Abstract Topical glucocorticoid (GC) therapy has been successfully used in the treatment of several common cutaneous diseases in clinical
practice for a long time, and skin atrophy is one of the most typical cutaneous side effects of this therapy. The aim of this
study was to evaluate the potential of noninvasive fluorescence spectroscopy (FS) technique in the detection and classification
of GC-induced skin atrophy. A total of 20 male Wistar rats were used in the experimental protocol under controlled environmental
conditions and with free access to food. One group received topical application of clobetasol propionate 0.05% for 14 days
to induce cutaneous atrophy (atrophic group) and the other (control) group received only vehicle application following the

AbstractProtease‐activated receptor 2 (PAR2) is a G protein‐coupled receptor which mediates a variety of functions in the skin including cutaneous inflammation. SLIGKV‐NH2, an agonist peptide for PAR2, enhanced the interleukin (IL)‐17‐induced production of two CXC chemokines, CXCL1 (GRO‐α) and CXCL8 (IL‐8), in normal human epidermal keratinocytes (NHEK) in a concentration‐dependent manner. The enhanced production of those chemokines was suppressed by a PAR2‐specific siRNA. The SLIGKV‐NH2‐induced production of both CXCL1 and CXCL8 was markedly reduced by cyclosporine A. The enhanced production of CXCL1 was suppressed by 1α, 24R‐dihydroxyvitamin D3, an active form of vitamin D3, and weakly by glucocorticoids, dexamethasone and clobetasol propionate, whereas producti...

AbstractItch accompanies various skin diseases. As a number of mediators other than histamine can be involved in the itch sensation, H1 receptor antagonists are not necessarily effective in treating itch. External application of anti‐pruritic drugs is occasionally used as an alternative therapy for pruritic skin conditions, such as pruritus on primary non‐diseased, non‐inflamed skin. Even so, the actual effects of these drugs on the itch sensation have yet to be studied in detail. To verify the anti‐pruritic effects of crotamiton, capsaicin, and a corticosteroid on the itch sensation, we examined the inhibitory effects of these drugs on various pruritogen‐induced scratching behaviors in mice. Topical application of 10% crotamiton moderately inhibited histamine‐, serotonin‐, a...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

CONCLUSIONS: The current limited evidence demonstrates the efficacy of clobetasol propionate, mometasone furoate, and pimecrolimus in treating genital lichen sclerosus. Further RCTs are needed to determine the optimal potency and regimen of topical corticosteroids, examine other topical interventions, assess the duration of remission or prevention of flares, evaluate the reduction in the risk of genital squamous cell carcinoma or genital intraepithelial neoplasia, and examine the efficacy in improving the quality of the sex lives of people with this condition.

Authors: Wang J, Barak LS, Mook RA, Chen W
    Abstract
The process of neurogenesis in mammals, which is prolific and widespread at birth, gradually slows with aging and in humans becomes restricted to areas including the cerebellum and hippocampus. It has been reported that exposure to glucocorticoids can impair neurogenesis in both adults and children. Glucocorticoids are known to bind with high affinity to intracellular receptors. Glucocorticoid blood levels are normally regulated by environmental stresses, but because of their clinical utility, exogenous glucocorticoids are frequently administered in drug formulations. Consequently, concerns have arisen about the consequences of glucocorticoid use on neurogenesis and health, especially in the pediatric population. In this article, ...

Conclusions: The results show that the concept of the risk‐to‐benefit ratio is reflected by the prescription profile of topical corticosteroids (therapeutic index). However, even corticosteroids with a less favorable therapeutic index are sometimes prescribed, perhaps due to economic reasons. (Source: JDDG)

Psoriasis is a chronic, autoimmune skin disease affecting approximately 2% of the world's population. Clobetasol propionate which is a superpotent topical corticosteroid is widely used for topical treatment of psoriasis. Conventional dosage forms like creams and ointments are commonly prefered for the therapy. The purpose of this study was to develop a new topical delivery system in order to provide the prolonged release of clobetasol propionate and to reduce systemic absorption and side effects of the drug. Clobetasol propionate loaded-poly(D,L-lactic-co-glycolic acid) (PLGA) microspheres were prepared by oil-in-water emulsion-solvent evaporation technique. Particle size analysis, morphological characterization, DSC and XRD analyses and in vitr...

