponding ligand also binds to that similar site. On one hand, this target hopping case is probably rare because it requires a high similarity between the binding sites. On the other hand, it could be a strong rational evidence to highlight possible off-target reactions and possibly a potential undesired side effect. This target-based drug repurposing can be exten...

Erectile dysfunction (ED) affects 8% of 40‐year‐old men, increasing to 40% in 60‐year‐old men. It can be the first presentation of vascular disease and the average time between onset of ED and the first coronary vascular event is 3 years. The prevalence of ED is increasing and will continue to increase as the population ages. Prostate surgery commonly causes ED and nurses need to identify men with ED early, as treatment for ED should be commenced at the earliest opportunity provided the man and his partner want to engage in sexual activity. Options for treatment include phosphodiesterase type 5 inhibitors (sildenafil, tadalafil or vardenafil) as required, or ‘once daily’ for tadalafil. Alternative options include urethral pellets, intracavernosal injections, vacuum devices, cou...

It has recently been shown that healthy athletes use PDE5i as performance enhancers, hence in the present study we investigated whether the long-lasting PDE5i tadalafil influences energy metabolism in C2C12 skeletal muscle cells by evaluating lactate producti...

It is of great importance to investigate an effective and reliable medication against chronic renal failure (CRF)-related erectile dysfunction (ED), which aims to improve patients' life qualities. The concentrations of cyclic guanosine monophosphate (cGMP) in the corpus cavernosal smooth muscle of both CRF and control rabbits were measured. The effects of various concentrations of tadalafil, papaverine, and sodium nitroprusside on the relaxation responses of corpus cavernosal smooth muscle pre-contracted with phenylephrine in CRF rabbits were observed. There was significant difference in the concentration of cGMP between CRF and control rabbits (P&lt;0.01). Tadalafil had the greatest impacts on CRF rabbits when given the same concentration of papaverine ...

Abstract: Lower urinary symptoms (LUTS), including the overactive bladder (OAB) syndrome, can be found in 10–15% of all men and women and have often major effects on quality of life and social functioning. The first line of pharmacological treatment of OAB in women has been and still is antimuscarinic drugs. In men α1-adrenoceptor (AR) antagonists remain the standard treatment of LUTS. However, recent advances in the physiology/pathophysiology of LUTS/OAB, recognizing the functional contribution of the urothelium, the spontaneous myocyte activity during bladder filling, and the diversity of nerve transmitters involved, have sparked interest in novel possibilities to treat these conditions. For example, new, selective α1-AR antagonists (naftopidil, silodosin), β3-AR agonists (mirabegro...

Authors: Corbin JD, Francis SH
Phosphodiesterase-5 (PDE5) is a dimer containing a cGMP-specific catalytic domain and an allosteric cGMP-binding subdomain (GAF A) on each subunit. PDE5 exhibits three conformational forms that can be separated by Native PAGE and are denoted as Bands 1, 2, and 3 in decreasing order of mobility. A preparation comprised mainly of Band 2 PDE5 was partially converted to Band 3 PDE5 by 1h incubation with cGMP or the PDE5-specific inhibitors sildenafil, vardenafil, or tadalafil, but not with cAMP, milrinone (PDE3-specific), or rolipram (PDE4-specific). Band 2 PDE5 was converted almost entirely to Band 3 PDE5 by overnight incubation with sildenafil at 30°C. This time-dependent conversion was accompanied by a 7-fold increase in allosteric cGMP-binding activity, ...