Authors: Usatine RP, Tinitigan M
Lichen planus is a chronic, inflammatory, autoimmune disease that affects the skin, oral mucosa, genital mucosa, scalp, and nails. Lichen planus lesions are described using the six P's (planar [flat-topped], purple, polygonal, pruritic, papules, plaques). Onset is usually acute, affecting the flexor surfaces of the wrists, forearms, and legs. The lesions are often covered by lacy, reticular, white lines known as Wickham striae. Classic cases of lichen planus may be diagnosed clinically, but a 4-mm punch biopsy is often helpful and is required for more atypical cases. High-potency topical corticosteroids are first-line therapy for all forms of lichen planus, including cutaneous, genital, and mucosal erosive lesions. In addition to clobetasol, topical tac...

To the Editor: A recent survey conducted in the United States revealed that 120 of 500 (24%) respondents had at least one tattoo. Of these respondents, 32 (26%) had nonprofessional tattoos applied by themselves, a family member, or a friend. The dangers of this approach are highlighted by the case of a 24-year-old woman who was evaluated for severe pruritus, swelling, pain, and draining sores at the sites of “glow in the dark” tattoos on both wrists; her symptoms had been worsening over the course of 5 months. A friend used a needle to insert LumiNova (United Mineral & Chemical Corp, Lyndhurst, NJ) without dilution under the skin. The physical examination revealed violaceous, edematous, and ulcerated linear plaques in the shape of smiling faces circumferentially around both wrists (). ...

To the Editor: The research letter entitled “Percuta-neous absorption of potent topical corticosteroids in patients with severe atopic dermatitis” reported that patients with suppressed serum cortisol at discharge from hospital had normal serum cortisol levels an average of 41 days after hospitalization, despite maintenance doses of clobetasol (mean dose, 27 g/wk) or betamethasone valerate (mean dose, 34 g/wk). The study presumably utilized medication weights to assess how much medication patients used; however, medication weights and self-reporting of outpatient topical medication use can grossly overestimate the actual dosage used. The possibility of poor adherence must be considered and, without objective assessment, it is difficult to know whether medication was truly applied. The...

A 15-year-old African-American girl presented with a 5-year history of asymptomatic enlarging facial plaques. The plaques had been previously diagnosed as tinea and treated with various topical and oral antifungals without improvement. Examination revealed a 2 cm × 1 cm hyperkeratotic, indurated plaque on the left medial canthus and similar smaller plaques with central depression and elevated borders on the nose and right cheek (, A). Analysis of a skin biopsy specimen confirmed discoid lupus erythematosus (DLE). Serologic evaluation revealed a positive SSA/Ro antibody; all other serologic and routine laboratory test values were normal. Hydroxychloroquine 200 mg twice daily, clobetasol ointment twice daily, and photoprotection were instituted, which stabilized the plaques and prevented t...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Authors: Fontana MC, Rezer JF, Coradini K, Leal DB, Beck RC
We developed a dermatological nanomedicine containing clobetasol propionate-loaded nanocapsules and evaluated its efficacy in a model of contact dermatitis after topical administration in rats. Hydrogels containing clobetasol propionate-loaded lipid-core nanocapsules or nanoemulsion (HG-CP-NC and HG-CP-NE, respectively) were prepared to evaluate the influence of the polymeric wall. They presented adequate pH values (5.50-6.50) and drug content (0.5mgg(-1)) and their rheograms exhibited a non-Newtonian pseudoplastic behavior. The best in vitro drug release control was obtained for HG-CP-NC (1.03±0.11μgcm(-2)h) compared to the HG-CP-NE (1.65±0.19μgcm(-2)h) and the hydrogels containing nonencapsulated drug (HG-CP) (2.79±0.22...

We present a case of a 29-year- -old circumcised man with an asymptomatic annular lesion of the penis. Histopathological features were consistent with LP. Topical clobetasol was prescribed, with clinical improvement. It is important to consider annular LP among the possible diagnoses of individual annular genital lesions.

Abstract: Some cosmetic products manufactured in Korea have been suspected to contain anti-inflammatory corticosteroids, such as clobetasol propionate and betamethasone dipropionate, for the treatment of eczema, seborrhea and psoriasis, without any indication on the label of the cosmetic products. Due to their severe side effects, such as permanent skin atopy, these two corticosteroids in cosmetic products need to be monitored from a forensic point of view. Cosmetic product samples (number of samples=47) of manufacturers charged by consumers have been collected in local and online markets of Korea, and they were validated and analyzed by a simple high performance liquid chromatography (HPLC) method with ultraviolet diode array (UV-DAD). LC–MS/MS and LC–MS were used to confirm these ste...

CONCLUSIONS: Ultrapotent topical corticosteroids (clobetasol propionate; 20 to 40g/day) are effective treatments for BP with fewer systemic side-effects than oral high-dose corticosteroids. Systemic corticosteroids are effective but doses greater than 0.5mg/kg per day are associated with severe side-effects, including decreased survival. The effectiveness of the addition of plasma exchange or immunosuppressants (azathioprine, mycophenolate mofetil) to systemic corticosteroids has not been established. Combination treatment with tetracycline and nicotinamide needs further validation.

We report a 62-year-old female patient presenting with an 18-month history of pruritus and burning of the vulva. Clinical examination of the vulva showed a tender nodule of the right labium minus. Histology confirmed Langerhans cell histiocytosis. Systemic involvement was excluded. Within 1 month the use of clobetasol propionate ointment led to resolution of both the patient's symptoms and the clinical appearance of the affected right labium minus. This resolution was maintained 12 months later.

We report the cases of two women who developed erythematous skin plaques with pustules that coalesced and evolved into erosions and crusted areas. Histology showed epidermal spongiosis with subcorneal pustules and a dermal infiltrate with eosinophils and neutrophils. Lesions were treated with topical clobetasol propionate 0.05% for 10 days followed by topical tacrolimus daily until complete resolution, and then twice weekly for 1 year, without relapse. The response to topical corticosteroids and tacrolimus further support the close relationship with erosive pustular dermatosis of the scalp. Topical therapy with tacrolimus may offer good long-term disease control.

We report on an 8-month-old female infant who
developed Cushing’s syndrome and adrenal insufficiency after diaper dermatitis treatment through the misuse of Clobetasol
without doctor’s prescription. Physiologic dose of hydrocortisone was prescribed to prevent an adrenal crisis for 3 months
and discontinued when HPA axis recovery was confirmed by normal morning cortisol and ACTH levels.

To the Editor: We read with great interest the case report published by Mariwalla et al in the August 2007 issue of the Journal regarding a 12-year-old girl with severe myiasis and scalp psoriasis. Recently, we had a similar case involving a 13-year-old girl who presented to our clinic with multiple larvae on the scalp. She had a 4-year history of psoriasis mainly affecting the scalp. For the previous 6 months, she had been treated with clobetasol propionate shampoo 0.05% twice a week with poor control. Five days earlier, the patient and her family reported a short stay in a marshy area. The day preceding consultation, her relatives noticed the presence of numerous larvae on the scalp measuring 4 mm to 7 mm in length and 2 mm to 3 mm in diameter. The physical examination revealed the pres...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

CONCLUSIONS: Very potent topical steroids are effective and safe treatments for BP, but their use in extensive disease may be limited by side-effects and practical factors. Milder regimens (using lower doses of steroids) are safe and effective in moderate BP. Starting doses of prednisolone greater than 0.75 mg/kg/day do not give additional benefit, lower doses may be adequate to control disease and reduce the incidence and severity of adverse reactions. The effectiveness of adding plasma exchange, azathioprine or mycophenolate mofetil to corticosteroids, and combination treatment with tetracycline and nicotinamide needs further investigation.