Conclusions: Oral tadalafil plus low dose vasodilator led to a significantly better clinical response than high dose vasodilator. Penile color duplex ultrasound parameters showed no difference between the 2 modes. Thus, this mixed mode emerges as a possible alternative to high dose vasodilator injection. But scientists from the University of Guelph have come up with evidence natural aphrodisiacs exist. In fact, if you want to spice up your sex life, try doing it literally -- with spices.Specifically, ginseng and saffron added to the diet are proven sexual performance boosters, according to the researchers' new scientific review of hundreds of studies of natural aphrodisiacs. Yohimbine, a natural chemical from yohimbe trees in West Africa, was also found to improve human sexual function.People reported increased sexual desire after eating muira puama, a flowering plant found in Brazil, and maca root, a mustard plant grown in the Andes, too.
Conclusions.  Rattlesnake CC relaxation is mediated by the NO‐cGMP‐PDE5 pathway in a manner similar to mammals. The novel TTX‐resistant Na channel identified here may be responsible for the slow response of smooth muscle following nerve stimulation and could explain the extraordinary duration of snake coitus. I’ve just spent two weeks in a row on the road at different meetings—first the Health Research Alliance (HRA) meeting in New York and then the Muscular Dystrophy Association meeting in Las Vegas.

HRA is an organization made up of most of the major non-profit funders of health research -- the members meetings are designed to keep us up to date on the latest trends in research and funding and to allow us to learn from one another. I chair the HRA Working Group on “Funders of Drug and Other Therapeutics Development” which met over breakfast in New York. For this working group meeting we invited three industry gue...

Area: Evidence > Drug Class Focused Reviews
CRD Summary: This generally well-conducted review concluded that oral PDE-5 inhibitors improved erectile function and had similar efficacy and safety profiles; the efficacy and value of hormone treatments remained uncertain. Despite some limitations, the review included a large number of studies and the direction of effect seemed to be consistent across studies and analyses. The conclusions are likely to be reliable.
[ Included studies evaluated sildenafil, vardenafil, tadalafil, mirodenafil, udenafil and hormonal treatments (oral, intramuscular, gel, cream or patch testosterone); drug regimens varied across studies. ]
CRD Commentary: The authors addressed a clear research question with appropriate inclusion criteria. Several so...

Authors: Park K, Hwang EC, Kim SO
Erectile dysfunction (ED) is an important worldwide health issue that has a significant negative impact on the quality of life and life satisfaction of both the affected individual and his partner. Here we review the prevalence of ED in Asia, associated factors that may influence sexual attitudes and sexual behaviours, and randomized clinical trials (RCTs) of phosphodiesterase-5 (PDE-5) inhibitors to evaluate the clinical efficacy and safety of PDE-5 inhibitors in Asian men. We searched for English-language articles in MEDLINE and PubMed from January 2000 to September 2010. Our results showed that the overall reported prevalence rate of ED in Asia ranged widely, from 2% to 88%. This finding indicates that ED is a common and major health problem in this...

Area: News
In 2010, UK Border Agency (UKBA) officers seized over 8.5 million fake and unlicensed Viagra tablets, worth around £13 million, before they could enter the black market. The Agency refers seized pharmaceutical drugs to the MHRA who then look to take criminal enforcement action against those attempting to trade the tablets on the black market.

In a related story, a gang who planned to make half a million counterfeit tadalafil tablets with a street value of £1.6 million from a residential flat in central London has been jailed at the Old Bailey, following a City of London Police investigation. (Source: NeLM - News)

This study evaluated the effects of single-dose administration and steady-state concentrations of tipranavir 500 mg and ritonavir 200 mg (TPV/r) combination on the pharmacokinetics of tadalafil 10 mg (TAD) in an open-label study. Seventeen healthy male volunteers received sequential dosing of the studied product: TAD (day 1) alone in a single dose for 7 days followed by TAD (day 8) in a single dose with TPV/r (500/200 mg twice daily, days 8-18). Pharmacokinetic parameters were determined in a non-compartmental analysis. The geometric mean ratio and 90% confidence interval were used to evaluate drug interactions. The effect of a single dose of TAD on the pharmacokinetics of TPV/r resulted in a small decrease in exposure after either first-dose or steady-state TPV/r (geometric mean ratios [9...