S Panda, K LahiriIndian Journal of Dermatology 2010 55(3):259-261A patient was referred to us with asymptomatic, erythematous, nonitchy, scaly lesions present bilaterally on the dorsa of his feet and toes since the last 2 months. Both the legs had pitting edema as well. There were hyperkeratosis, focal parakeratosis, acanthosis and scattered spongiosis in the epidermis, and proliferation of capillaries with perivascular infiltration of lymphomononuclear cells in the dermis. There was no serological evidence of hepatitis C virus. Laboratory investigations revealed hypoalbuminemia and low-normal serum zinc. On clinicopathological correlation, we made a diagnosis of necrolytic acral erythema (NAE). The lesions responded dramatically to oral zinc sulfate and topical clobetasol propionate withi...

Bullous pemphigoid is the most frequent autoimmune bullous skin disease and affects subjects who are about 80Â years old. The risk factors are neurological degenerative diseases, poor Karnovski's status and some drugs (aldactone and neuroleptics). Typically, the disease consists of itching eczematous or urticarial sheets, surmounted by blisters. The blisters heal without scars. Mouth and head are rarely involved. The diagnostic is made by histological examination . It shows a subepidermal blister with some degree of dermal infiltrate with lymphocytes and eosinophils. Direct immunofluorescence reveals a linear pattern of IgG deposition along the basal membrane, which signs the diagnosis. Indirect immunofluorescence detecting anti-basal membrane antibod...

MedWorm Sponsor Message: Find the best January Sales in the UK.

The work aimed to study a new mucoadhesive prolonged release tablet containing 24 mug clobetasol-17 propionate (CP) suitable for the management of oral lichen planus. Low swellable dosage forms were designed by combining a mucoadhesive polymer, i.e. poly(sodium methacrylate, methylmethacrylate), with hydroxypropylmethylcellulose and MgCl(2). This formulation was selected to modify the tablet erosion rate in order to obtain a release of CP over a 6 h period. A double-blind controlled study was performed using three groups of patient (n=16) which received 3 applications-a-day over 4 weeks of the developed CP tablets (group CP-T), placebo tablets (group CP-P) or commercial CP ointment for cutaneous appl...

Contact dermatitis is a common inflammatory skin condition characterized by erythematous and pruritic skin lesions that occur after contact with a foreign substance. There are two forms of contact dermatitis: irritant and allergic. Irritant contact dermatitis is caused by the non-immune-modulated irritation of the skin by a substance, leading to skin changes. Allergic contact dermatitis is a delayed hypersensitivity reaction in which a foreign substance comes into contact with the skin; skin changes occur after reexposure to the substance. The most common substances that cause contact dermatitis include poison ivy, nickel, and fragrances. Contact dermatitis usually leads to erythema and scaling with visible borders. Itching and discomfort may also occur. A...

Authors: Mann DJ, Vergilis-Kalner IJ, Wasserman JR, Petronic-Rosic V
A 71-year-old black woman presented to the dermatology clinic with a several-year history of increasing numbers of itchy white spots spreading over the chest, back, elbows, and legs. In 2004, the patient developed clusters of mildly pruritic hypopigmented and depigmented macules on her back and left shoulder along with depigmented flat papules on the lower extremities. The patient's condition was unresponsive to topical triamcinolone and tacrolimus, after which she was lost to follow-up for 3 years until her entire chest became involved. Her medical history was significant for hypertension and diabetes mellitus and her medications included amlodipine, aspirin, atorvastatin, hydrochlorothiazide, irbesartan, and metform...

Conclusion: This study suggests that the excimer laser can safely and effectively be used in the treatment of prurigo form of atopic dermatitis. For the long-term, the excimer laser might be a good alternative to topical corticosteroids and a option in case of therapy resistant patients.

CONCLUSION: In this study, we noted cutaneous superinfection in nine of 30 (30%) patients receiving topical corticosteroids for bullous pemphigoid, among which were three cases of fatal NF due to streptococcus A (10%). The infectious risks associated with TCS must not be neglected, particularly since treated patients are old and fragile, and frequently have multiple well-known risk factors for NF (e.g. extensive lesions, diabetes, etc.). In the event of signs of cutaneous superinfection, especially in cases of diabetes and prolonged TCS treatment, bacteriological analysis should be conducted. Adequate treatment should be initiated without delay, especially in cases of beta-haemolytic streptococcal infection.

Conclusions We found that topical calcitriol restored both the epidermal permeability and antimicrobial barrier that had been impaired by corticosteroids. This restoration was mediated by both an activation of the cutaneous vitamin D pathway and an increase of epidermal lipids and antimicrobial peptides, promoted by the formation of the LB and the activity of epidermal lipid synthesis-related enzymes.
Conclusions: Topical clobetasol propionate used twice daily for clinical treatment of phimosis does not affect the hypothalamus-pituitary-adrenal axis in most patients. However, salivary cortisol level should be considered as a laboratory marker in long-term treatment or during repeated cycles to detect possible hypothalamus-pituitary-adrenal axis suppression.
We report here the case of a 38-year-old woman who referred to us for an infiltrated, red-brownish plaque localized on her left cheek. The patient had been treated for a cutaneous leishmaniasis, confirmed by the histologic examination, localized at the same site. She was completely healed after an appropriate local and systemic treatment. She experienced the occurrence of the new plaque at the site of the previously healed cutaneous leishmaniasis three month later. Histologic examination and laboratory tests were consistent with a diagnosis of discoid cutaneous chronic lupus erythematosus. Treatment with hydroxychloroquine, topical clobetasol and topical tretinoin resulted in flattening and clearing of the lesion. Our case is the first case of isotopic response where a discoid chronic lupu...

MedWorm Sponsor Message: Find the best January Sales in the UK.

We report a 22-year-old man who developed severe inflammatory and keloidal, moderately bullous allergic reaction after henna paint-on tattoo. We did a patch test separately with these painting products (henna powder, PPD) and with the European standard series. PPD was strongly positive (+++) on day 2 and remained positive for following days. After treating with topical clobetasol-17 butyrate, resolution was obtained in two weeks. But some keloidal reaction remained.
Conclusions: We found that topical calcitriol restored both the epidermal permeability and antimicrobial barrier that had been impaired by corticosteroid. This restoration was mediated by both an activation of the cutaneous vitamin D pathway and an increase of epidermal lipids and antimicrobial peptides, promoted by the formation of the lamellar body and the activity of epidermal lipid synthesis-related enzymes.

The British Medical Journal has featured a review on the diagnosis and management of lichen sclerosis. The review starts with a case vignette, and discusses clinical features and investigations required for the diagnosis.

In terms of management, the review states, "After diagnosis, the accepted treatment is with the very potent topical steroid clobetasol propionate ointment. No controlled trials have been published to establish the best regime, but the guidelines suggest a single application at night for four weeks followed by alternate nights for four weeks and then twice a week for four weeks. Once symptoms are well controlled, the steroid ointment is used as required. Some patients will achieve complete remission; others will require ongoing treatment....

Please cite this paper as: Topical hydrocortisone, clobetasol propionate, and calcipotriol do not increase photocarcinogenesis induced by simulated solar irradiation in hairless mice. Experimental Dermatology 2010.Abstract: Topical corticosteroids such as hydrocortisone-17-butyrate (HCB) and clobetasol-17-propionate (CP) and vitamin D3 derivatives such as calcipotriol (CAL) are widely used to treat psoriasis. The immunosuppressive effects of corticosteroids make their topical use a concern for skin carcinogenicity. Few studies have assessed the effect of topical corticosteroids and topical vitamin D3 derivatives on photocarcinogenesis induced by ultraviolet radiation. We investigated whether HCB, CP, or CAL can accelerate photocarcinogenesis using simulated solar radiation (SSR). HCB, CP, ...