ConclusionsThe pathophysiology of male LUTS is complex and not completely understood. LUTS may occur independently of BPH or secondary to BPH but in both cases involve obstructive or irritative mechanisms with substantial pathophysiological overlap. While the precise mechanism remains unclear, inhibition of PDE5 seems to have an effect on several pathways that may impact LUTS. © 2011 Wiley‐Liss, Inc.
Conclusions.  Our results demonstrate that PDE5 is expressed in human visceral adipocytes and that acute exposure to PDE5i selectively stimulates ARO expression, which is related to a specific PDE5 blockade. We speculate that modulation of ARO activity by PDE5i could be one of the mechanisms responsible, at least in part, for the beneficial effects of PDE5i on endothelial and metabolic functions.
AbstractStarting from tadalafil as a template, a series of functionalized tetrahydro‐β‐carboline derivatives have been prepared and identified as novel potent and selective PDE5 inhibitors. Replacing the 3,4‐methylenedioxyphenyl at position 6 of tadalafil, together with elongation of the N2‐methyl substituent and manipulation of the stereochemical aspects of the two chiral carbons led to the identification of compound XXI, a highly potent PDE5 inhibitor (IC50 = 3 nM). Compound XXI was also highly selective for PDE5 versus PDE3B, PDE4B, and PDE11A, with a selectivity index of 52 and 235 towards PDE5 rather than PDE11 with both cAMP and cGMP as substrate, respectively.PDE5 has been successfully targeted for treatment of male erectile dysfunction.
PDE5A is a cyclic nucleotide phosphodiesterase (PDE) that specifically catalyses the hydrolysis of the phosphodiester bond of 3',5'-cyclic GMP. It is a homodimer composed of two ~100 kDa subunits, each of which contains a regulatory and a catalytic domain. PDE5 is phosphorylated by PKG or PKA and phosphorylation is stimulated by allosteric cGMP binding. Phosphorylation of PDE5 occurs in intact cells when cGMP is elevated. This increases the affinity of allosteric cGMP binding as well as increasing both the affinity of the catalytic site for the cGMP substrate and the rate of cGMP hydrolysis. PDE5 is potently inhibited by vardenafil (Levitra), tadalafil (Cialis) and sildenafil (Viagra and Revatio) - medications that are used clinically for treatment of erectile dysfunction and pulmonary hyp...

Vasodilatation in the muscle microcircle was evaluated using VO2on kinetics in constant load cycle ergometer exercise, at load less of 10% than that corresponding to anaerobic threshold in previous incremental test identified. At onset of this constant load exercise, VO2 increases early and fast [first phase (φI)]; it follows a slower increase [second phase (φII)], which appears with variable delay, and reaches in about 3 minutes steady-state [third phase (φIII)] by single exponential function. We excluded φI from our measurements because invalided by many fakes. ΨII was attributed to O2 decreased content in active muscles blood, where this undergoes a gas exchange. ΨII time constant (τ) can be decreased by exercise training so as heart disease therapy, whereas heart diseases prolon...

Background:
To assess the measurement properties of the Benign Prostatic Hyperplasia Impact Index (BII) for use in men with Lower Urinary Tract Symptoms (LUTS) secondary to Benign Prostatic Hyperplasia (BPH) treated with tadalafil.
Methods:
Data from a dose-titration (Study 1) and a dose-finding placebo-controlled (Study 2) tadalafil studies of men 45 years of age or older with moderate to severe LUTS (N=281; N=1053) were included in this post-hoc analysis. Measures included the BII, International Prostate Symptom Score (IPSS), IPSS Quality of Life Index (IPSS-QoL), LUTS Global Assessment Question, uroflowmetry measure peak flow rate (Qmax) and postvoid residual volume (PVR). Spearman rank and Pearson correlation coefficients were computed between the BII score and the other measures at ea...

You may believe that you will have difficulty with this forever or that is not solvable unless you take medication for the problem but a pill may not fix all of the trouble. Here's why.... For most men, erectile problems are caused by an anxiety issue, not a medical issue. While prescriptions such as Viagra, Levitra and Cialis may help to solve the problem of getting your penis hard, you may not get to the core of why your penis is having difficulty getting hard in the first place. By then you are committed to taking prescriptions and "timing" when we are sexual for the rest of your sexual relationship.