(Source: Australasian Journal of Dermatology)

(Source: Australasian Journal of Dermatology)

The aim of the present study was to evaluate through ultrasound imaging the response to treatment of twice and once daily application of clobetasol propionate 0.05% foam on psoriatic skin as well as the atrophogenic potential of once daily application of the foam on healthy skin. The study included a total of 40 participants, 30 affected by Psoriasis vulgaris and 10 healthy volunteers. Patients with psoriasis were branched in two groups of 15 subjects: in the first group, clobetasol propionate 0.05% foam was applied twice daily for up to 2 weeks on targeted plaques, in the second group, it was applied once daily for up to 4 weeks. Ten healthy adult volunteers were instructed to apply the foam to a 4 × 4 cm area on the volar aspect of the forearm once daily for 4 weeks. Ultrasound evaluati...

In December 2004, a 69-year-old man came to the ENT unit of the University of Parma complaining of a painful cutaneous lesion in the left retroauricular region. The patient reported that the lesion appeared 4 weeks after a cochlear implantation procedure performed elsewhere 2 months before because of a bilateral profound deafness. The lesion was initially managed by his practitioner with oral and topical antibiotics in the suspicion of impetigo. Nevertheless, no benefits came after a 15-day antibiotic course. The patient's medical history included diagnosis of oral pemphigus vulgaris (PV) 6 years before that was treated with oral azathioprine and topical clobetasol dipropionate ointment. After complete clinical remission of the disease, a low-dose immunosuppressive therapy was prescribed f...

MedWorm Sponsor Message: Find the best January Sales in the UK.

Treatment of scalp psoriasis with clobetasol-17 propionate 0.05% shampoo: a study on daily clinical practice

Conclusions Short-contact treatment with clobetasol-17 propionate 0.05% shampoo has high user convenience and patient satisfaction rates. Moreover, the treatment is well-tolerated and efficacious from patients' perspective. Subgroup analyses did not reveal factors predicting treatment outcome, although treatment success tended to be more evident in patients who had received fewer treatments previously. The aim of this study was to evaluate the efficacy of a 0.05% clobetasol propionate ointment administered in trays to 22 patients with desquamative gingivitis in a double-blind, crossover, placebo-controlled trial. Patients received container number 1 and were instructed to apply the ointment 3 times a day for 2 weeks, and to reduce the application to once a day in the third week. Next, the patients were then instructed to discontinue the treatment for 2 weeks, and were then given container 2, used in the same way and for the same length of time as container 1. Regarding signs, 17 patients presented some improvement, while 5 experienced worsening with clobetasol propionate. With the placebo, 14 patients presen...

Conclusion. The results of our small retrospective study suggest that TCPO may be more effective than either 0.1% tacrolimus or clobetasol propionate 0.05% ointment monotherapy in the treatment of recalcitrant CLE. Randomized controlled trials are needed to confirm these preliminary findings. (Source: Clinical And Experimental Dermatology)

This study investigated the effect of potent intracanal corticosteroids on periodontal healing of replanted avulsed teeth and evaluated the systemic absorption of these corticosteroids. Sixty-seven extracted dog premolar roots were randomly assigned to one of the following groups: groups 1–3 filled with gutta-percha and replanted immediately and after 40 and 60 minutes, respectively; groups 4 and 5 filled with 0.05% clobetasol; and groups 6 and 7 filled with 0.05 % fluocinonide. Groups 4 and 6 were replanted after 40 minutes and groups 5 and 7 after 60 minutes. After 4 months, roots were evaluated histologically for signs of periodontal healing. Roots treated with clobetasol and fluocinonide healed more favorably than roots filled with gutta-percha and were different from each other at 6...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

In this report, we describe
the case of a prepubertal girl presenting with a junctional melanocytic nevus of the labia minora superimposed by lichen sclerosus.
The lesion was completely excised and free margins confirmed by histological examination. After a 3 months course of topical
clobetasol treatment the girl remained asymptomatic. No local recurrence was notified with a follow-up time of 12 months.
